Description
Изобретение относится к медицине, в частности к урологии, а именно к способу илеоцистоанастомоза при антирефлюксной операции.The invention relates to medicine, in particular to urology, namely to a method of ileocystoanastomosis during antireflux surgery.
При кишечной пластике верхних мочевыводящих путей используют сегменты подвздошной кишки различной длины. Показаниями к кишечной пластике мочеточников являются их стриктура или облитерация длиной более 12 см, включая тотальное поражение. При замещении мочеточника в средней и нижней трети проксимально формируют уретероилеоанастомоз (кишечный трансплантат с мочеточником). При тотальной кишечной пластике мочеточника проксимально формируют анастомоз трансплантата с лоханкой почки (Пиелоилеоанастомоз), либо с почечной чашкой (Каликоилеоанастомоз). Во всех случаях илеоуретеропластики дистальный анастомоз формируется между кишечным трансплантатом и стенкой мочевого пузыря. При наложении такого анастомоза по стандартной методике происходит сшивание дистального конца кишечного трансплантата со стенкой мочевого пузыря (конец в бок). В шов включены все слои стенки мочевого пузыря и кишечного трансплантата. Учитывая большой диаметр кишечной трубки (1-1.5см), при микции зачастую происходит обратный ток содержимого мочевого пузыря в кишечный трансплантат (рефлюкс). При этом возникает чувство боли в поясничной области соответствующей стороны, повышение температуры тела. В анамнезе у пациента нередко встречаются рецидивирующие пиелонефриты, инструментально выявляется нарушение уродинамики верхних мочевыводящих путей. В совокупности все факторы могут привести к прогрессированию почечной недостаточности, нефросклерозу и/или терминальному гидронефрозу [1, 2, 3].In intestinal plastic surgery of the upper urinary tract, segments of the ileum of various lengths are used. The suture includes all layers of the bladder wall and intestinal graft. Given the large diameter of the intestinal tube (1-1.5 cm), during miction, a backflow of bladder contents into the intestinal graft (reflux) often occurs. In this case, there is a feeling of pain in the lumbar region of the corresponding side, and an increase in body temperature. The patient's medical history often includes recurrent pyelonephritis; disturbances in the urodynamics of the upper urinary tract are detected instrumentally. Taken together, all factors can lead to the progression of renal failure, nephrosclerosis and/or terminal hydronephrosis [1, 2, 3].
Наиболее близким к заявляемому способу является способ хирургического лечения протяженного сужения мочеточника, захватывающего его прилоханочный отдел с выраженным рубцово-спаечным процессом в области лоханки и почечных сосудов [4]. В ходе осуществления данного способа выполняют люмботомию. Рубцово-измененный мочеточник отсекают от лоханки и резецируют. Рану в поясничной области оставляют открытой. Из лапаротомного (лапароскопического) доступа формируют трансплантат на брыжейке из сегмента тонкой кишки длиной 20-25 см. Сформированный трансплантат располагают изоперистальтически, анастомозируют его дистальный конец с мочевым пузырем («конец в бок»). Лапаротомную рану ушивают. Проксимальный конец трансплантата через люмботомическую рану анастомозируют с лоханкой/чашкой. Недостатком данного способа является рефлюкс мочи из мочевого пузыря в кишечный трансплантат при микции, что вызывает болевой синдром, задержку мочи в кишечном трансплантате, электролитные нарушения (ацидоз), рецидивы пиелонефрита, в тяжелой форме приводящие к уросеспсису. Со временем частый рефлюкс может привести к нарушению уродинамики верхних мочевых путей, которое влечет за собой почечную недостаточность, нефросклероз, терминальный гидронефроз.The disadvantage of this method is the reflux of urine from the bladder into the intestinal graft during miction, which causes pain, urinary retention in the intestinal graft, electrolyte disturbances (acidosis), relapses of pyelonephritis, which in severe form lead to urosepsis. Технический результат достигается тем, что в способе илеоцистоанастомоза с антирефлюксной методикой, в ходе которого обеспечивают доступ в брюшную полость , рецезируют рубцово-измененный мочеточник, отступя 20-25 см от илеоцекального угла рецезируют участок подвздошной кишки на брыжейке, причем, согласно изобретению, на дистальном конце кишечного трансплантата 1,5-2 см кишки отсекают от брыжейки, формируя свободный конец, затем образуют илеоцистоанастомоз с погружением свободного конца трансплантата в полость мочевого пузыря.The technical result is achieved by the fact that in the method of ileocystoanastomosis with an antireflux technique, during which access to the abdominal cavity is provided , the scarred ureter is resected, at a distance of 20-25 cm from the ileocecal angle, a section of the ileum is resected on the mesentery, and, according to the invention, on the distal At the end of the intestinal graft, 1.5-2 cm of intestine is cut off from the mesentery, forming a free end, then an ileocystoanastomosis is formed with the free end of the graft immersed in the cavity of the bladder.

Первоночально формирование илеоцистоанастомоза выполняют в положение пациента лежа на спине под общей анестезией (лапаротомическим/ лапароскопическим ) доступом. Производят резекцию рубцово-измененного мочеточника. Далее формируют трансплантат из сегмента тонкой кишки. Причем для формирования трансплантата, отступя 20-25 см от илеоцекального угла, резецируют участок подвздошной кишки на брыжейке. Дистальный конец кишечного трансплантата на 1,5-2 см скелетируют от брыжейки. Затем полученный трансплантат располагают изоперисталически после чего проксимально формируют пиелоилеоанастомоз/ каликоилеоанастомоз/ уретероилеоанастомоз, а дистально илеоцистоанастомоз формируют следующим образом.Initially, the formation of ileocystoanastomosis is performed with the patient lying on his back under general anesthesia (laparotomy/laparoscopic) access. По окружности тонкокишечного трансплантата, включая все слои мочевого пузыря, накладывают непрерывный двурядный шов (викрил 2,0) по схеме. Первый ряд двумя нитями (викрил 2,0): стенка кишечного трансплантата — серозно-мышечный шов, стенка мочевого пузыря — изнутри кнаружи. По окружности кишечного трансплантата начиная с 6 до 12 часов условного циферблата. по часовой стрелке, далее с 6 до 12 часов условного циферблата против часовой стрелки. В ходе операции обеспечили доступ в забрюшинное пространство и брюшную полость (люмболапаротомия), рецезировали рубцово-измененный мочеточник справа, сформировали трансплантат из сегмента тонкой кишки и расположили его изоперисталически, после чего анастомозировали проксимальный (пиелоилеоанастамоз) и дистальный (илеоцистоанастамоз) концы трансплантата, затем ушили рану.
