Хроническое критическое состояние (ХКС) представляет собой продленную полиорганную недостаточность со сменяющимся преобладающим синдромом недостаточности жизненных функций. Данный термин объединяет группу пациентов, переживших первичное воздействие пускового этиологического фактора критического состояния и оставшихся длительно зависимыми от методов интенсивной терапии (Методические рекомендации «Реабилитация в отделении реанимации и интенсивной терапии (РеабИТ)» 2021 г.). Распространенность ХКС колеблется от 5 до 20% среди пациентов ОРИТ. Пациенты с ХКС имеют иммуносупрессивный статус, который отражается в сниженном уровне HLA-DR и ALC, на фоне более высоких значений sPD-L1 и IL-10. В настоящее время в Российской Федерации известны клинические рекомендации «Профилактика инфекций области хирургического вмешательства» Министерства здравоохранения Российской Федерации и «Национальной ассоциации специалистов по контролю инфекций, связанных с оказанием медицинской помощи» («НАСКИ»), утвержденные в 2022 году. Эти рекомендации регламентирую комплекс мер профилактики инфекционных осложнений после хирургического вмешательства (ИОХВ), включающий в себя также периоперационную антибиотикопрофилактику. Согласно указанным рекомендациям, при проведении периоперационной антибиотикопрофилактики (ПАП) ИОХВ препаратом выбора являются Цефотаксим или Цефуроксим для чистых плановых и экстренных хирургических вмешательств. Рекомендовано использование при аллергии на бета-лактамы клиндамицина или ванкомицина (последнего - в случае высокой частоты MRSA-инфекции в учреждении). Рекомендована доза антибиотика, используемая при проведении ПАП, соответствующая разовой терапевтической дозе данного препарата. Рекомендовано вводить антибиотик для ПАП внутривенно, что позволяет быстро и предсказуемо обеспечить его необходимую тканевую концентрацию к моменту начала операции. Рекомендовано оптимальное время для введения предоперационной дозы - за 30-60 минут до начала хирургического вмешательства, после чего к моменту разреза достигается эффективная тканевая концентрация антибиотика. Рекомендовано для некоторых препаратов (фторхинолоны, ванкомицин) более раннее введение для достижения оптимальных тканевых концентраций к моменту кожного разреза; их введение должно быть осуществлено за 120 минут до начала хирургического вмешательства. Риск инфекционных осложнений после операции у нейрохирургических пациентов в ХКС, обусловленном повреждением головного мозга больше, чем у остальных категорий пациентов, несмотря на проведение антибиотикопрофилактики согласно клиническим рекомендациям. Так как абсолютное большинство пациентов с ХКС нуждается в протекции дыхательных путей посредством трахеостомии и длительно пребывает в отделении реанимации, происходит процесс обсеменения дыхательных путей патологической реанимационной флорой даже без развития клинической пневмонии. При кашле пациента и санации патологическая реанимационная флора выделяется в окружающую пациента среду. Также из дыхательных путей при определенных условиях, например, в условиях стресса, которым является нейрохирургическая операция, может происходить гематогенный заброс патологической флоры дыхательных путей в область операции. В то же время по причине устойчивости реанимационной флоры к антибиотикам и образования биологических пленок некоторыми бактериями, не всегда возможно вывести патологическую реанимационную флору из дыхательных путей перед плановой операцией у данной категории пациентов путем применения только антибактериальной терапии. При этом, согласно стандартному протоколу периоперационной антибиотикопрофилактики, для пациентов реанимационного профиля за 120 минут до разреза пациенту вводят 1 г Ванкомицина внутривенно. Если оперативное вмешательство длится более 8 часов, через 8 часов от начала операции пациенту дополнительно вводят внутривенно 1 г. Через 23 дня после поступления в ОРИТ ФНКЦ РР пациенту была проведена операция «Пластика дефекта свода черепа в левой лобно-теменно-височной области материалом Palacos MV+G с применением технологии 3D печати». В периоперационном периоде в соответствии со стандартным протоколом была проведена периоперационная антибиотикопрофилактика, включающая введение 1 г. Ванкомицина внутривенно капельно в разведении на 250.0 NaCl 0,9% за 120 минут до разреза. Дополнительно в течение 2 дней до операции пациенту ингаляционно посредством аппарата ИВЛ было введено по 1 дозе БКБ в концентрации по Грация 1×104-9,9×107 БОЕ/мл, вспомогательного вещества в форме физиологического раствора до 5 мл, 2 раза в день. После проведенной операции в течение 7 дней пациент продолжал получать БКБ по 1 дозе 2 раза в сутки ингаляционно. Лечебно-реабилитационные мероприятия в ФНКЦ РР были продолжены в течение 35 дней. Рана зажила первичным натяжением, инфекционных осложнений в послеоперационном периоде зафиксировано не было. Швы были сняты на 7-е сутки после операции. 21.03.2023 поступил для проведения лечебно-реабилитационных мероприятий в ОРИТ. 28.04.2023. в связи с грубым резистентным к консервативной терапии спастическим синдромом пациенту была проведена операция «Дорзальная селективная ризотомия с нейрофизиологическим мониторингом». В периоперационном периоде в соответствии со стандартным протоколом пациент получал периоперационную антибиотикопрофилактику, включающую введение 1 г. Ванкомицина внутривенно капельно в разведении на 250.0 NaCl 0,9% за 120 минут до разреза. Дополнительно в течение 2 дней до операции пациенту ингаляционно посредством небулайзера было введено по 1 дозе 3 раза в день БКБ в концентрации по Грация 1×104-9,9×107 БОЕ/мл, вспомогательного вещества в форме физиологического раствора до 5 мл, а также 7 дней после оперативного вмешательства. После проведенной операции пациент продолжал лечебно-реабилитационные мероприятия в ФНКЦ РР в течение 25 дней. Рана зажила первичным натяжением, инфекционных осложнений в области оперативного вмешательства в послеоперационном периоде зафиксировано не было. Швы были сняты на 7 сутки после операции.
