Изобретение относится к области медицины, кардиологии и сердечно-сосудистой хирургии и может быть использовано для прогнозирования риска развития аортального стеноза (АС) у пациентов с врожденным пороком сердца: бикуспидальный аортальный клапан (ВПС:БАК) и дилатацией восходящего отдела аорты (ДВОА).The invention relates to the field of medicine, cardiology and cardiovascular surgery and can be used to predict the risk of developing aortic stenosis (AS) in patients with congenital heart disease: bicuspid aortic valve (ACV:BAV) and dilatation of the ascending aorta (ADOA).
Известен способ оценки кальциноза аортального клапана при помощи ультразвуковой денситометрии (патент РФ №2306104, опубл. 20.09.2007 г.), включающий выполнение ультразвукового исследования сердца на диагностической системе Nemio 35 (TOSHIBA) для визуализации используют стандартную кардиальную программу (IHeartA), кардиальный датчик - фазированную решетку и стандартные заводские настройки получения изображения: уровень Gain-80, частоту инсонации датчика - 2,5 МГц без применения 2-й гармоники; при этом изображение аортального клапана выводится из парастернальной позиции датчика с сечением по короткой оси, аортальный клапан оконтурируется включая клапанное кольцо по методу эллипса и после проведения ультразвуковой денситометрии с использованием программы 2Б-гистограммы оценивается значение показателя Mean.There is a known method for assessing calcification of the aortic valve using ultrasound densitometry (RF patent No. 2306104, published on September 20, 2007), which includes performing an ultrasound examination of the heart on the Nemio 35 diagnostic system (TOSHIBA) for visualization using a standard cardiac program (IHeartA), a cardiac sensor - phased array and standard factory settings for image acquisition: Gain-80 level, sensor insonation frequency - 2.5 MHz without using the 2nd harmonic; in this case, the image of the aortic valve is displayed from the parasternal position of the sensor with a cross-section along the short axis, the aortic valve is contoured including the valve ring using the ellipse method, and after ultrasonic densitometry using the 2B-histogram program, the value of the Mean indicator is estimated.

Для прогноза развития АС у пациентов с ВПС:БАК и ДВОА были построены логистические уравнения, при построении которых использовались количественные и качественные предикторы в различных сочетаниях, с пошаговым алгоритмом включения, позволивший получить несколько десятков уравнений логистической регрессии, из которых производился отбор уравнений, имеющего самые высокие значения верного предсказания-более 70%. Уровень значимости для включения предикторов в уравнение регрессии задавался таким, чтобы достигнутый уровень значимости критерия Вальда Хи-квадрат для каждого предиктора по окончании пошаговой процедуры не превышал 10%. Пошаговый порядок (Step) включения отобранных предикторов (Variable) в уравнение с указанием процента верного предсказания на каждом шаге и коэффициентов регрессии отражен в таблице 1, по которой можно проследить динамику предсказательной ценности предикторов и их совокупности при оценке уравнения логит-регрессии в целом. Соответствие модели и реальных данных оценивалось с использованием теста согласия Хосмера-Лемешова (Hosmer and Lemeshov Goodness-of-Fit Test). Для активного применения предлагаемой математической модели в практике нами был создан вероятностный калькулятор развития аортального стеноза у пациентов с врожденным пороком сердца: бикуспидальным аортальным клапаном и дилатацией восходящего отдела аорты с использованием стандартного табличного редактора MS Excel включенным в пакет программы MS Office 2007позволяющей интегрировать указанное выше уравнение и вносить в данный калькулятор индивидуальные числовые характеристики пациента по указанным выше критериям прогноза. Пример 1. Пациент М. 50 лет проходил стационарное лечение в кардиологическом отделении ФГБУ НМИЦ им В.А. Алмазова с диагнозом: ВПС. Бикуспидальный АК. Гипертоническая болезнь II ст., риск ССО 3. Помимо стандартного общеклинического обследования пациенту М. был проведен анализ лабораторных показателей: уровень креатинина составил 78 мкмоль/л, уровень холестерина 5.5 ммоль/л, уровень sRANKL 0.59 пмоль/л, уровень остеопротегерина 8.1 пмоль/л, также было выполнено эхокардио графическое исследование: выявлен бикуспидальный АК, аортальная недостаточность 1 степени, размер восходящей аорты составил 44 мм, скорость на АК составила 1.7 м/с. С помощью предлагаемой математической модели прогнозирования встроенной в программу MS Excel, 2007 был проведен расчет вероятности развития АС у данного пациента, риск развития составил 94.3%, что является высокой градацией риска. В дальнейшем пациент ежегодно наблюдался и обследовался в ФГБУ НМИЦ им В.А. Алмазова, показаний к оперативному лечению до настоящего времени выявлено не было. Через 5 лет при выполнении эхокардиографического исследования был выявлен легкий АС, скорость на АК составила 2.4 м/с.Example 1.
