Изобретение относится к области медицины, а именно к эндокринологии, и представляет собой способ процентного определения риска развития диабетической ретинопатии у больного сахарным диабетом 2-го типа (СД 2-го типа).The invention relates to the field of medicine, namely to endocrinology, and is a method for determining the percentage risk of developing diabetic retinopathy in a patient with type 2 diabetes mellitus (type 2 diabetes).
СД занимает важное место среди заболеваний, темпы роста которых приобретают характер неинфекционных эпидемий, вследствие значительной их распространенности, снижения качества жизни, высокой смертности и инвалидности пациентов. Распространенность СД в мире по данным IDF составляет около 8% [IDF Diabetes Atlas, 9th edition. Brussels: International Diabetes Federation; 2019; Available from: https://www.diabetesatlas.org/en]. Среди поздних микрососудистых осложнений СД - диабетическая ретинопатия (ДР), которая является основной причиной слепоты среди лиц трудоспособного возраста и третьим по частоте фактором снижения зрения у лиц старше 65 лет. Распространенность ДР среди больных с установленным СД варьирует от 10 до 61% [Нероев В.В., Зайцева О.В., Михайлова Л.А. Among the late microvascular complications of diabetes is diabetic retinopathy (DR), which is the leading cause of blindness among people of working age and the third most common cause of vision loss in people over 65 years of age. The prevalence of DR among patients with established diabetes varies from 10 to 61% [Neroev V.V., Zaitseva O.V., Mikhailova L.A. Известен способ прогнозирования риска раннего развития микрососудистых осложнений у детей с СД 1 типа (патент РФ №2557928 от 27.07.2015), при котором определяют длительность заболевания, возраст больного, содержание десквамированных эндотелиальных клеток, уровень холестерина липопротеидов высокой плотности, общего холестерина, триглицеридов, коэффициент атерогенности, уровень гликированного гемоглобина, среднесуточный уровень гликемии. Изобретение обеспечивает прогнозирование высокого риска развития микрососудистых осложнений в течение года (коэффициента ≥1) или низкого уровня развития микрососудистых осложнений (при значении коэффициента <1). Недостатком метода является отсутствие указаний на детализацию осложнений у больных СД 1-го типа.There is a known method for predicting the risk of early development of microvascular complications in children with type 1 diabetes (RF patent No. 2557928 dated July 27, 2015), in which the duration of the disease, the patient’s age, the content of desquamated endothelial cells, the level of high-density lipoprotein cholesterol, total cholesterol, triglycerides, atherogenicity coefficient, glycated hemoglobin level, average daily glycemic level. The invention provides prediction of a high risk of developing microvascular complications during the year (coefficient ≥1) or a low level of development of microvascular complications (with a coefficient value <1). The disadvantage of the method is the lack of instructions for detailing complications in patients with type 1 diabetes.
Известен способ прогнозирования формирования осложнений сахарного диабета (патент №2668202 от 29.09.17) основанный на исследовании показателей микроциркуляторного русла у лиц с СД методом компьютерной капилляроскопии ногтевого ложа. К недостаткам способа относят - трудоемкость и техническую сложность, так как необходимо иметь специальное программно-аппаратное обеспечение, что ограничивает его применение в клинической практике.There is a known method for predicting the formation of complications of diabetes mellitus (patent No. 2668202 dated September 29, 2017) based on the study of microcirculatory parameters in people with diabetes using computer capillaroscopy of the nail bed. Способ прогнозирования риска развития диабетической ретинопатии у больных СД 2-го типа разработан на основании результатов логистической регрессионной модели на базе Microsoft Office Excel. С помощью математического моделирования, логистической регрессии, корреляционного анализа установлены наиболее значимые факторы риска, ассоциированные диабетической ретинопатией у больных с СД 2-го и вычислен процентный риск развития диабетической ретинопатии.A method for predicting the risk of developing diabetic retinopathy in patients with type 2 diabetes was developed based on the results of a logistic regression model based on Microsoft Office Excel. Using mathematical modeling, logistic regression, and correlation analysis, the most significant risk factors associated with diabetic retinopathy in patients with type 2 diabetes were established and the percentage risk of developing diabetic retinopathy was calculated.

Указанный результат достигается с помощью определения у больных с СД 2-го типа анамнестических (возраст, длительность сахарного диабета 2-го типа) и лабораторных (уровень постпрандиальной гликемии) показателей, с последующим их использованием для расчета прогнозируемого риска развития диабетической ретинопатии у больных СД 2-го типа.This result is achieved by determining anamnestic (age, duration of type 2 diabetes) and laboratory (postprandial glycemia level) parameters in patients with type 2 diabetes, with their subsequent use to calculate the predicted risk of developing diabetic retinopathy in patients with type 2 diabetes -th type.

При разработке способа на первом этапе была проведена оценка клинико-анамнестических данных у 121 пациента с СД 2-го типа. На втором этапе с помощью корреляционного анализа выделены наиболее значимые факторы, достоверно ассоциированные с диабетической ретинопатией. На последнем этапе с помощью метода логистической регрессии определены прогностически значимые факторы риска развития диабетической ретинопатии у больных СД 2-го типа.When developing the method, at the first stage, clinical and anamnestic data were assessed in 121 patients with type 2 diabetes. Интегральный показатель имеет более высокую степень эффективности 73,55% (чувствительность 75,38%, специфичность 71,43%) по сравнению с отдельными показателями.
