Abstract
Description
The invention relates to medicine, namely to gynecology, and can be used for the complex treatment of menstrual disorders such as oligomenorrhea and amenorrhea in women of reproductive age with metabolic syndrome.
Хотя точная этиология остается неясной, известно, что он характеризуется рядом гормональных нарушений, включая гиперандрогению, инсулинорезистентность и гиперинсулинемию (Rojas J, Chávez М, Olivar L, Rojas M, Morillo J, Mejías J, Calvo M, Bermúdez V. Polycystic ovary syndrome, insulin resistance, and obesity: navigating the pathophysiologic labyrinth. Int J Reprod Med. 2014;2014:719050. doi: 10.1155/2014/719050. Epub 2014 Jan 28. PMID: 25763405; PMCID: PMC4334071.) Today, 10-20% of patients in the world suffer from anovulatory infertility, and they are more likely to have obesity, metabolic disorders and symptoms of hyperandrogenism, as well as morphological signs of polycystic ovaries compared to women with a regular menstrual cycle. Polycystic ovary syndrome is a common endocrine-metabolic disorder that has a variety of serious consequences for women's health, including alarming rates of infertility. Although the exact etiology remains unclear, it is known to be characterized by a number of hormonal disorders, including hyperandrogenism, insulin resistance and hyperinsulinemia (Rojas J, Chávez M, Olivar L, Rojas M, Morillo J, Mejías J, Calvo M, Bermúdez V. Polycystic ovary syndrome, resistance, and obesity: navigating the pathophysiologic labyrinth. Int J Reprod Med. 2014;2014:719050. doi: 10.1155/2014/719050. Epub 2014 Jan 28. PMID: 25763405; PMCID: PMC4334071.) The key element of the metabolic syndrome is the abdomen minal obesity with the subsequent development of insulin resistance in individuals with a genetic predisposition to the disease and hyperinsulinemia, which leads to increased ovarian androgen synthesis and the development of hyperandrogenism. The prevalence of insulin resistance among patients with ovarian dysfunction is extremely high - according to some authors, it is observed in 90-100% of obese patients and in 26-41% of normal weight patients.
(Frisch RE. The right weight: body fat, menarche and fertility. Proc Nutr Soc. 1994 Mar; 53(l):113-29. doi: 10.1079/pns19940015. PMID: 8029220).Ovulatory dysfunction occurs in obese women due to dysregulation of the hypothalamic-pituitary-ovarian axis. Obesity alters egg quality and endometrial receptivity due to excess free fatty acids and adipokines. Adipose tissue can be a source of estrogens by aromatizing androgens, can reduce the ability of estrogens to bind to sex hormone-binding globulin and accumulate steroid hormones (Frisch RE. The right weight: body fat, menarche and fertility. Proc Nutr Soc. 1994 Mar; 53( l):113-29. doi: 10.1079/pns19940015. PMID: 8029220).
(MantziosM, WilsonJC. Mindfulness, eating behaviours and obesity: a review and reflection on current findings. Curr.Obes.Rep.2015; 4(1): 141-6).However, the issues of etiology and pathogenesis of menstrual and reproductive dysfunctions in obesity and metabolic syndrome are still far from being fully understood, as well as treatment approaches, among which there are two areas - lifestyle modification and drug therapy, including the use of insulin sensitizers (metformin), and /or combined estrogen-progestin drugs (oral contraceptives with antiandrogenic action), each of which has certain disadvantages and limited effectiveness (MantziosM, WilsonJC. Mindfulness, eating behaviors and obesity: a review and reflection on current findings. Curr.Obes.Rep. 2015;4(1):141-6).
The methods of drug therapy used do not always have a comprehensive pathogenetic approach, do not contribute to faster and more intense weight loss, retention of achieved results and prevention of relapse, and the fear of worsening metabolic disorders while taking hormonal drugs is not dispelled.
Metformin, traditionally used to correct insulin resistance, has a positive effect on carbohydrate metabolism, but does not affect menstrual function and sometimes leads to such negative phenomena as lactic acidosis, decreased levels of testosterone, thyroid-stimulating hormone, deficiency of B vitamins, which may limit its use in the fight against metabolic diseases. violations. One of the serious complications that limit the use of estrogen-progestin drugs is a negative effect on hemostatic parameters of the blood, and they also have a toxic effect on the liver.
Prototype
В качестве прототипа нами выбран «Способ лечения синдрома поликистозных яичников, ассоциированного с метаболическим синдромом» (патент №2749783).As a prototype, we chose “Method for the treatment of polycystic ovary syndrome associated with metabolic syndrome” (patent No. 2749783).
The prototype method is as follows: the following medications are prescribed orally: myo-inositol at a dose of 2000-3000 mg per day, D-chiro-inositol at a dose of 400-600 mg per day in combination with oral administration of Sulodexide at a dose of 500-750 lipoprotein lipase units per day for at least 6 months.
Недостатком данного способа восстановления менструальной функции у женщин с поликистозными яичниками и метаболическим синдромом является ограниченность применения одного из компонентов лечебного комплекса (Сулодексид) во время беременности (I триместр), в случае ее наступления во время лечения, и ряд нежелательных побочных эффектов со стороны желудочно-кишечного тракта (рвота, тошнота, боль в эпигастрии).The disadvantage of this method of restoring menstrual function in women with polycystic ovaries and metabolic syndrome is the limited use of one of the components of the treatment complex (Sulodexide) during pregnancy (first trimester), if it occurs during treatment, and a number of undesirable side effects from the gastrointestinal tract. intestinal tract (vomiting, nausea, epigastric pain).
Purpose of the invention
The purpose of the invention is to increase the effectiveness of treatment of menstrual disorders such as oligomenorrhea and amenorrhea in women of reproductive age with metabolic syndrome. Achieving this goal will lead to a decrease in the frequency of menstrual irregularities, improvement of hormonal status, carbohydrate and lipid metabolism, will reduce the frequency of anovulatory infertility, and will also help reduce oxidative stress.
The essence of the invention
The declared technical result is achieved by using a combination of two isomers of inositol (myo- and D-chiro-inositol) and the dietary supplement coenzyme Q10. Patients are prescribed oral myo-inositol at a dose of 2000 mg 2 times a day and D-chiro-inositol at a dose of 400 mg 2 times a day from the 1st day of the menstrual cycle, if any, or on any day for amenorrhea, and additionally prescribed coenzyme -Q10 at a dose of 100 mg 2 times a day, for at least 6 months.
As the results show (Table 1), the combined use of coenzyme Q10 and inositol has a pronounced positive synergistic effect on menstrual function and metabolic profile; their combined use is more effective than the use of the components of the invention separately.
Inositols and their phosphates function as second messenger molecules for various cellular signaling pathways, including insulin signaling, calcium transport, lipid metabolism, cytoskeletal protein assembly, cell growth and differentiation, modulation of serotonergic pathways, and involvement in oocyte maturation.
Mitochondria are the main site of lipid degradation. Cells protect themselves from lipotoxicity and death by either oxidizing free fatty acids or sequestering them into triglycerides in the form of lipid droplets. It is well known that mitochondrial function is required for normal glucose-stimulated insulin secretion from pancreatic β-cells. Mitochondrial oxidative activity and ATP synthesis are reduced, these data support the hypothesis that insulin resistance in humans arises from defects in mitochondrial fatty acid oxidation, which in turn leads to an increase in intracellular fat metabolites.
Balanced energy production and reduced oxidative stress can be achieved by supplementing coenzyme Q10 (CoQ10, ubiquinone), an electron transporter in the inner mitochondrial membrane. Its physiological function is to act as an important cofactor for ATP production and to perform important antioxidant functions in the body.
(CoQH2).Coenzyme Q10 is mainly found in the inner membrane of mitochondria, and is involved in the transport of electrons along the respiratory chain and thus participates in the synthesis of ATP. It is known that mitochondria are the main source of reactive oxygen species - oxygen radicals. With a lack of coenzyme Q10, when its quantity is not enough for the transport of electrons, they begin to go into the intermembrane space and contribute to the formation of reactive oxygen species. Therefore, mitochondria, on the one hand, are the main generator of energy, ATP, and on the other hand, the main source of reactive oxygen species in case of the slightest disturbance, the synthesis of coenzyme Q10. Coenzyme Q10 has a powerful antioxidant effect. Unlike other antioxidants, ubiquinone is regenerated by the body's enzyme system, converting to a reduced form - ubiquinol (CoQH2).
Coenzyme Q10 not only comes from food, but is also synthesized by the body in all cells. Synthesis is carried out from mevalonic acid, and metabolic products of tyrosine and phenylalanine with the participation of vitamins B2, B3, B6, B12, C, folic and pantothenic acids, as well as a number of trace elements. This is a complex, multi-step process regulated by several enzyme systems. With a deficiency of vitamins and microelements, with violations of regulatory enzyme systems, and even outside of any pathology, the endogenous biosynthesis of Q10 does not meet the body's needs. Physical activity, stress, alcohol consumption in large quantities, smoking can also be causes of Q10 deficiency, and endogenous Q10 synthesis is suppressed by statins - drugs that reduce cholesterol synthesis.
