В развитых странах, постоянно растет число людей перенесших операции по наложению кишечной стомы в связи с различными заболеваниями. Несмотря на высокий уровень хирургической реабилитации число стомированных пациентов в России около 150000 человек. Стомированные пациенты нуждаются в комплексной медико-социальной программе реабилитации. Общими принципами реабилитации являются индивидуальность, последовательность. Осложнения после операций с формированием стомы являются значительной проблемой для многих пациентов. Нагноение парастомальной раны способствует возникновению последующих осложнений, таких как ретракция колостомы, некроз стенки кишки, параколостомическим абсцессам, эвентрации других отделов кишечника, межкишечных абсцессов, деформации колоректального анастомоза. Разработанные способы и устройства в основном направлены на лечение осложнений колостомы, а не на их профилактику. Все это указывает на необходимость создания и совершенствования методов профилактики и лечения осложнений колостомы. Исследования, проведенные в разных странах показывают, что до 71% пациентов с илеостомой и 43% с колостомой имеют осложнения.In developed countries, the number of people undergoing intestinal stoma operations due to various diseases is constantly growing. The general principles of rehabilitation are individuality and consistency. Complications after stoma surgery are a significant problem for many patients. Suppuration of the parastomal wound contributes to the occurrence of subsequent complications, such as retraction of the colostomy, necrosis of the intestinal wall, paracolostomy abscesses, eventration of other parts of the intestine, interintestinal abscesses, and deformation of the colorectal anastomosis. The developed methods and devices are mainly aimed at treating complications of colostomy, and not at their prevention. All this indicates the need to create and improve methods for the prevention and treatment of colostomy complications. Studies conducted in different countries show that up to 71% of patients with ileostomy and 43% with colostomy have complications.

Недостатками этого метода являются то, что использование струи с низким выходным давлением не обеспечивает качественной очистки раны от гнойно-некротических тканей; механическая очистка производится только в поверхностных слоях раны, в то время как основная масса микробов, включающая анаэробную инфекцию, находится в глубине инфильтрата; имеется реальная опасность бактериального загрязнения операционной полости вследствие образования большого количества брызг, так как истечение струи жидкости под небольшим давлением для достижения желаемого эффекта по законам физики требует большего его количества, а так же недостатками старых традиционных методов лечения, так же как и большинства новых, являются недостаточная эффективность в отношении очищения раны от гнойно-некротических тканей и ликвидации очага инфекции, особенно если имеется хронический гнойный процесс(нагноение шовного материала). Кроме того, использование многих современных инструментальных средств связано с большими материальными затратами и сложностью технического обслуживания (Даценко Б.М. Белов С.Г. Тимм Т.И. Задачей изобретения является создание способа лечения осложнений, вызванных воспалением колостомы, обеспечивающего получение технического результата, состоящего в улучшении качества консервативного лечения, уменьшение срока заживления и снижения частоты побочных местных реакций.The objective of the invention is to create a method for treating complications caused by inflammation of a colostomy, providing a technical result consisting in improving the quality of conservative treatment, reducing the healing time and reducing the frequency of adverse local reactions.

Особенностью является то, что снимают нагноившийся шовный материал или трубку для фиксации стомы, рану осушают салфетками, на пораженные участки вокруг стомы распыляют смесь порошков Целоформа и Стрептоцида в равных пропорциях, а поверх накладывают калоприемник, перевязки проводят один раз в 3 дня до заживления раны.A special feature is that the festering suture material or tube for fixing the stoma is removed, the wound is dried with napkins, a mixture of Celoform and Streptocide powders in equal proportions is sprayed onto the affected areas around the stoma, and a colostomy bag is placed on top, dressings are carried out once every 3 days until the wound heals.

Способ осуществляется следующим образом.The method is carried out as follows.
После снятия нагноившегося шовного материала или трубки для фиксации стомы пораженные участки промывают антисептическим раствором. Затем пораженные участки промокают салфетками до полной по возможности сухости в ране. Готовят смесь порошков, состоящую из препарата «Целоформ», в состав которого входит модифицированная хлопковая целлюлоза, и препарата «Стрептоцид» (сульфаниламид). Препараты смешиваются между собой 1:1. Смесь порошков распределяется вокруг стомы на пораженные участки, а поверх приклеивают калоприемник.After removing the festering suture material or tube to fix the stoma, the affected areas are washed with an antiseptic solution. Для оценки эффективности проведено исследование, в которое были включены 20 пациентов с параколостомическими свищами. У всех больных отмечали высокую эффективность заживления, прекращалось выделение свободного гноя. Часть турунды остаются практически сухими, поскольку раневое отделяемое впитывается препаратом на турунде, что позволяет калоприемнику не отклеиваться и не создает неудобства пациенту.To evaluate the effectiveness, a study was conducted that included 20 patients with paracolostomy fistulas. Пациент И. 65 лет. Выполнена экстренная операция выведение одноствольной колостомы по поводу острой непроходимости кишечника. После выписки пациента из стационара, пациент в домашних условиях не смог самостоятельно справиться с техникой обработки колостомы. Спустя две недели у пациента появился кожный дерматит за счет химического воздействия кишечного отделяемого. Возник зуд, жжение. При осмотре пациента было установлено, наличие покраснения и сочащихся язв вокруг стомы. После производилась обработка кожных инфицированных покровов антисептическим раствором, затем накладывалась смесь порошков Целоформа и Стрептоцида в равных пропорциях, на которую в дальнейшем клеился калоприемник на трое суток. Перевязки длились на протяжении двух недель до полного заживления.Patient I. 65 years old. An emergency operation was performed to remove a single-barrel colostomy for acute intestinal obstruction. Пациент К. 56 лет обратился в кабинет реабилитации стомированных больных. При осмотре у него имеется одноствольная сигмостома с несколькими паростомальными осложнениями. Имеется частичная ретракция колостомы, присутствуют нити из нерассасывающего шовного материала которые не были вовремя сняты. Вокруг лигатур начали формироваться свищевые ходы и микроабсцессы. За счет частичной ретракции стомы, вызванной тем, что ее наложение проводилось подшивание краев апоневроза и кожи, вокруг нее сформировалась хроническая рана, заполненная гипергрануляциями. Уход за такой стомой и ношение калоприемника становится невозможным. Были сняты швы, удалены гипергрануляции и гипертрофированные участки кожи, перевязки проводились один раз в 3 дня с препаратами Целоформ и Стрептоцид. На фоне лечения свищевые ходы стали заживать, микроабсцессы дренированны. За счет устранения гипергрануляций с краев раны началась эпителизация дефекта кожи. На 15 день после лечения дефект кожи полностью эпителизировался, кишка стала принимать вид «столбика», но свежие рубцы пока еще плотные темно красного цвета. На 26 день кожа полностью зажила, рубцы созрели и стали эластичными, приняли светлую окраску, кишка полностью приняла вид «столбика».Patient K., 56 years old, came to the rehabilitation office for ostomy patients. The sutures were removed, hypergranulations and hypertrophied areas of the skin were removed, dressings were carried out once every 3 days with the drugs Celoform and Streptocide. During treatment, the fistula tracts began to heal, and microabscesses were drained. By eliminating hypergranulations from the edges of the wound, epithelization of the skin defect began. On the 15th day after treatment, the skin defect was completely epithelialized, the intestine began to take the form of a “column”, but fresh scars are still dense, dark red in color. On the 26th day, the skin had completely healed, the scars had matured and become elastic, they had taken on a light color, and the intestine had completely taken on the appearance of a “column.”
