Тромбозы глубоких вен (ТГВ) нижних конечностей остаются одними из важнейших проблем современной флебологии и сосудистой хирургии. Согласно данным Росстата, частота встречаемости ТГВ в Российской Федерации составляет 1,5-1,6 случаев на 1000 населения. Наиболее опасным осложнением ТГВ является тромбоэмболия легочной артерии, которая встречается с частотой 0,5-0,7 случаев на 1000 населения [1]. Следовательно, проведение качественной диагностики имеет важнейшее практическое значение.Deep vein thrombosis (DVT) of the lower extremities remains one of the most important problems of modern phlebology and vascular surgery. According to Rosstat, the incidence of DVT in the Russian Federation is 1.5-1.6 cases per 1000 population. The most dangerous complication of DVT is pulmonary embolism, which occurs with a frequency of 0.5-0.7 cases per 1000 population [1]. Therefore, high-quality diagnostics is of great practical importance.
Глубокая вена бедра (ГВБ) – основной приток бедренной вены, располагающийся на расстоянии 8–9 см ниже уровня паховой связки. Значение её в венозном возврате настолько велико, что при наличии флотирующих тромбов в бедренной вене, допустима резекция бедренной вены ниже уровня впадения глубокой вены бедра [2]. В то же время ГВБ сама может быть источником тромбов [4]. Согласно данным классической анатомии глубокая вена бедра представлена одним стволом [5]. Однако М.В. Вахитов и соавторы сообщают, что удвоение ГВБ может наблюдаться в 45,3 % случаев [6]. Р.Е. Калинин и соавторы сообщают, что два ствола ГВБ может встречаться в 80 % наблюдений, при этом один из стволов располагается на латеральной стороне от бедренной вены [7]. Кроме того, есть варианты развития с переходом подколенной вены и бедренной вены в ГВБ [8]. При этом в первом случае ГВБ отходит от подколенной вены и идет поверхностнее большой приводящей мышцы, во втором - ГВБ анастомозирует с бедренной веной сразу по выходу из приводящего канала, а сама бедренная вена гипоплазирована.The deep femoral vein (DFT) is the main tributary of the femoral vein, located 8–9 cm below the level of the inguinal ligament. Kalinin et al reported that two trunks of the HVB can occur in 80% of cases, with one of the trunks located on the lateral side of the femoral vein [7]. In addition, there are variants of development with the transition of the popliteal vein and femoral vein to the GVB [8]. At the same time, in the first case, the HVB departs from the popliteal vein and goes more superficially than the adductor great muscle, in the second case, the HVB anastomoses with the femoral vein immediately after exiting the adductor canal, and the femoral vein itself is hypoplastic.
Современным стандартом диагностики сосудистой системы нижних конечностей является ультразвуковое дуплексное ангиосканирование (УЗДАС) [9-11]. Известен способ исследования ГВБ с помощью дуплексного сканирования при классической анатомии ГВБ [Патент RU № 2751819 С1] [12]. При этом способе сначала исследование проводится в положении больного лёжа на спине, а линейный датчик в верхней трети бедра ставится по проекционной линии бедренных сосудов, и визуализируются устья глубоких бедренных. Далее используется конвексный датчик, в верхней трети бедра сканирование проводится по проекционной линии бедренных сосудов, а в средней и нижней трети бедра датчик проводится по линии, располагающейся на 2 см медиальнее проекционной линии бедренных сосудов, при этом сам датчик отклоняется кзади на 15°. Проводится компрессия вен, оценивается проходимость артерий. В положении больного лёжа на животе, конвексный датчик проводится по вертикальной линии расположенной на 3 см. кнаружи от задней срединной линии бедра.The modern standard for diagnosing the vascular system of the lower extremities is ultrasound duplex angioscanning (USDAS) [9-11]. With this method, the study is first carried out with the patient lying on his back, and a linear sensor in the upper third of the thigh is placed along the projection line of the femoral vessels, and the mouths of the deep femoral vessels are visualized. Next, a convex sensor is used, in the upper third of the thigh, scanning is carried out along the projection line of the femoral vessels, and in the middle and lower third of the thigh, the sensor is carried out along a line located 2 cm medially to the projection line of the femoral vessels, while the sensor itself deviates posteriorly by 15°. Compression of the veins is performed, the patency of the arteries is assessed. In the position of the patient lying on his stomach, the convex probe is carried out along a vertical line located 3 cm outward from the posterior midline of the thigh.

Исследование глубокой вены бедра проводится в положении больного лёжа на спине. Сначала линейный датчик ставится по проекционной линии бедренного сосудисто-нервного пучка в верхней трети бедра, визуализируется место впадения глубокой вены бедра в бедренную вену с латеральной стороны. Далее датчик проводится по линии, располагающейся под углом 90° к проекционной линии бедренного сосудисто-нервного пучка в латеральном направлении (фиг.1, 2) до пересечения с линией, идущей от передней верхней подвздошной ости к латеральному мыщелку бедра. Далее используется конвексный датчик, сканирование проводится по указанной линии, идущей от передней верхней подвздошной ости к латеральному мыщелку бедра, вниз (фиг.3). Затем снова используется линейный датчик, который ставится по проекционной линии бедренного сосудисто-нервного пучка в верхней трети бедра, визуализируется место впадения глубокой вены бедра в бедренную вену с медиальной стороны. Далее используется конвексный датчик, сканирование проводится вниз по проекционной линии бедренного сосудисто-нервного пучка, в средней и нижней трети бедра датчик проводится по линии, располагающейся на 2 см медиальнее проекционной линии бедренного сосудисто-нервного пучка. В нижней трети бедра, на 4 см выше медиального мыщелка бедра, датчик располагается под углом 120° к проекционной линии бедренного сосудисто-нервного пучка (фиг. 4, 5) Оценивается проходимость глубокой вены бедра. Далее больной переворачивается на живот. В положении лёжа на животе используется линейный датчик, который проводится вверх по вертикальной срединной линии, расположенной в подколенной области и на задней поверхности бедра, до уровня средней трети бедра (фиг.6, 7). Проксимальнее уровня средней трети бедра используется конвексный датчик, который проводится вверх. Оценивается проходимость глубокой вены бедра.The study of the deep vein of the thigh is carried out in the position of the patient lying on his back. Then a linear sensor is used again, which is placed along the projection line of the femoral neurovascular bundle in the upper third of the thigh, the place where the deep femoral vein flows into the femoral vein from the medial side is visualized. Next, a convex sensor is used, scanning is carried out down the projection line of the femoral neurovascular bundle, in the middle and lower third of the thigh, the sensor is carried out along a line located 2 cm medially to the projection line of the femoral neurovascular bundle. In the lower third of the thigh, 4 cm above the medial femoral condyle, the sensor is located at an angle of 120° to the projection line of the femoral neurovascular bundle (Fig. 4, 5). The patency of the deep femoral vein is assessed. Next, the patient turns over on his stomach. In the supine position, a linear sensor is used, which is carried up along the vertical midline located in the popliteal region and on the back of the thigh, to the level of the middle third of the thigh (Fig.6, 7). Proximal to the level of the middle third of the thigh, a convex probe is used, which is carried upwards. The patency of the deep femoral vein is assessed.
