Abstract
Description
The invention relates to medicine, namely to therapy.
Arterial hypertension is a multifactorial disease in which risk factors are equally common in both men and women, but their combination may differ. It is known that with increasing age, involutional processes occur in the body, comorbid pathology occurs, and all this can affect both the course of arterial hypertension and its prognosis. At the same time, in many cases, arterial hypertension is the cause of myocardial infarction, stroke, and renal failure. The risk of developing cardiovascular complications in arterial hypertension depends on the magnitude of blood pressure, risk factors, damage to target organs, the presence of associated conditions and concomitant comorbid pathology. Cardiovascular risks in arterial hypertension are well understood. Thus, the Framingham scale, the Score table, was created, according to which the risk of cardiovascular death within 10 years is estimated in patients without coronary heart disease, while taking into account gender, age, smoking, blood pressure and total blood cholesterol. At the same time, the risk is low (1%), medium (1-4%), high (5-9%) and very high (10% or more), etc. It is noted that the presence of several risk factors in a patient increases the risk of developing cardiovascular disease. -vascular complications, and the combination of hypertension, obesity, hypercholesterolemia and hyperglycemia is known as the "deadly quartet". Of interest is the qualitative and quantitative assessment of risk factors for stage III arterial hypertension, depending on gender.

The method consists in "determining the coagulation characteristics of the blood - activated partial thromboplastin time (APTT), prothrombin time (PT), thrombin time (TV) and the level of antithrombin III (ATIII), characterized in that the level of von Willebrand factor (EF), endothelin is additionally determined -1 (E-1) and D-dimers and with APTT ranging from 28 to 35 s, PT from 12 to 15 s, TT from 9 to 13 s, ATIII level from 29 to 70%, EF level from 105 to 115%, E-1 not more than 0.32 fmol / ml and D-dimer from 245 to 520 ng FEE / ml predict a slow progression of the course of CKD, and with the same values of APTT, PT, TT, ATIII level, and EF level more 115%, E-1 more than 0.32 fmol/ml, D-dimer more than 620 ng FEE/ml predict accelerated progression of chronic kidney disease.
This method has the following disadvantages:
- используются показатели, исследование которых не входит в программу предоставления медицинской услуги по обязательному медицинскому страхованию;- indicators are used, the study of which is not included in the program for the provision of medical services under compulsory health insurance;

Known (prototype) "Method for early diagnosis of progression of nephropathy in patients with arterial hypertension of the 1st degree" (RF patent for the invention No. 2632760 dated 09.10.2017). The method is carried out by "conducting a Doppler study, for which an ultrasonic sensor with an insonation frequency of 20 MHz is placed on the area of the nail fold of the third finger of the hand and a Dopplerogram is recorded, the linear velocity of blood flow is measured at rest and after a functional test, characterized in that as a functional test they use cold test, measure the maximum systolic velocity according to the average Vas velocity curve before and after the test, and if a paradoxical reaction of the microvasculature to the test is detected, manifested in a change in blood flow by more than 47%, the rapid progression of hypertensive nephropathy is determined.
However, this method has the following disadvantages:
1) требуется наличие оборудования и специалиста для проведения допплерографического исследования;1) the presence of equipment and a specialist is required to conduct a Doppler study;
2) исследование не входит в программу ОМС и требует оплаты его стоимости самим пациентом;2) the study is not included in the CHI program and requires payment of its cost by the patient himself;
3) the method can be implemented only on the basis of a large diagnostic center.
Thus, the development of a new method for assessing the risk of progression of chronic kidney disease is required. In the claimed method, the listed disadvantages are eliminated.
