Изобретение относится к медицине, а именно к области кардиохирургии и может быть использовано при лечении острой тромбоэмболии легочных артерий (ТЭЛА). Разработанное изобретение является альтернативным способом хирургического лечения массивной острой тромбоэмболии легочных артерии путем дополнительного доступа в просвет легочной артерии, применяемого в специализированных стационарах.The invention relates to medicine, namely to the field of cardiac surgery and can be used in the treatment of acute pulmonary embolism (PE). Впервые успешное выполнение тромбоэмболоэктомии из легочной артерии сделано в 1961 году Cooley D.A. et al., этот способ с небольшими модификациями применяется в настоящее время большинством клиник, занимающихся проблемой хирургического лечения ТЭЛА [2]. Способ (по Cooley D.A.) хирургического лечения острой ТЭЛА [2] выполняется на остановленном, релаксированном сердце с использованием аппарата искусственного кровообращения (ИК). Вскрывается продольно ствол легочной артерии (ЛА) и с помощью хирургических инструментов извлекаются тромбы. При вскрытии ствола ЛА хирург визуализирует левую главную ветвь с долевыми артериями, и начальную часть правой легочной артерии. При удалении тромбов из правой ветви визуализация дистальных сегментов отсутствует. Выполняется удаление тромбов без прямого зрения или с применением полимерного катетера для тромбэкстракции (катетер Фогарти). Эти манипуляции приводят к фрагментации тромбов, не полное удаление из артерии и повреждению эндотелиального слоя артерии. Данный способ выбран за прототип предлагаемого изобретения. Недостатками способа является выполнение тромбэмболэктомии из правой и левой легочной артерии через главную ветвь легочной артерии. В результате возможно не полное удаление тромбов, что сохранит обструкцию в легочном русле и приведет к неудовлетворительным результатам.This method is chosen for the prototype of the present invention. [3]. Также зачастую применение консервативных методик не показывает хорошие клинические результаты и являются не эффективными. Наличия тромбов в основном стволе и главных долевых ветвях вызывает выраженную обструкцию в легочной артерии, которая приводит к острому нарушения гемодинамики. Тромбы в легочном русле в 48% случаев располагаются в основном стволе, в 65% случаев в главных ветвях ЛА. После проведения срединной стернотомии производят канюляцию аорты и полых вен, начинают искусственное кровообращение. Проводится кардиоплегия путем введения в корень аорты любого кардиоплегического раствора, для полной электромеханической остановки сердца. Далее через дренажную канюлю, установленную в правую верхнюю легочную артерию, достигается полное или частичное удаления крови из камер сердца, с дальнейшим поддержанием работы канюли. Первым этапом производится продольный разрез ствола легочной артерии. На осушенном сердце при помощи окончатого зажима, производится удаление тромбов из главной ЛА, из левой ЛА и ее долевых и средних ветвей. Вторым этапом выделяют правую легочную артерию от окружающих тканей и в продольном направлении от аорты в сторону верхней полой вены выполняется разрез скальпелем длинной 1,5 см. С помощью доступа под прямым углом зрения визуализируется проксимальная часть правой легочной артерии и ее долевые ветви, и производится тотальное удаление тромбов из правой легочной артерии. Выполняют деэмболизацию, деаэрацию отделов сердца, с восстановлением сердечной деятельности. Пациент М., 54 года, был доставлен из онкологического центра по неотложным показаниям, где у пациента после удаления опухоли почки в послеоперационном периоде на 3-4 сутки появились боли в проекции икроножных мышц с последующим появлением одышки при незначительной физической нагрузке. В анамнезе тромбофлебит глубоких вен нижних конечностей. По данным эхокардиографии высокая легочная гипертензия СДЛА 75 мм рт.ст., расширение камер сердца, в большей степени правого желудочка, умеренная трикуспидальная недостаточность, фракция выброса 54%. По данным компьютерной томографии с контрастированием в долевых ветвях правой и левой легочной артерии отмечаются массивные организованные тромбы с окллюзированием просвета с обеих сторон. По ультразвуковому исследованию вен нижних конечностей, флотирующий тромбов не выявлено. Учитывая массивную тромбоэмболию ветвей легочных артерий проведено оперативное лечение по описанной методике. В раннем послеоперационном периоде отмечалось снижение давления в легочной артерии СДЛА 31 мм рт.ст., уменьшение размеров камер сердца, улучшение внутрисердечной гемодинамики. Положительный результат хирургического лечения отмечен спустя 24 месяца после вмешательства, по данным эхокардиографии отмечается нормальное давление в легочной артерии СДЛА 33 мм рт.ст., тромботические массы отсутствуют.Patient M., 54 years old, was delivered from the oncology center for urgent indications, where after the removal of the kidney tumor in the postoperative period, the patient developed pain in the projection of the calf muscles on days 3-4, followed by shortness of breath with little physical exertion. A positive result of surgical treatment was noted 24 months after the intervention, according to echocardiography, there was a normal pressure in the pulmonary artery of SPPA of 33 mm Hg, there were no thrombotic masses.

Способ хирургического доступа при лечении острой тромбоэмболии легочной артерии широко применяется на базе ГАУЗ МКДЦ по жизненным показаниям.
