2. Известен способ лечения анальных трещин путем баллонной дивульсии. Одна из первых публикаций датирована 1914 годом, когда автор (Hirschman L.J. 1914) раздувал баллон в анальном канале без анестезии и контроля давления в баллоне. В публикациях последних лет (Nielsen М.В. et al. 1993; Renzi A. et al. 2005; Sohn N. et al. 1992; Sohn N. 1997; Walfisch S. 1998) авторы использовали баллон диаметром 30-40 мм, раздували его в анальном канале до 1,4 атмосфер и выдерживали экспозицию до 6 мин (Renzi A. et al. 2005). При этом следует отметь, что авторы анальную трещину не иссекали. Кроме того, манипуляция проходила только под местной анестезией.2. A known method of treating anal fissures by balloon divulsion. Существенными недостатками являются в раннем послеоперационном периоде возможно образование гематом и абсцессов в области сфинктеротомии. В отдаленном послеоперационном периоде образование свищей прямой кишки отмечено в 0,2-1,6%. Следует подчеркнуть, что у 0,6-19,6% больных сохраняется слабость сфинктера заднего прохода, из них I степень инконтиненции регистрируется в 0,6-11,6% наблюдений, а II степени - в 6-8%. Рецидив заболевания развивается у 0,6-11,1% пациентов (Шелыгин Ю.А. с соавт. 2005; Полетов Н.Н, с соавт. 2003; Tocchi А. 2004).Significant disadvantages are the formation of hematomas and abscesses in the area of sphincterotomy in the early postoperative period. Технический результат данной работы заключается в том, что предлагаемый метод боковой дозированной сфинктеротомии позволяет рассматривать ее как операцию выбора для лечения анальных трещин и длительно незаживающих ран анального канала, в том числе - осложненных стриктурой. Предлагаемый метод, в отличие от классической боковой закрытой сфинктеротомии, позволяет значительно уменьшить риски чрезмерного пересечения внутреннего сфинктера, а также дает возможность дополнительного пересечения порции сфинктера при недостаточной степени снятия спазма его. Метод удобен и прост в применении, его методическое освоение врачами - колопроктологами требует кратковременного курса обучения. В этой связи применение этого метода лечения возможно в амбулаторных условиях или в стационаре одного дня.The technical result of this work is that the proposed method of lateral dosed sphincterotomy allows us to consider it as the operation of choice for the treatment of anal fissures and long-term non-healing wounds of the anal canal, including those complicated by stricture. The proposed method, in contrast to the classical lateral closed sphincterotomy, can significantly reduce the risk of excessive crossing of the internal sphincter, and also makes it possible to additionally cut a portion of the sphincter with an insufficient degree of spasm relief. The method is convenient and easy to use, its methodological development by coloproctologists requires a short course of study. In this regard, the use of this method of treatment is possible on an outpatient basis or in a one-day hospital.
Техника операции: Под эпидурально-сакральной, спинальной или местной анестезией в положении пациента как для геморроидэктомии, на 3 часах, отступя 0,5 см от края ануса, в проекции межсфинктерной борозды в вертикальном направлении проводим дугообразный, в сторону от ануса, разрез кожи длиной 1 см (рис. 1.). По межсфинктерной борозде тупым путем с использованием зажима москит и стоматологического инструмента "гладилка" с дополнительно изогнутым концом мобилизовываем на всю глубину внутренний сфинктер (рис. 2-4). Под последним проводим две свободные лигатуры, при этом весь внутренний сфинктер оказывается взятым на лигатуры. Далее, производится пересечение дистальных 2/3 мобилизованного внутреннего сфинктера с оставлением нетронутого 1/3 проксимального его отдела (рис. 5). Next, the distal 2/3 of the mobilized internal sphincter is transected, leaving 1/3 of its proximal section untouched (Fig. 5). Previously held ligatures remain loose in the wound, not tied (Fig. 6).

При сохранении спазма сфинктера, определяемого клинически и сфинктерометрически под небольшой местной анестезией, завязываем свободные лигатуры, раннее оставленные в ране, с пересечением сфинктера между ними.On the 2nd day after the operation, in the absence of spasm, loose ligatures are removed. Пациент О.А. 35 лет поступил с жалобами на боли в области ануса вовремя и после стула, длящиеся несколько часов, выделение алой крови, стулобоязнь.Patient O.A. Местно: в перианальной области на 6 часах имеется послеоперационная рана 3*1,5 см, болезненная при пальпации. 31.05.2021 г. пациенту было выполнено оперативное вмешательство: модифицированная дозированная боковая сфинктеротомия и радиоволновая фульгурация послеоперационной раны.On May 31, 2021, the patient underwent surgery: modified dosed lateral sphincterotomy and radio wave fulguration of the postoperative wound.
Под эпидурально-сакральной анестезией на 3 часах, отступя примерно 0,5 см от края ануса, в проекции межсфинктерной борозды в вертикальном направлении проведен дугообразный, в сторону от ануса, разрез кожи длиной 1 см. По межсфинктерной борозде тупым путем с использованием зажима москит и стоматологического инструмента "гладилка" с дополнительно изогнутым концом мобилизирован внутренний сфинктер на всю глубину. С помощью зажима москит или диссектором проведены две свободные лигатуры, захватывающие внутренний сфинктер на всю мобилизованную глубину. Дистальные 2/3 мобилизованного сфинктера взяты на зажимы, перевязаны викрилом 3/0. Сфинктер между лигатурами пересечен. В удовлетворительном состоянии пациент через 2 часа был выписан на амбулаторное долечивание. В послеоперационном периоде во время и после стула боли уменьшились, но полностью не купировались. Боль купирована приемом Кетанов 1 табл.In a satisfactory condition, the patient was discharged after 2 hours for outpatient aftercare. Осмотр проведен на вторые сутки после операции. При пальцевом исследовании спазм сфинктера был меньше, но сохранялся. Под местной анестезией свободно оставленные в ране лигатуры завязаны, сфинктер между ними пересечен. Через 30 минут пациент отпущен домой. Стул был на следующий день. В обезболивании пациент не нуждался, отмечал улучшение состояния. При осмотре на четвертые сутки после дополнительной сфинктеротомии отмечается отсутствие спазма и практически - отсутствие болей.The examination was carried out on the second day after the operation. Подобным образом оперировано трое пациентов с хронической анальной трещиной, осложненной стриктурой и двое пациентов с длительно незаживающей раной и также со стриктурой анального канала после геморроидэктомии. У всех пациентов имел место выраженный спазм сфинктера. У троих пациентов тонус сфинктера после вмешательства пришел в норму. В двух случаях сфинктеротомия в данной модификации оказалась недостаточной, в связи с чем на 2-е сутки после вмешательства у них были перевязаны раннее оставленные лигатуры в ране с пересечением небольшой порции сфинктера между ними, что привело к нормализации тонуса сфинктера в последующем.Three patients with chronic anal fissure complicated by stricture and two patients with a long-term non-healing wound and also with anal stricture after hemorrhoidectomy were operated on in a similar way. All patients had severe spasm of the sphincter. In three patients, the sphincter tone returned to normal after the intervention. In two cases, the sphincterotomy in this modification was insufficient, and therefore, on the 2nd day after the intervention, the previously left ligatures in the wound were tied up with the intersection of a small portion of the sphincter between them, which led to the normalization of the sphincter tone in the future.
