Abstract
Description
The present invention relates to medicine, namely to therapy and can be used in the complex treatment of children with uncomplicated necrotizing enterocolitis (NEC).
Infectious and inflammatory diseases are the most common pathology in children of the neonatal period, which is explained by the peculiarity of the protective mechanisms at this stage of ontogenesis. The most pronounced insufficiency of humoral and cellular immunity, the immaturity of the barrier functions of the skin and mucous membranes in children born prematurely. Necrotizing enterocolitis is a severe polyetiological disease complicated by intestinal perforation, abdominal infiltrate, peritonitis and sepsis. The high mortality rate in NEC is the reason for the active study of its pathogenesis. Complicating the course of NEC is bacterial infection.
749).In medical practice, antibiotic therapy is very often used as a formal reinsurance for various borderline conditions in the neurological and somatic status. The question of the prophylactic use of antibiotics in small preterm infants at risk for intrauterine and intranatal infection (sick mother, long anhydrous period, intrauterine growth retardation, acute and chronic hypoxia) is discussed, as a rule, the situation is resolved in favor of prescribing an antibiotic, even in cases so-called non-infectious pneumopathies (Yatsyk GV, Bombardirova EP Peculiarities of antibacterial therapy in premature newborns//Russian Medical Journal, 2000, No. 18, p. 749).
The initial successes in the use of antibacterial drugs (ABs) in the 50s and 60s of the last century allowed scientists and clinicians to express an optimistic opinion about the victory of medicine over microbes and the real elimination of infectious diseases in society. However, today the situation has been complicated by the emergence of antibiotic-resistant staphylococci and pneumococci and gram-negative bacteria.
It has been proven that in patients with a nosocomial infection frolicking in a hospital, the risk of death increases by 5.5 times, and the average duration of inpatient treatment increases by 8.4 days in adults and by 10.7 days in children. At the same time, in nosocomial infections, 88.1% of pathogens were characterized by multiresistance (resistance to 3 or more antibiotics), and 7.1% were extremely resistant, that is, they remained sensitive to only one or a maximum of two antibiotics.
Hsueh// Pediatr. Dev. Pathol. - 2003. - Vol.6. - P.6-23; Ford H. R. Mechanism of nitric oxide-mediated intestinal barrier failure: insight into the pathogenesis of necrotizing enterocolitis / H. R. Ford // J. Pediatr. surg. - 2006. - Vol. 41, № 2. - P. 294-299). In addition to standard antibiotic therapy (Gelfand B.R. Antibacterial therapy of abdominal surgical infection / B.R. Gelfand, V.A. Gologorsky, S.Z. Burievich. - M.: Mirror, 2000. - 144 p.), the use of enteral administration of antibiotics to prevent enterocolitis in premature and low birth weight newborns (Hsueh W. Neonatal necrotizing enterocolitis/W. Hsueh// Pediatr. Dev. Pathol. - 2003. - Vol.6. - P.6-23; Ford H. R. Mechanism of nitric oxide-mediated intestinal barrier failure: insight into the pathogenesis of necrotizing enterocolitis / H. R. Ford // J. Pediatr. surg. - 2006. - Vol. 41, No. 2. - P. 294-299).
At the present stage, according to WHO, the unreasonable and irrational use of antibiotics in newborns, especially in the group of premature babies with borderline conditions (perinatal damage to the central nervous system, functional disorders of the gastrointestinal tract, necrotizing enterocolitis 1A, B stage, IIA stage), causes to revise old views and introduce new approaches in the treatment of bacterial infections in children.
It is known (instruction Staphylococcal bacteriophage) on the use of bacteriophages in enterocolitis of newborns, including premature babies. Bacteriophage is used in the form of high enemas (through a vent tube or catheter) 2-3 times a day at a dose of 5-10 ml. In the absence of vomiting and regurgitation, it is possible to use the drug through the mouth. In this case, it is mixed with breast milk. Perhaps a combination of rectal (in the form of high enemas) and oral (through the mouth) use of the drug. The course of treatment is 5-15 days. With a recurrent course of the disease, repeated courses of treatment are possible. In order to prevent sepsis and enterocolitis in case of intrauterine infection or the risk of nosocomial infection in newborns, the bacteriophage is used in the form of enemas 2 times a day for 5-7 days.
Причем анаэробы вызывают тяжелые стадии НЭК (IIВ; IIIА, В).However, monotherapy with specific bacteriophages does not completely cover the entire spectrum of pathogenic flora, since the causative agents of NEC are both aerobic and anaerobic pathogens. Moreover, anaerobes cause severe stages of NEC (IIB; IIIA, B).
ч. Bacteroides fragilis, Bacteroides distasonis, Bacteroides ovatus, Bacteroides thetaiotaomicron, Bacteroides vulgatus), Fusobacterium spp., Veillonella spp., Prevolella spp. (Prevotella bivia, Prevotella buccae, Prevotella disiens) и некоторых грамположительных микроорганизмов (Eubacterium spp., Clostridium spp., Peptococcus spp., Peptostreptococcus spp).Known antiprotozoal and antimicrobial drug Metrogyl is a derivative of 5-nitroimidazole, the mechanism of action of which is the biochemical reduction of the 5-nitro group of metronidazole by intracellular transport proteins of anaerobic microorganisms and protozoa. The reduced 5-nitro group of metronidazole interacts with the DNA of microorganism cells, inhibiting the synthesis of their nucleic acids, which leads to the death of bacteria. Metrogyl is active against Trichomonas vaginalis, Entamoeba histolytica, Gardnerella vaginalis, Giardiai ntestinalis, Lamblia spp., and obligate anaerobes Bacteroides spp. (including Bacteroides fragilis, Bacteroides distasonis, Bacteroides ovatus, Bacteroides thetaiotaomicron, Bacteroides vulgatus), Fusobacterium spp., Veillonella spp., Prevolella spp. (Prevotella bivia, Prevotella buccae, Prevotella disiens) and some Gram-positive microorganisms (Eubacterium spp., Clostridium spp., Peptococcus spp., Peptostreptococcus spp).
Aerobic microorganisms and facultative anaerobes are insensitive to metronidazole, but in the presence of mixed flora (aerobes and anaerobes), metronidazole acts synergistically with antibiotics effective against ordinary aerobes. Increases the sensitivity of tumors to radiation, causes disulfiram-like reactions, stimulates reparative processes.
From the combined use of specific bacteriophages in combination with Metrogyl solution, experts can expect an antimicrobial effect, although data on the results of treatment with this combination of bacteriophages in NEC in newborns have not been found.
Lymphocytes are an important component of human blood. In a healthy person, T-cells predominate among lymphocytes. The main populations of lymphocytes are represented by T-helperamia and cytotoxic T-lymphocytes. Helper T cells help in the development of an immune response against infections, while cytotoxic T lymphocytes take part in the direct destruction of infected cells. Usually, the number of T-helpers somewhat exceeds the number of cytotoxic T-lymphocytes.
CD8.To determine the relative content of these cells in the blood, the flow cytometry method is used, which allows you to determine the number of cells that have different antigens on their surface. On the surface of T-helpers is the CD4 antigen, while on the surface of cytotoxic T-lymphocytes is the CD8 antigen.
It is these antigens that are used to calculate both the absolute number of T-cells in these subpopulations and their relative number.
2,6. The ratio of T-helpers to cytotoxic T-lymphocytes is called the immunoregulatory index. The CD4 to CD8 ratio (immunoregulatory index) describes general immune dysfunction and may be the best biomarker of disease progression, treatment response, morbidity, and mortality in virus-suppressed patients. This index indicates whether the patient's immune system is working optimally. The values of the immunoregulatory index are normally in the range from 1.2 to 2.6.
If the value of the immunoregulatory index is less than 1, this may indicate the following processes: congenital immunodeficiency, HIV infection, stress, anemia, or in the presence of cancer.
Values of the immunoregulatory index exceeding 2.6 may be associated with the presence of: bacterial infection, viral infection, T-cell leukemia.
The objective of the invention is to create a method for the treatment of uncomplicated necrotizing enterocolitis in newborns in the presence of intrauterine and bacterial infections (stages I, II according to M. Walsh, R. Kleigman, 1986) without the use of antibiotics, which positively affects the immune status.
The technical result is an increase in the effectiveness of treatment without the use of antibiotics, the preservation of normal flora, the exclusion of the toxic effect of antibiotics, and the activation of the immune response.
The technical result is achieved by the fact that in a method for treating uncomplicated necrotizing enterocolitis in newborns with a bacterial infection and suspected intrauterine infection, including the introduction of a bacteriophage intraorally and rectally with C-reactive protein> 12 mg/l, the bacteriophage is injected into the stomach through a nasogastric tube, 5 ml 2 times a day with an exposure of 30 minutes and a mixture of 5 ml of a bacteriophage with a solution of Metrogyl (7.5 mg/kg), heated to 37.2 ° C, is administered rectally through a gas outlet tube according to the principle of high enemas with an exposure of 30 minutes 3 times a day. day in a course of 10 days.
The method is carried out as follows:
In patients with suspected NEC or with stage I, II NEC, with suspected intrauterine infection (IUI) (based on the history and characteristics of the course of pregnancy), the value of C-reactive protein and pathogenic agents are determined. If the value of C-reactive protein is more than 12 mg / l, a specific bacteriophage is prescribed through a nasogastric tube into the stomach, 5 ml 2 times a day (morning, evening) with an exposure of 30 minutes and rectally, a mixture of 5 ml of a specific bacteriophage with a solution of Metrogil 7.5 mg / kg with an exposure of 30 minutes 3 times a day. Every 8 hours, when the colon is filled with contents, the colon is washed with saline (sol. natrii chloridi 0.9% - 10.0 ml), the temperature of which is 28°C. Then, per rectum, after heating to 37.2 ° C, a bacteriophage-Metrogil mixture is injected through the gas outlet tube according to the principle of high enemas with an exposure of 30 minutes. The treatment is carried out in a course of 10 days, followed by monitoring of inflammation markers, bacterial culture data from the gastrointestinal tract and the results of PCR diagnostics. If necessary, the course is repeated.
To illustrate the proposed method, we present extracts from the case histories.

Thus, the proposed method for the treatment of necrotizing enterocolitis allows selective action on pathogenic pathogens without suppressing the normal flora of the gastrointestinal tract. The adequacy of the presented therapy activates the links of immunity and contributes to the regression of the inflammatory process in the body, which is relevant if IUI is suspected.
The proposed method of treatment is simple for use in practical healthcare. The effectiveness of the proposed method is 100%, as it consists in improving the results of treatment due to the complex specific effects on the aerobic and anaerobic flora without total suppression of the normal intestinal flora. Relief of the inflammatory process due to the accuracy of the impact on pathogenic pathogens prevents complications and mortality in newborns with necrotizing enterocolitis.
