Способ лапароскопической резекции селезенки относится к области медицины, в частности к абдоминальной хирургии, и может быть использовано для оперативного органосберегающего лечения доброкачественных заболеваний селезенки.The method of laparoscopic resection of the spleen refers to the field of medicine, in particular to abdominal surgery, and can be used for surgical organ-sparing treatment of benign diseases of the spleen.

Предлагаемый способ лапароскопической резекции селезенки выполняют следующим образом. Пациента укладывают на операционном столе на спину в положении Фаулера. В асептических условиях под комбинированным интубационным наркозом в супраумбиликальной области производят разрез кожи 1 см, выполняют лапаролифтинг цапками, выполняют лапароцентез иглой Вереша, который подтверждают капельным тестом, шприцевой пробой, аппаратной пробой, создается карбоперитонеум, вводится лапароскоп, подключенный к камере. Таким образом, достигается минимальная кровопотеря и хорошая визуализация за счет работы на «сухом» операционном поле. С помощью аппарата Thunderbeat выполняют резекцию патологического новообразования по границе здоровой ткани. Производят гемостаз линии резекции биполярной коагуляцией. Пациентка Ж. поступила в клинику с диагносцированной кистой селезенки. При плановом УЗИ органов брюшной полости в январе 2019 года диагностирована киста селезенки (~5,0 см). Рекомендовано динамическое наблюдение. В динамике образование сохраняется, увеличивается в размерах. Выполнена СКТ - картина кисты селезенки. Селезенка увеличена в размерах за счет образования овоидной формы с четкими ровными контурами. Селезенка обычных размеров и окраски, из верхнего полюса исходит округлое образование диаметром до 70 мм, плотной структуры, капсула образования белого цвета, с бугристой поверхностью. Желудок обычных размеров и окраски, в области большой кривизны определяется округлое образование на широком основании диаметром 5 мм, белой окраски, плотной консистенции. С помощью аппарата Thunderbeat желудочно-селезеночная и ободочно-селезеночная связки пересечены, произведена мобилизация селезенки. Селезеночная артерия выделена на расстоянии от места на 4 см проксимальнее ворот до места разветвления на сегментарные артерии. На артерию наложен мягкий сосудистый зажим DeBakey для временной окклюзии. С помощью аппарата Thunderbeat выполнена резекция верхнего полюса селезенки с кистозным образованием по границе здоровой ткани. Гемостаз биполярной коагуляцией. Ложе удаленной кисты дополнительно укреплено 2 пластинами Tachocomb, гемостатической системой PerClot. Сосудистый зажим DeBakey снят, извлечен из брюшной полости. Время тепловой ишемии составило 20 минут. Контроль гемостаза -сухо. С помощью ножниц в холодном режиме выполнено иссечение новообразования желудка до подслизистого слоя. Наложен мышечно-серозный узловой шов нитью Poly sorb 3-0. Контроль гемостаза - сухо. Удаленные препараты помещены в пластиковый контейнер, извлечены из брюшной полости через разрез в левой подвздошной области, направлены на гистологическое исследование. Брюшная полость дренирована через левую контраппертуру. Инструменты, троакары извлечены из брюшной полости. Десуффляция. Троакарные раны ушиты при помощи системы EndoClose. Течение послеоперационного периода гладкое. Проводилась инфузионная, антибактериальная, симптоматическая терапия. Пациентка выписана в удовлетворительном состоянии на вторые сутки после операции.
