Abstract
Description
The invention relates to medicine, namely to dentistry, and can be used in the clinic of orthopedic dentistry.
A known method of preparing teeth to the patent of the Russian Federation No. 2186546, publ. 08/10/2002, including the formation of longitudinal and transverse marker grooves with a depth of 0.5 to 1.5 mm on the prepared surface of the tooth with an interval of 2.5 mm, and the actual grinding of the resulting squares to the required depth.
The disadvantage of this method is that it does not take into account the difference between the anatomical shape of the tooth and the required geometric shape of its prepared stump, and therefore, it is necessary to correct the latter under visual control, which reduces the accuracy of the tooth preparation.
A known method of preparing front teeth for artificial crowns to the patent of the Russian Federation No. 2593057, publ. 07/27/2016. The method includes forming vertical and horizontal calibration grooves with a depth of 0.5-1.5 mm on the prepared tooth surface and subsequent preparation of the proximal, oral, vestibular surfaces to a predetermined depth and forming a ledge with repetition of the gum contour.
The disadvantage of this method, as in the case of the first analogue, is its focus on the anatomical shape of the tooth. Also, the method is intended for the preparation of the anterior teeth, which limits the possibility of its application.
- С. 194-298).A known method of preparing teeth using a silicone template, which is as follows. Before starting the preparation, a silicone template is made, which rests on the teeth that are not to be prepared. At the stage of tooth preparation, the dentist has the opportunity to control the correctness of the manipulation by applying a silicone template and evaluating the size of the prosthetic space between the inner surface of the silicone template and the surface of the tooth stump (see Shilinburg, G.T. Fundamentals of tooth preparation for the manufacture of cast metal, metal-ceramic and ceramic restorations / G.T. Shilinburg, R. Jacobi, S. Brackett. - St. Petersburg: Azbuka Publishing House, 2006. - P. 194-298).
The disadvantage of this method is that it is designed to control the volume of the created prosthetic space, while the actual formation of the abutment tooth stump occurs under visual control.
The prototype of the claimed invention is a method of preparing teeth to US patent No. 6659772 B2, publ 29.08.2002. The method consists in preliminary modeling of the future orthopedic structure on the diagnostic model, its transfer to the supporting teeth with a composite material (mock-up technique) and marking the preparation depth relative to the surface of the reproduced shape of the future structure with special burs with a fixed immersion depth. Further steps are performed based on the protocol of tooth preparation chosen by the doctor. The method allows to limit the required amount of intervention, which meets the principle of minimal invasiveness in dental treatment.
The disadvantage of the prototype method is that the initial guideline for the preparation of teeth is the intended surface of the future orthopedic design, which is not always clinically appropriate to meet the requirements for the stumps of the prepared teeth, namely: parallelism, a given taper, no undercuts.
The objective of the invention is to improve the quality of the preparation of teeth, which consists in increasing the geometric accuracy of the stumps of the prepared teeth.
The essence of the claimed invention is as follows.
A method for high-precision preparation of teeth for orthopedic constructions, including marking the depth of preparation and the actual preparation of teeth according to the traditional algorithm, characterized in that marking the depth of preparation is performed using specially made preparation caps, the outer surface of which is modeled taking into account the data of preliminary preparation of supporting teeth on diagnostic models and 3 mm away from the planned surface of the stumps. At the same time, the depth of preparation of hard tissues is marked by rotating end diamond burs by immersing them by 3 mm relative to the outer surface of the preparation caps along the guides that go normally to the planned surface of the stumps of the prepared teeth.
The claimed method is carried out as follows. Before the preparation of the teeth, diagnostic models of the patient's jaws are obtained and their 3D scanning is performed. Then, abutment teeth are prepared on the diagnostic model and a second 3D scan of the diagnostic model with the prepared teeth is performed. According to the obtained three-dimensional models of the patient's jaws, preparation caps are made in such a way that they can be applied to the teeth before preparation, and their outer surface is 3 mm away from the planned surface of the prepared teeth stumps, and having guides with a diameter of 1 mm, located normally to the surface of the planned stump prepared tooth. Marking the depth of tooth preparation is carried out with the preparation caps applied by immersing a rotating diamond end bur with a diameter of 1 mm along the guides to a depth of 3 mm relative to their outer surface.
The technical result of the claimed invention is to increase the accuracy of the geometric shape of the stumps of the prepared teeth by controlling the thickness of the reduction of their hard tissues.
