Изобретение относится к области медицины, а именно к оториноларингологии, и предназначено для экспресс-диагностики слуха с целью раннего выявления его нарушений.The invention relates to medicine, namely to otorhinolaryngology, and is intended for express diagnostics of hearing in order to detect early hearing disorders.
Наиболее близким аналогом заявляемого технического решения является способ диагностики уровня слуха, заключающийся в проверке слуха при помощи речевой таблицы В. Воячека, записанной в память цифрового устройство в звуковом формате mp3. После повторения пациентом слов определяют процент правильно повторяемых от общего числа слов таблицы, используя клавиатуру устройства для регистрации ответа пациента при громкостях звукового сигнала 10, 20 и 30% от максимальной мощности наушников соответственно, а затем выявляют снижение процента разборчивости речи [1].The closest analogue of the proposed technical solution is a method for diagnosing the level of hearing, which consists in checking the hearing using V. Недостатком известного способа является сложность вычисления уровней громкости, соответствующих 10, 20 и 30% от максимальной мощности наушников, так как разные наушники, как и различные мобильные телефоны, имеют отличающиеся характеристики мощности звука при воспроизведении. Кроме того, недостатком является дороговизна процесса, ведь если использовать одно цифровое устройство, к нему необходимо приобрести большое количество комплектов одноразовых наушников (так как наушники не могут быть подвергнуты стерилизации), дабы избежать передачи инфекции наружного уха среди пациентов. Также, в речевых таблицах В. Воячека нет четкого разделения слов по частотам, в основном слова подобраны так, что затрагивают только низкие и средние частоты, и при проверке возможно пропустить снижение слуха у пациента в том случае, если оно началось с высоких частот. Данная задача достигается за счет того, что воспроизводится набор тестовых слов, подобранных по методике «речевого банана», используется аппарат с записанными шепотом в его память словами в звуковом формате mp3 и на основании правильности повторения пациентом слов выявляют наличие снижения разборчивости речи, по которому судят о возможном снижении слуха и направляют на дальнейшее обследовании на аудиометре для подтверждения диагноза и назначения лечения; либо отсутствие снижения разборчивости речи, по которому судят о том, что слух в норме. Слова воспроизводятся на фиксированной громкости 40 дБ, на следующих частотах: 125, 250, 500, 1000, 2000, 4000, 6000, 8000 Гц. Расстояние от аппарата до испытуемого 6 метров. Используется 3 варианта набора тестовых слов для исключения их запоминания. Методика «речевого банана» заключается в том, что звуки, необходимые для разговорного языка, воспринимаются человеком на определенных частотах. Подбираются слова, которые содержат звуки определенной частоты. Если человек не может воспроизвести или не правильно воспроизводит услышанное слово, значит он не воспринимает звуки на данной частоте. Это говорит о снижении слуха. Заявляемое техническое решение было апробировано в ЛОР-кабинете БУЗ УР «ГКБ №2 МЗ УР» г. Ижевска. Было обследовано 53 пациента в возрасте от 19 до 76 лет, из них 22 мужчины и 31 женщина. Пациентам была проведена отоскопия, удалены серные пробки, исключены острые заболевания наружного и среднего уха.The claimed technical solution was tested in the ENT-cabinet BUZ UR "GKB No. 2 MZ UR" in Izhevsk. Устройство находилось от пациента на расстоянии 6 метров. Пациент повторял услышанные слова. Врач анализировал произнесенные испытуемым слова. В результате исследования у 29 человек при помощи данного способа было выявлено снижение слуха: 10 человек - снижение слуха на высоких частотах; 15 человек - снижение слуха на высоких и средних частотах; 4 человека - снижение слуха на высоких, средних и низких частотах. Данным пациентам было проведено аудиометрическое исследование.
