Description
Изобретение относится к медицине, а именно к кардиохирургии для фиксации грудины после срединной стернотомии. В хирургии сердца срединная стернотомия является наиболее распространенным доступом, благодаря которому становятся хорошо доступными органы переднего средостения. Нестабильность грудины и раневая инфекция после срединной стернотомии - серьезные осложнения, встречающиеся по данным разных авторов от 0,3 до 5% оперированных пациентов. Приблизительно у половины пациентов, у которых послеоперационный период осложнился нестабильностью грудины, в конечном счете, развивается стернальную инфекцию. Так, при вдохе создается небольшое разрежение на краях стернотомной раны, и если грудина нестабильна, то возможно проникновение экзогенных бактерий способствующих инфицированию средостения. Швы на грудине являются единственной силой, удерживающей края грудины вместе. Имеет значение материал, из которого сделаны швы, их количество, направление, расположение, а также напряжение, которое они оказывают на кость и его распространение. Теоретически, чем больше число швов, тем плотнее сопоставление, меньшее напряжение и т.д. Основная проблема стабилизации грудины заключается в том, что швы прорезывают грудину в течение первых часов или дней после операции. Сопоставление краев грудины сначала ослабляется, а затем вследствие респираторных движений, они фрагментируют грудину. Внезапное, индуцированное напряжение, чихание или кашель могут ускорить эти события.The invention relates to medicine, namely to cardiac surgery for fixing the sternum after a median sternotomy. Sternum instability and wound infection after a median sternotomy are serious complications that, according to different authors, occur from 0.3 to 5% of operated patients. Approximately half of patients whose postoperative period is complicated by instability of the sternum eventually develop sternal infection. So, when inhaling, a small vacuum is created at the edges of the sternotomy wound, and if the sternum is unstable, then the penetration of exogenous bacteria that contribute to the infection of the mediastinum is possible. The sternum stitches are the only force holding the edges of the sternum together. What matters is the material from which the sutures are made, their number, direction, location, as well as the stress they exert on the bone and its distribution. In theory, the greater the number of seams, the tighter the juxtaposition, less stress, etc. The main problem with sternum stabilization is that the sutures are cut through the sternum during the first hours or days after surgery. The juxtaposition of the edges of the sternum is first weakened, and then, due to respiratory movements, they fragment the sternum. Sudden, induced exertion, sneezing, or coughing can precipitate these events.

Задачей, на решение которой направлено заявляемое изобретение, является предотвращение неправильного совмещения частей грудины при ее фиксации, прорезывания швами кости грудины, снижение риска расхождения краев стернотомной раны после закрытия грудины. Для решения поставленной задачи в способе ушивания грудины после срединной стернотомии, включающем наложение швов на грудину с использованием проволочного шовного материала, согласно изобретению, что первую пару швов накладывают на рукоятку грудины, остальные швы накладывают на тело грудины, после наложения четного количества одиночных швов, первым этапом скручивают попарно проволочные нити с одной и с другой стороны по схеме: 1-я со 2-й слева, 3-я с 4-й справа и 5-ю с 6-й слева; затем, используя нескрученные проволочные нити, осуществляют предварительное сведение грудины, контролируя совмещение частей грудины по яремной вырезке, после этого попарно скручивают оставшиеся проволочные нити по схеме: 1-я со 2-й справа, 3-я с 4-й слева и 5-ю с 6-й справа, окончательно сопоставляя края грудины, на следующем этапе скручивают расположенные напротив концы попарно скрученных проволочных нитей друг с другом, окончательно фиксируя грудину.The problem to be solved by the claimed invention is to prevent misalignment of parts of the sternum during its fixation, cut through the sutures of the sternum bone, reduce the risk of divergence of the edges of the sternotomy wound after closing the sternum. Заявляемое изобретение поясняется рисунками. На рисунке 1 схематично изображено наложение четного количества одиночных проволочных нитей, которые накладывают последовательно на грудину. На рисунке 2 - схематично изображен первый этап - скручивают попарно проволочные нити с одной и с другой стороны по схеме 1-я со 2-й слева, 3-я с 4-й справа и 5-ю с 6-й слева. На рисунке 3 - схематично изображен этап - используя нескрученные проволочные нити, осуществляется предварительное сведение грудины с контролем совмещения частей грудины по яремной вырезке (С), после чего попарно скручивают оставшиеся проволочные нити, также по схеме 1-я со 2-й справа, 3-я с 4-й слева и 5-ю с 6-й справа, окончательно сопоставляя края грудины (рисунок 4). На рисунке 5 показан следующий этап, когда скручивают напротив лежащие концы попарно уже скрученных проволочных нитей друг с другом, тем самым окончательно фиксирую грудину. На рисунке 6 - сравнение распределения напряжения проволочных нитей на кость (грудину) при стандартном одиночном шве (А) и при заявленном способе (Б).The claimed invention is illustrated by drawings. Figure 5 shows the next stage, when the opposite ends of the already twisted wire threads are twisted in pairs with each other, thereby finally fixing the sternum. Figure 6 shows a comparison of the stress distribution of wire threads on the bone (sternum) with a standard single suture (A) and with the claimed method (B).

Пример 1. Больной А. 64 года. Поступил в госпиталь с ДЗ: ИБС: стенокардия напряжения 3 ФК, постинфарктный кардиосклероз. Сахарный диабет 2 типа, стадия субкомпенсации.Example 1. На коронарографии мультифокальное поражение коронарного русла с поражением ствола левой коронарной артерии до 90%. Выполнена операция аортокоронарного шунтирования в объеме 5 шунтов в условиях искусственного кровообращения и тепловой кровяной кардиоплегии. Пример 2. Больная Б. 67 лет.Поступила в клинику с ДЗ: Вторичный инфекционный эндокардит, хроническое течение. Аортальная недостаточность 3 ст. Абсцесс некоронарной створки АК. Кардиомегалия. NYHA IV. Пациентке была выполнена операция: Протезирование аортального клапана (Hancock-II - 25). ИК и ККП. В ходе операции, после срединной стернотомии, отмечен остеопорозный процесс грудины. Надкостница коагулирована. Губчатое вещество грудины обработано SPONGOSTAN. Выполнен основной этап операции - протезирование аортального клапана в условиях искусственного кровообращения. При стабильной гемодинамике отключен АИК. После выполнения основного этапа операции грудина ушита по описанной методике. Послеоперационный период протекал без осложнений. Заживление операционной раны первичным натяжениием. Весь период консолидации грудины (6 месяцев) протекал без особенностей - грудина консолидирована, кожный шов в области металлических лигатур без особенностей.Example 2. Patient B. 67 years old. В отделении прооперировано 100 пациентов с применением заявленного способа ушивания грудины. Случаев прорезывания грудины металлическими швами не отмечено.
