ОТГС - это острое патологическое состояние, при котором возникает повышение интерстициального давления в фасциальном футляре, что приводит к сдавлению сосудов и нервов, ишемии ткани и, в конечном итоге, некрозу. В зависимости от тяжести развития патологического процесса в тканях, исход заболевания может быть различным - от минимальных функциональных нарушений, до потери конечности или даже летального исхода. Необратимые изменения в тканях происходят в ближайшие часы с момента возникновения ОТГС, поэтому его ранняя диагностика - это первостепенная задача для врача. В настоящее время нет четких критериев, клинических рекомендаций для диагностики ОТГС. Учитывается клиническая картина, анамнез травмы, и только после предварительного диагноза ОТГС возможно инструментальное инвазивное подтверждение, путем измерения внутритканевого давления.OTGS is an acute pathological condition in which there is an increase in interstitial pressure in the fascial sheath, which leads to compression of blood vessels and nerves, tissue ischemia and, ultimately, necrosis. Depending on the severity of the development of the pathological process in the tissues, the outcome of the disease can be different - from minimal functional disorders to loss of a limb or even death. Irreversible changes in tissues occur in the next few hours from the moment of occurrence of OTGS, therefore its early diagnosis is a primary task for a doctor. Currently, there are no clear criteria, clinical guidelines for the diagnosis of OTGS. The clinical picture, anamnesis of trauma are taken into account, and only after a preliminary diagnosis of OTGS is instrumental invasive confirmation possible by measuring the interstitial pressure.
При возникновении ОТГС в мышечно-фасциальных футлярах верхних или нижних конечностей существуют методики измерения давления путем введения иглы, подсоединенной к манометру, в мышечную ткань. Технический результат достигается тем, что способ измерения внутритканевого давления в фалангах пальцев кисти включает маркировку точек вкола на ладонной поверхности по дистальному краю межфаланговой и/или пястно-фаланговой складки в проекции края сухожилий сгибателя пальцев, а на тыльной поверхности в проекции дистального края коллатеральной связки пястно-фалангового и/или межфалангового сустава по латеральному или медиальному краю сухожилий разгибателей пальцев. После чего осуществляют вкол иглы в дистальном направлении под острым углом, после прокола кожи иглу вводят в подкожно-жировую ткань параллельно кости и снимают показания с манометра. Способ измерения внутритканевого давления фаланг пальцев кисти осуществляется следующим образом. На пораженной фаланге отмечают точки - это места вколов для инвазивного измерения внутритканевого давления, на ладонной поверхности по дистальному краю межфаланговой и/или пястно-фаланговой складки в проекции края сухожилий сгибателя пальцев, а на тыльной поверхности в проекции дистального края коллатеральной связки пястно-фалангового и/или межфалангового сустава по латеральному или медиальному краю сухожилий разгибателей пальцев. После чего осуществляют вкол иглы в дистальном направлении под острым углом, после прокола кожи иглу вводят в подкожно-жировую ткань параллельно кости и снимают показания с манометра. Данный способ применен у 17 больных в период с 2018 по 2019 годы. У 9 пациентов было диагностировано повышение внутритканевого давления выше 30 мм рт.ст. и выполнена декомпрессия, у 5 пациентов - от 20 до 30 мм ртутного столба, у 3 пациентов - менее 20 мм ртутного столба - этим пациентам декомпрессию не производили.This method was applied in 17 patients in the period from 2018 to 2019. Пациент И., 56 лет, находился на лечении в хирургическом отделении ФКУ «ЦВКГ им. П.В. Мандрыка» МО РФ с диагнозом: «костно-суставной панариций проксимальной фаланги второго пальца левой кисти». Считает себя больным около 2 недель, когда повредил палец в быту. К врачу не обращался. В связи с усилением болей во втором пальце левой кисти обратился к хирургу поликлиники, был направлен на стационарное лечение. При поступлении состояние удовлетворительное. Локально: визуально в области средней фаланги второго пальца левой кисти определяется гиперемия, припухлость, локальная гипертермия, пальпация болезненная. Диагностирован костно-суставной панариций второго пальца левой кисти. До операции выполнена маркировка на проксимальной фаланге второго пальца левой кисти точек вкола иглы для измерения внутритканевого давления. Проведено измерение на тыльной поверхности и ладонной поверхности внутритканевого давления. Результат 35 мм и 37 мм соответственно ртутного столба. Диагностирован острый тканевой гипертензионный синдром. Помимо операции (санация, некрэктомия, дренирование) выполнены декомперссионные разрезы по боковым поверхностям проксимальной фаланги второго пальца левой кисти. Рана заживала вторичным натяжением. Выписан с рекомендациями под наблюдение хирурга поликлиники. Контрольный осмотр через 1 месяц.
