При коррекции миопии посредством эксимерного лазера формируется более плоская поверхность роговицы. Обычная роговица имеет форму вытянутого эллипсоида, а после стандартной операции при коррекции миопии поверхность роговицы может стать сферической или приобрести форму сплюснутого эллипсоида. Это приводит к возникновению сферических аберраций при расширенном зрачке, обусловленных вновь сформированной поверхностью роговицы после операции, что наиболее существенно сказывается на качестве сумеречного и ночного зрения.When correcting myopia with an excimer laser, a flatter corneal surface is formed. Коррекция миопии с формированием роговицы в форме выпуклого эллипсоида вращения с отрицательной конической константной позволяет предкомпенсировать возникающие в результате коррекции положительные сферические аберрации созданием отрицательных сферических аберраций на периферии оптической зоны. Такая предкомпенсация сферических аберраций позволяет оптимизировать корнеальный волновой фронт, существенно расширить уплощение волнового фронта на периферии оптической зоны. Если все аберрации глаза скомпенсированы, то волновой фронт глаза, измеренный аберрометром, является плоским. Для решения поставленной задачи предложен способ лазерной коррекции миопии до - 8 диоптрий, при котором определяют сфероэквивалент рефракции глаза, определяют среднюю кератометрию глаза, воздействуют излучением эксимерного лазера «Микроскан Визум» с длиной волны 193 нм, энергией в импульсе 0,7 мДж, диаметром лазерного пятна 0,8 мм, длительностью импульса 7 нс, частотой следования импульсов до 300 Гц на роговицу глаза с формированием оптической зоны как оптической поверхности в виде выпуклого эллипсоида вращения с отрицательной конической константой, при кератометрии большей 40 и меньшей или равной 42 диоптриям формируют выпуклый эллипсоид вращения с отрицательной конической константой от -0,2 до -0,34, причем, чем больше модуль сфероэквивалента рефракции глаза, тем больше модуль конической константы формируемого эллипсоида вращения, при кератометрии большей 42 и меньшей или равной 45 диоптриям формируют выпуклый эллипсоид вращения с отрицательной конической константой от -0,18 до -0,4, причем, чем больше модуль сфероэквивалента рефракции глаза, тем больше модуль конической константы формируемого эллипсоида вращения, а при кератометрии большей 45 и меньшей или равной 49 диоптриям формируют выпуклый эллипсоид вращения с отрицательной конической константой от -0,24 до -0,12, причем, чем больше модуль сфероэквивалента рефракции глаза, тем меньше модуль отрицательной конической константы. Если кератометрия больше 42 и меньше или равна 45 диоптриям, роговица считается нормальной. Это позволит получить высокие показатели остроты и качества зрения в условиях различной степени освещенности. При кератометрии от 40 до 42 диоптрий роговица считается «плоской», коническая константа составляет от -0,2 до -0,34 для плоской роговицы, причем, чем больше модуль сфероэквивалента, тем больше модуль конической константы. Это позволяет получить более плоский волновой фронт на периферии оптической зоны, обеспечить заданную величину функциональной оптической зоны и соответственно обеспечить высокие показатели остроты и качества зрения в условиях различной освещенности для глаз с плоской роговицей. При кератометрии от 45 до 49 диоптрий роговица считается крутой, у такой роговицы сравнительно небольшой радиус. Для получения хорошего зрения после операции на роговице с кератометрией более 45 диоптрий при эксимерлазерном воздействии на роговице формируют выпуклый эллипсоид вращения с модулем отрицательной конической константы от -0,24 до -0,12, причем при большем значении модуля сфероэквивалента, значение модуля конической константы меньше. Высокое значение модуля конической константы при коррекции миопии с большим модулем значения сфероэквивалента на крутой роговице может привести к уменьшению функциональной оптической зоны. The approach to the formation of an aspherical (elliptical) postoperative cornea shape, taking into account the patient's degree of myopia and the conical constant, can be used for other methods of vision correction, for example, for vision correction by forming a lenticle in the cornea with a femtosecond laser beam and then removing the lenticule.
