Изобретение относится к области медицины, а именно оториноларингологии и относится к способам ускоренного обучения врачей-оториноларингологов базовым навыкам эндоскопической эндоназальной хирургии с помощью тренажера Sinus Model Otorhino-Neuro Trainer (S.I.M.O.N.T).The invention relates to medicine, namely otorhinolaryngology, and relates to methods of accelerated training of otorhinolaryngologists in the basic skills of endoscopic endonasal surgery using the Sinus Model Otorhino-Neuro Trainer (S.I.M.O.N.T.).
В настоящее время для практического обучения основам эндоскопической эндоназальной хирургии используется целый ряд учебных методик, которые требуют длительного периода обучения:Currently, a number of educational techniques are used for practical training in the basics of endoscopic endonasal surgery, which require a long training period:

При использовании стандартных методик, закрепившийся в процессе длительного обучения навык, часто содержит ошибочные приемы и стереотипные действия, затрудняющие усвоение правильной техники эндоскопических эндоназальных манипуляций. Кроме того, для сохранения навыка им следует систематически пользоваться, в противном случае возникает деавтоматизация навыков - ослабление или даже полное разрушение выработанных умений.All these teaching methods have a disadvantage, which is associated with the inability to develop a clear skill of coordinated hand movements in a short period of time, which negatively affects the need for endoscopic endonasal manipulations in real conditions, when you have to work with both hands in different planes at the same time. В качестве ближайшего аналога выбрана методика отработки основ эндоскопической эндоназальной хирургии с помощью тренажера S.I.M.O.N.T. Поставленная задача решается благодаря тому, что в способе ускоренного обучения базовым хирургическим навыкам эноскопических эндоназальных манипуляций, включающем отработку автоматического навыка технического упражнения путем многократного последовательного попадания с помощью длинной иньекционной иглы под контролем эндоскопа в 14 тренировочных точек, расположенные на значимых структурах полости носа, на симуляторе S.I.M.O.N.T., нет необходимости смены съемного модуля лицевой части черепа. Способ обучения основан на многократном повторении тестового задания, он обеспечивает эффективное освоение навыков координированных движений обеими руками на субсенсорном (неосознанном) и сенсорном (осознанном) уровнях, что уменьшает вероятность технических ошибок во время проведения эндоскопического осмотра полости носа и эндоскопических эндоназальных операций.The problem is solved due to the fact that in the method of accelerated training in basic surgical skills of enoscopic endonasal manipulations, including the development of an automatic skill of a technical exercise by repeated successive hits with a long injection needle under the control of an endoscope at 14 training points located on significant structures of the nasal cavity, on a simulator SIMONT, there is no need to change the removable skull face module. Сущность предложенного способа заключается в следующем: на предварительной стадии преподаватель демонстрирует суть упражнения и просит занимающегося повторить. Навык считается освоенным, если занимающийся попадает в предложенные точки в течение 60 сек в строго определенном порядке, без потери инструмента из вида и без промахов. Заявляемый способ опробован в процессе обучения основам базовых навыков врачей с различным стажем работы и хирургическим опытом.The inventive method was tested in the process of teaching the basics of basic skills of doctors with various length of service and surgical experience.
Для оценки эффективности данного тренировочного упражнения на тренажере S.I.M.O.N.T. было проведено исследование, в котором приняли участие 16 врачей-оториноларингологов без опыта выполнения эндоназальных эндоскопических операций. Занятия на тренажере проходили в течение 16 академических часа в течение 2 дней. Все этапы исследования фиксировали методом наружной видеосъемки. Оценку мануальных навыков проводили до и по окончании тренинга на основании времени выполнения упражнений и количества технических ошибок, допущенных при выполнении упражнений.To evaluate the effectiveness of this training exercise on the S.I.M.O.N.T. A study was conducted in which 16 otorhinolaryngologists with no experience in performing endonasal endoscopic operations took part. The simulator sessions lasted 16 academic hours during 2 days. All stages of the study were recorded by the method of outdoor video filming. При сравнении полученных данных уже через 7 часов тренинга наблюдали статистически значимое уменьшение времени выполнения упражнений (р<0,05), а также статистически значимое снижение количества ошибок, допущенных при выполнении упражнений (р<0,05), что свидетельствует об эффективности предложенного способа.Results. Разработанный способ ускоренного обучения базовым навыкам эноскопических эндоназальных манипуляций, включающем отработку автоматических навыков технического упражнения путем многократного последовательного попадания с помощью длинной иньекционной иглы под контролем эндоскопа в 14 тренировочные точки, расположенные на значимых структурах полости носа, на симуляторе S.I.M.O.N.T. позволяет добиться полного освоения моторно-координационного навыка работы обеими руками (эндоскоп + инструмент) в полости носа в течение 7 академических часов (р<0,05).Conclusions. The developed method of accelerated teaching of the basic skills of enoscopic endonasal manipulations, including the development of automatic skills of technical exercise by repeated sequential hits with a long injection needle under the control of an endoscope at 14 training points located on significant structures of the nasal cavity on the S.I.M.O.N.T. allows to achieve complete mastering of the motor-coordinating skill of working with both hands (endoscope + instrument) in the nasal cavity within 7 academic hours (p <0.05).
