Заболевания костно-мышечной системы, приводящие к временной или постоянной утрате трудоспособности, занимают лидирующие позиции в современном обществе. Высокая распространенность заболеваний тазобедренного сустава в детском возрасте, частое наступление инвалидности в работоспособном возрасте, потребность в длительном и дорогостоящем лечении определяют медико-социальную значимость проблемы их лечения. Наибольшая частота обращений к детскому ортопеду связана именно с диспластическими и дегенеративными поражениями тазобедренных суставов [1]. Среди дегенеративных заболеваний, приобретенных в период роста, наиболее распространена болезнь Легг-Кальве-Пертеса [2]. В структуре ортопедической патологии в общей популяции данная нозология составляет 0,17-1,9%, в детской популяции 25-30%. Данное заболевание часто встречается у детей 2–12 лет, в 3–4 раза чаще у мальчиков, двустороннее поражение отмечают в 10–15 % случаев [3, 4]. Частота встречаемости асептического остеонекроза головки бедренной кости в среднем составляет 0,4–20,9 : 100 000 [5]. Болезнь Легг-Кальве-Пертеса представляет собой идиопатический остеонекроз головки бедра с последующими осложнениями в виде деформации и развития остеоартроза [6]. Из данных литературы известно, что наиболее частым и значимым дефектом при дисплазии тазобедренного сустава является недоразвитие крыши вертлужной впадины, в частности ее верхнепереднего края, которое в 40-60% случаев требует хирургической коррекции [7].Diseases of the musculoskeletal system leading to temporary or permanent disability occupy a leading position in modern society. The greatest frequency of visits to a pediatric orthopedist is associated with dysplastic and degenerative lesions of the hip joints [1]. Among the degenerative diseases acquired during the growth period, the most common Legg-Calve-Perthes disease [2]. In the structure of orthopedic pathology in the general population, this nosology is 0.17-1.9%, in the pediatric population 25-30%. This disease is often found in children 2-12 years old, 3-4 times more often in boys, bilateral damage is noted in 10-15% of cases [3, 4]. The frequency of occurrence of aseptic osteonecrosis of the femoral head is on average 0.4–20.9: 100,000 [5]. Legg-Calve-Perthes disease is an idiopathic osteonecrosis of the femoral head with subsequent complications in the form of deformation and development of osteoarthrosis [6]. From the literature data it is known that the most common and significant defect in hip dysplasia is the underdevelopment of the roof of the acetabulum, in particular its upper front edge, which requires surgical correction in 40-60% of cases [7].
Лечение дисплазии тазобедренных суставов предусматривает применение как консервативных, так и оперативных методов [8]. Однако существующая возможность консервативного ведения пациентов с данной патологией, несмотря на достижение хороших результатов в случае раннего применения методов, не обеспечивает полную коррекцию - у ряда больных остается недоразвитие крыши вертлужной впадины, выполняется избыточная антеверсия и вальгусная деформация шейки бедра, констатируется подвывих бедра. В подобных ситуациях для создания условий нормального развития тазобедренного сустава необходима своевременная коррекция соотношений в тазобедренном суставе с целью достижения конгруэнтности между головкой бедра и крышей вертлужной впадины с помощью хирургических методик [9, 10]. Применение хирургического метода считается типичным у детей от двух лет и старше. Допускается оперативное лечение с 6 месяцев при наличии тяжелой степени болезни (выраженное недоразвитие вертлужной впадины, высокий вывих) [8, 11]. При лечении диспластического коксартроза у детей актуально применение хирургических методов лечения, которые условно можно разделить на три группы: остеотомия проксимального отдела бедренной кости, транспозиция большого вертела, корригирующая остеотомия для нормализации соотношений тазового компонента и бедренной кости. Наиболее адекватным хирургическим лечением диспластического коксартроза является дополнение недостающих анатомических элементов алло- или гомотрансплантатами.The treatment of hip dysplasia involves the use of both conservative and surgical methods [8]. In such situations, to create conditions for the normal development of the hip joint, timely correction of the ratios in the hip joint is necessary in order to achieve congruence between the femoral head and the acetabulum roof using surgical techniques [9, 10]. The use of the surgical method is considered typical in children from two years of age and older. Surgical treatment is allowed from 6 months in the presence of a severe degree of the disease (pronounced underdevelopment of the acetabulum, high dislocation) [8, 11]. In the treatment of dysplastic coxarthrosis in children, it is important to use surgical methods of treatment, which can conditionally be divided into three groups: osteotomy of the proximal femur, transposition of the greater trochanter, corrective osteotomy to normalize the ratios of the pelvic component and the femur. The most appropriate surgical treatment for dysplastic coxarthrosis is the addition of missing anatomical elements with allo or homografts.

Существующие методики нацелены, в основном, на планирование операций остеотомии проксимального отдела бедренной кости. Существует необходимость дооперационного планирования объема оперативного вмешательства при хирургическом лечении диспластического коксартроза у детей путем коррекции ацетабулярного компонента тазобедренного сустава. Существует способ хирургической коррекции проксимального отдела бедренной кости методом межвертельной остеотомии с предварительным расчетом угла антеторсии шейки бедренной кости и длины поворота бедренной кости с помощью компьютерной томографии на этапе предоперационного планирования[12]. Угол деротации вычисляют исходя из значений истинного угла антеторсии и возрастной нормы антеторсии. Необходимая длина поворота определяется исходя из угла деротации, а также среднего диаметра окружности бедренной кости по предлагаемой авторами формуле. Известен способ планирования оперативного лечения при нестабильности тазобедренного сустава при врожденном вывихе бедра, учитывающий как бедренный, так и тазовый компонент[13]. Метод заключается в рентгенологическом исследовании ТБС (тазобедренного сустава) пациента в положении под углом 65° между фронтальной плоскостью таза и экраном, оценке угла переднего покрытия головки бедра. Дополнительно выполняют рентгенограмму при максимальной внутренней ротации пораженной конечности с последующим сравнением значений углов переднего покрытия головки бедра, заключенного между лучами от центра головки вертикально и к точке переднелатерального края свода впадины. Исходя из результатов расчета, планируют корригирующую (деторсионную) остеотомию бедра в самостоятельном варианте, либо при патологических значениях рассчитанного угла переднего покрытия головки бедра планируют коррекцию положения как тазового, так и бедренного компонента ТБС. Недостатком метода является также отсутствие визуального контроля вычислений; необходимость сложной укладки, что затрудняет применение метода при обследовании детей раннего возраста.There is a method of planning surgical treatment for instability of the hip joint with congenital dislocation of the hip, taking into account both the femoral and pelvic components [13]. Наиболее близким является способ корригирующей остеотомии бедренной кости, включающий 3D моделирование, печать модели на 3D принтере и выполнение корригирующей остеотомии бедренной кости[14]. Дополнительно в 3D программе рассчитывают угол антеверзии, а также шеечно-диафизарный угол. В ходе хирургического вмешательства, после поднадкостничного выделения бедренной кости, на межвертельную область надевают шаблон, по которому на бедренную кость наносят линии остеотомии и детерсионные метки, затем выполняют корригирующую остеотомию бедренной кости, фрагменты бедра сопоставляют, для коррекции угла антеверзии дистальный отдел бедренной кости ротируют относительно оси диафиза до сопоставления детерсионных меток. Недостатками данного метода является сложность реализации, необходимость обученного 3D моделированию персонала, ограниченная применимость в связи с финансовыми затратами при реализации. Кроме того, данный метод рассматривает корригирующую остеотомию бедренной кости, но не затрагивает необходимость коррекции тазового компонента при диспластическом коксартрозе. Сущность предлагаемого метода реализуется следующим образом. На этапе планирования оперативного вмешательства выполняют рентгенологическое исследование тазобедренных суставов в стандартной укладке пациента, после осуществляют инверсию рентгенологического снимка здорового тазобедренного сустава и наложение его изображения на изображение больного сустава в программе Photoshop CC 2015.5. Следующим этапом определяют степень недостаточности ацетабулярного компонента исходя из угла Виберга, по полученным результатам определяют объем планируемого оперативного вмешательства, а именно рассчитывают угол расщепа при надацетабулярной остеотомии подвздошной кости и подбирают размер импланта.The essence of the proposed method is implemented as follows. Клинический пример осуществления способа. Пациентка Б., 12 лет. Диагноз: диспластический коксартроз слева II степени с подвывихом головки бедренной кости. При поступлении: укорочение левой нижней конечности на 2 см, жалобы на быструю утомляемость при опоре на левую нижнюю конечность и нарушение походки – прихрамывание на левую ногу. Положительный симптом Тренделенбурга. В 6 лет пациентка перенесла болезнь Легг-Кальве-Пертеса с неблагоприятным исходом в виде деформации головки тазобедренного сустава (ТБС) с формированием диспластических изменений. В процессе роста и развития ребенка появились вторичные изменения со стороны крыши ТБС и подвывих головки бедренной кости. Боли и дискомфорт в области левого ТБС стали беспокоить последние 4 месяца, когда пациент прибавила в весе. Выполнена рентгенография ТБС пациента Б. в стандартной укладке. Рентгенологически определяется недоразвитие наружного края крыши вертлужной впадины слева. АИ – 55°, ШДУ 130°, антеторсия слева – 55°, угол Виберга 11°, коэффициент покрытия слева – 0,5, индекс покрытия – 4. Дополнительно в программе Photoshop CC 2015.5 выполнена инверсия и наложение изображения здорового ТБС на изображение пораженного сустава. Степень недостаточности ацетабулярного компонента определена исходя из угла Виберга, инверсионное наложение снимка позволило визуализировать картину. Расчет угла расщепа при надацетабулярной остеотомии подвздошной кости и подбор размера импланта выполнен исходя из полученных результатов исследования.A clinical example of the method. Underdevelopment of the outer edge of the acetabulum roof on the left is determined by X-ray. AI - 55 °, ШДУ 130 °, anthorsion on the left - 55 °, Viberg angle 11 °, coverage coefficient on the left - 0.5, coverage index - 4. Additionally, in Photoshop CC 2015.5, the image of a healthy TBS was inverted and superimposed on the image of the affected joint . The degree of insufficiency of the acetabular component is determined based on the Wiberg angle, and the inverse overlay of the image made it possible to visualize the picture. The calculation of the cleavage angle with nadacetabular osteotomy of the ilium and the selection of the size of the implant is based on the results of the study.
Проведена операция внесуставной надацетабулярной остеотомии подвздошной кости с моделированием ацетабулярного компонента имплантом из пористого никелида титана. Рана ушита послойно, швы сняты на 10-е сутки после оперативного лечения. Наложена кокситная гипсовая повязка сроком на 6 недель. После снятия гипсовой повязки проведен стандартный курс восстановительного лечения, включающий физиотерапию, массаж, лечебную физкультуру и кинезиотейпирование. Через 3 месяца после операции – минимальная дозированная нагрузка, через 4 месяца ходьба на костылях, через 5 месяцев – ходьба с тростью. Полная нагрузка на разрешена через 6 месяцев. Пациентка осмотрена через год. Функциональная способность конечности восстановлена. Full load is allowed after 6 months. The patient was examined after a year. The functional ability of the limb is restored. An X-ray examination demonstrates a formed biocomposite bone-porous implant bordering the joint in the form of a semicircle, layering as a single block. Head coverage is complete. There are no signs of implant loosening.
