Роль Toll-подобных рецепторов в патогенезе воспалительных заболеваний бронхолегочный системы // Инфекция и иммунитет. - 2016. - Т. 6, №2. - С. 109-116). В научно-медицинской литературе имеются сведения об исследовании экспрессии Toll-подобных рецепторов TLR2 и TLR4 у больных с рецидивом язвенного колита (Мамедова Л.Н., Тарасова Г.Н., Чумакова Е.А., Столярова И.Г. Динамическое определение Toll-рецепторов у больных с язвенным колитом // Медицинский вестник Юга России. - 2013. - С. 90). Нами были сформированы 2 группы: 34 больных с подтвержденным диагнозом ЭСО и контрольная группа из 31 здорового человека - всего 65 человек в возрасте от 3 до 56 лет, обратившихся в оториноларингологическое отделение Муниципального бюджетного учреждения здравоохранения Городской больницы №1 им. Н.А. Семашко, г. Ростова-на-Дону (МБУЗ). Проводили забор периферической крови из локтевой вены пациентов. Выделяли моноциты периферической крови по методике Рекалде, описанной в работе Л.В. Ковальчук и соавт. (Иммунология. Практикум: учебное пособие - найдено на сайте http://kursak.net/immunologiya-praktikum-ucheb-posobie-kovalchuk-l-v-i-dr-2010). На выделенных моноцитах определяли экспрессию Toll-подобных рецепторов, %: TLR2 и TLR4 по методике, описанной в работе Л.Н. Мамедовой и соавт. (Мамедова Л.Н., Тарасова Г.Н., Чумакова Е.А., Столярова И.Г. Динамическое определение Toll-рецепторов у больных с язвенным колитом // Медицинский вестник Юга России. - 2013. - С. 91). С помощью программы Statistica 6.0 (StatSoft Inc., США) провели статистическую обработку результатов исследования. В результате статистической обработки результатов исследования были получены статистически достоверные (p<0,05) максимальные значения экспрессии исследуемых Toll-подобных рецепторов для группы больных ЭСО: TLR2=56,1% и TLR4=16,2%, что позволило сформулировать статистически достоверный критерий диагностики ЭСО: одновременное выполнение условий TLR2≤56,1% и TLR4≤16,2%.We have formed 2 groups: 34 patients with a confirmed diagnosis of ESP and a control group of 31 healthy people - only 65 people aged 3 to 56 years who applied to the otorhinolaryngological department of the Municipal Budgetary Healthcare Institution of the Municipal Hospital №1 named after. As a result of statistical processing of the study results, statistically significant (p <0.05) maximum expression values of the studied Toll-like receptors were obtained for a group of patients with ESP: TLR2 = 56.1% and TLR4 = 16.2%, which made it possible to formulate a statistically significant criterion diagnostics of ESP: simultaneous fulfillment of the conditions TLR2≤56.1% and TLR4≤16.2%.

Пример 1: пациент И., 4 лет, поступил в оториноларингологическое отделение для детей МБУЗ с жалобами со слов мамы на затрудненное носовое дыхание, храп в течение 1 года, что связывают с началом посещения детского дошкольного учреждения. В течение года ребенка беспокоит частый насморк. Последние 6 месяцев, особенно на фоне насморка, мама стала замечать косвенные признаки снижения слуха - частое переспрашивание, периодически возникающее отсутствие реакции ребенка на обращенную к нему речь, просмотр телевизора на высокой громкости. Пациенту И. была проведена эндоскопия полости носа, носоглотки, фарингоскопия, отоскопия. В носоглотке обнаружены аденоиды 2 степени гипертрофии с признаками хронического воспаления. Из анамнеза известно, что в течение трех лет пользуется сосудосуживающими каплями в нос. В детстве - частые насморки, ангины. Пациентке Б. была проведена эндоскопия полости носа, носоглотки, фарингоскопия, отоскопия. При эндоскопическом осмотре в полости носа обнаружена гипертрофия нижних носовых раковин на всем протяжении. С помощью заявляемого способа нами было обследовано 72 пациентов, 40 детей и 32 взрослых, в возрасте от 3 до 56 лет, поступивших в оториноларингологические отделения МБУЗ. Из 72 пациентов у 39 человек значение TLR2 было в пределах от 56,1% и ниже, значение TLR4 - от 16,2% и ниже. Поскольку у этих 39 обследованных пациентов одновременно выполнялось условие TLR2≤56,1% и TLR4≤16,2%, им был диагностирован экссудативный сред- TLR2≤56,1% и TLR4≤16,2%, им был диагностирован экссудативный средний отит.
