Известен способ прогнозирования госпитальных осложнений при операциях на открытом сердце с использованием шкалы STS [The Society of Thoracic Surgeons National Adult Cardiac Surgery Database). С его помощью определяют в процентах вероятность развития таких неблагоприятных событий, как послеоперационная летальность, увеличение срока госпитализации, развитие острого нарушения мозгового кровообращения, длительность искусственной вентиляции легких, инфекционные осложнения, почечную недостаточность, риск проведения повторной операции. Недостатком способа является его трудоемкость при расчетах, так как используется широкий набор критериев: возраст, пол, рост, вес, расовая принадлежность, проявления сердечной недостаточности в течение последних 2-х недель, применение диализа, уровень креатинина крови, наличие стабильной/нестабильной стенокардии, ОКС с подъемом ST или без подъема ST (при поступлении в клинику или непосредственно перед операцией), перенесенный инфаркт миокарда, нарушения ритма, хронические заболевания легких, цереброваскулярные заболевания, признаки периферического атеросклероза, наличие сахарного диабета, артериальной гипертензии, инфекционного эндокардита, применение иммуносупрессивной терапии, срочность оперативного вмешательства (плановая, экстренная, отсроченная), количество пораженных коронарных артерий, проведение реанимационных мероприятий, кардиогенный шок, применение внутриаортальной баллонной контрпульсации (ВАБК), инотропных препаратов, поражение клапанного аппарата сердца, предшествующая операция на сердце.A known method for predicting hospital complications in open heart surgery using the STS scale [The National Adult Cardiac Surgery Database). На основании отечественных и литературных данных проанализировано влияние факторов риска развития абдоминальных осложнений, выделены ведущие факторы риска абдоминальных осложнений для разработки системы прогнозирования на основании периоперационных предикторов для выделения среди пациентов группы высокого и очень высокого риска. Выделение среди пациентов лиц с высоким и очень высоким риском абдоминальных осложнений позволит проводить ранний скрининг и применить алгоритм лечебно-диагностических мероприятий для профилактики и лечения данной патологии.On the basis of domestic and literature data, the influence of risk factors for the development of abdominal complications is analyzed, the leading risk factors of abdominal complications are identified for developing a forecasting system based on perioperative predictors for identifying high and very high risk patients among patients. В исследование включены 6586 пациентов (5164 мужчин и 1422 женщин), которым в период с 2011 по 2017 г.были выполнены операции на сердце в кардиохирургических отделениях ФГБУ «ФЦССХ» Минздрава России (г. Челябинск). Для оценки значимости факторов производился расчет относительного риска. Построение модели для интегральной оценки риска абдоминальных осложнений производили с помощью многофакторного логистического регрессионного анализа с пошаговым включением переменных с последующим присвоением баллов избранным факторам. Для разделения больных на группы риска анализировали ROC-кривую для суммы баллов. Наиболее часто встречались желудочно-кишечные кровотечения - 42,5% (31 пациент) и динамическая кишечная непроходимость 37,0% (27 пациентов), реже - острый бескаменный (9,6% - 7 пациентов) и калькулезный холецистит (6,8% - 5 пациентов), мезентериальный тромбоз (8,2%, 6 пациентов), острый панкреатит (4,1%, 3 пациента).The frequency of abdominal complications in our population was 1.11% (73 patients out of 6586). Для упрощения использования данной модели на практике, каждому фактору был присвоен определенный балл (таб. 5), отражающий степень его вклада в итоговый риск. Для каждого обследованного больного мы рассчитали сумму баллов по предложенной нами шкале и точную предсказанную вероятность в соответствии с построенной моделью логистической регрессии. Далее мы провели с рассчитанными показателями ROC-анализ, использовав наличие/отсутствие осложнений в качестве переменной исхода (фиг.1, таблица 6). ROC-кривая для показателя «сумма баллов по шкале» практически совпадает с таковой для предсказанной вероятностью и обладает хорошей диагностической ценностью (AUC=0,805±0,033,p<0,001).For each patient examined, we calculated the sum of points on the scale we proposed and the exact predicted probability in accordance with the logistic regression model constructed. Группа умеренного риска - это пациенты, набравшие от 1 до 6 баллов, риск абдоминальных осложнений у них несколько выше, чем в популяции в целом. В группе высокого риска можно выделить небольшое количество пациентов, набравших 13 и более баллов (45 человек или 0,7% популяции), имеющих крайне высокий риск абдоминальных осложнений (31,1%)High risk group - patients who scored 7 or more points. Пример №1. Больной А., 57 лет, с диагнозом: ИБС. Стенокардия III ФК. ХСН I ФК II. Гипертоническая болезнь III ст., риск 4. По данным коронарографии выявлены стенозы передней межжелудочковой артерии - 80%, огибающей артерии - 85%, окклюзия правой коронарной артерии, госпитализирован для планового коронарного шунтирования с использованием искусственного кровообращения. У пациента не было мультифокального атеросклероза, сахарного диабета, постинфарктного кардиосклероза, инсульта. ФВ 60%. Оценка риска неблагоприятного исхода коронарного шунтирования по данным шкалы EuroSCORE II составила 0,33%, что соответствует низкому риску. Послеоперационный период протекал гладко (не потребовалось применения ЭКМО, ВАБК, вазопрессоров, рестернртомии, отсутствовала фибрилляция предсердий - по данным ЭКГ синусовый ритм). Риск развития абдоминальных осложнений был низкий (сумма баллов - 0). В раннем послеоперационном течении абдоминальных осложнений не зафиксировано, пациент выписан на 7-е сутки на второй этап реабилитации.
Пример №2. Больной Б., 63 лет, с диагнозом: ИБС. Стенокардия III ФК. Постинфарктный (2013) кардиосклероз. ХСН I ФК II. Гипертоническая болезнь III ст, риск 4. По данным коронарографии выявлены стенозы передней межжелудочковой артерии - 90%, ветви тупого края огибающей артерии - 70%, задней межжелудочковой ветви правой коронарной артерии 90%, госпитализирован для планового коронарного шунтирования с использованием искусственного кровообращения. ФВ 46%. Значение EuroSCORE II составило 1,42% (низкий риск). В ранней послеоперационном периоде выполнялась рестренотомия для остановки кровотечения из места формирования анастомоза между аутовенозным шунтом и аортой. Риск развития абдоминальных осложнений был высоким (сумма баллов - 9). На 4 сутки послеоперационного периода у пациента появились боли в правом подреберье, тошнота, двухкратная рвота. Выполнено ультразвуковое исследование и МСКТ органов брюшной полости: с контрастированием мезентериальных сосудов. Установлен диагноз острый бескаменный холецистит. В виду безуспешной консервативной терапии пациенту выполнена холецистэктомия. В дальнейшем послеоперационный период протекал без осложнений. In view of the unsuccessful conservative therapy, the patient underwent cholecystectomy. In the future, the postoperative period was uneventful. Discharged on the 18th day of the second stage of rehabilitation.

В послеоперационном периоде пароксизм фибрилляции предсердий - купирован кордароном. Риск развития абдоминальных осложнений был умеренный (сумма баллов - 5). В раннем послеоперационном течении абдоминальных осложнений не зафиксировано, пациент выписан на 9-е сутки на второй этап реабилитации.Example number 3. Предложенная нами шкала стратификации риска развития абдоминальных осложнений после операций коронарного шунтирования в условиях искусственного кровообращения позволяет выделить группы очень высокого и высокого риска развития абдоминальных осложнений, которые требуют проведения дополнительных профилактических и лечебно-диагностических мероприятий, для уменьшения послеоперационных осложнений и летальных исходов.The proposed scale of stratification of the risk of developing abdominal complications after coronary artery bypass surgery under conditions of artificial blood circulation makes it possible to identify groups of very high and high risk of developing abdominal complications that require additional preventive and therapeutic and diagnostic measures to reduce postoperative complications and deaths.
