The structure of these drains can not completely eliminate the reflux of urine from the bladder to the kidney.
(http://www.medp.spb.ru/production/catalog/imn/katmo.htm).The closest in technical essence is a single-use ureteral polymer radiopaque catheter, which is represented by a tube 710 mm long, made of flexible plastic, equipped with a rigid wire mandrel (conductor) and a centimeter scale, with a diameter from 3 to 8 indicating diameter of the tube from 1 mm to 2.66 mm, respectively. In order to avoid damage to the soft tissue when installing at the end of the tool has a spherical profile. Thanks to the ring marks, the depth of entry of the device is regulated, and the 2 holes on the sides allow you to remove urine and inject drugs. (http://www.medp.spb.ru/production/catalog/imn/katmo.htm).
However, this catheter has two openings, which increases the risk of embolization of the catheter with blood clots and mucus, and this in turn entails the need for re-catheterization of the ureter, which is traumatic and painful.
The task is to improve the drainage properties of the ureteral catheter, reducing trauma.
The task is achieved by a perforated ureteral catheter consisting of a tube with a length of 710 mm with a spherical end containing a centimeter scale and five drainage holes at the distal end, arranged in a staggered manner, in two planes, 1 cm apart from each other, with a total drainage surface 5 cm.
(1, 3, 5).The figure 1, front view, presents a perforated ureteral catheter - 1, where 2 - centimeter scale, 3 - draining holes (1, 3, 5).
(2, 4).Figure 2 shows its rear view, where 3 are drainage holes (2, 4).
The perforated ureteral catheter, 710 mm long, with a spherical end, is made of flexible plastic, has five holes, arranged in a staggered manner, in two planes, at its distal end, at a distance of 1 cm from each other.
The total drainage surface of the catheter is 5 cm, which significantly improves the drainage of the upper urinary tract, especially in female patients, taking into account the physiological features of their upper urinary tract. In order to avoid damage to the soft tissue when installed at the end of the catheter has a spherical profile. Thanks to the ring marks, centimeter scale, adjust the depth of the instrument. Hard wire mandrel, conductor, stiffens the catheter as it progresses along the urinary tract (IMP). After installation, mandrin is removed. Depending on the functional characteristics of the ureter, the tubes have numbers from 3 to 8, corresponding to diameters from 1 mm to 2.66 mm.
The use of the proposed "Perforated ureteral catheter" is as follows.
Under the conditions of the cystoscopic cabinet, observing all the rules of septic and antiseptics, a cystoscope is inserted into the bladder along the urethra, the ureteral orifice is visualized, into which the “Perforated Ureteral Catheter” is inserted, with a mandrel inside, 26 cm, after which the mandrin and cystoscope are completely removed, and catheterization of the bladder with a Foley-type urethral catheter. To hold the proposed catheter in the upper urinary tract, the catheters are fixed to each other with a tissue-based adhesive plaster.

According to the data of the control examination (ultrasound of the kidneys, CT of the kidneys with contrast enhancement), there is a positive trend, swelling of the parenchyma of the right kidney has decreased, the renal pelvis system of the right kidney has not been expanded. On the 11th day, the patient was discharged from the urology department in a satisfactory condition under the supervision of a urologist of the polyclinic at the place of residence for the purpose of dynamic observation of the patient.
Patient R., 57 years old, was examined and treated in the urology department with a diagnosis of Acute pyelonephritis on the right. As a result of the survey (renal ultrasound, CT of the kidneys with contrast enhancement), a pronounced edema of the right kidney parenchyma with foci of perfusion reduction of the contrast agent in the projection of the right kidney parenchyma was diagnosed. On the day of admission, the patient underwent right ureter catheterization with a “single-use ureteral ureteral catheter catheter” to drain the right pectoral-pelvic system of the right kidney, after which the pain syndrome was arrested, antibacterial therapy was prescribed. Hyperthermia was completely stopped on the 4th day after catheterization. On the 9th day, the catheter was removed, the independent urination was restored. Control tests are normal. On the 13th day, the patient was discharged from the urology department in a satisfactory condition under the supervision of a urologist of the polyclinic at the place of residence for the purpose of dynamic observation of the patient.
When carrying out catheterization of the renal pelvis system in the above examples, urine discharge through the ureteric catheters was noted with frequent drops, which confirms the presence of high intraocular pressure, which indicates the obstructive nature of pyelonephritis.
The use of "Perforated ureteral catheter" allows you to improve kidney drainage, inject drugs into the urinary tract, use for surgical interventions and manipulations, reduce the clinical time of the disease, patient stay in the hospital, which increases the medical and economic efficiency of treatment of patients with obstructive pathology of the upper urinary tract .
