На завершающем этапе операции на поверхность раны накладывали мембранную диализирующую повязку для животных, обеспечивая ее конгруэнтное прилегание и полное покрытие поверхности дефекта. После накладывания повязки на рану, ее фиксируют любым доступным способом, например при помощи адгезивного рулонного фиксирующего покрытия, предварительно выводя наружу растворообменные трубки, и заполняют полость на объема лекарственным гелем. По заявляемому способу пролечено 8 собак, 6 кошек, 12 коров и 10 лошадей, с наличием ожогов, обширных язв, поверхностных ран с потерей вещества, локализованных в области головы, дистальных участках конечностей. Оперативные вмешательства выполняли на фоне местной проводниковой анестезии. Поверхность раны очищалась от мертвых тканей, загрязнений, экссудата, инородных частиц, высушивалась и покрывалась мембранной диализирующей повязкой. Диализирующий гель вводили через растворообменные трубки, после фиксации мембраны на поверхности дефекта. During wound dialysis, dialysate was replaced once every 24–48 hours, as the dressing cavity was filled.
Основанием для прекращения диализа являлось улучшение общего состояния животного, уменьшение отечности, появление здоровых грануляций, исчезновение функциональных нарушений, нормализация лабораторных показателей. Длительность лечения составляла 4-5 суток. Клинические наблюдения в процессе лечения поверхностных плоскостных ран заявляемым способом показали следующее. Обезболивающий эффект наблюдали в течение первых часов после операции и начала диализа. У всех животных выявлено улучшение клинического состояния. Температура тела соответствовала нормальным значениям. На 2 сутки отечность заметно уменьшилась и окончательно исчезла к концу 2-3 суток. Инфильтрация тканей устранилась к 4-5 суткам. К 4-7 суткам у большинства животных поверхность ран полностью очистилась от некротизированных тканей и была покрыта нормальными ярко-розовыми плотными грануляциями. У всех животных в 1-5-7-ые сутки определялись клинические и биохимические показатели крови. Гематологические показатели при первичном осмотре, свидетельствовали о гиперэргическом течении воспалительной реакции. У всех видов животных наблюдали лейкоцитоз, гемоголобинемию, и сдвиги в электролитном балансе в сторону гиперкалиемии и гипокальциемии. У лошадей СОЭ возросла в 2,3 раза (115±0,09 мм/ч); лейкоциты - в 5,3 раза (53,9±0,02×109/л); эритроциты уменьшились в 1,4 (4,3±1,4±1012/л); гемоглобин - в 1,5 (98,0±13,6 г/л); калий возрос в 1,4 (5,2±2,3ммоль/л); кальций снизился в 1,4 (2,1±0,4 ммоль/л); общий белок - 49,7±4,2/л. У собак выявлено увеличение СОЭ - в 4,1 раза (25±0,09 мм/ч); лейкоцитов - в 3,9 раза (30,9±0,02×109/л); калия - в 1,4 (8,6±1,56 ммоль/л), при одновременном снижении эритроцитов - в 1,4 (4,3±1,4×1012/л); гемоглобина - в 1,5 (118,0±13,6 г/л); кальция - в 1,4 (1,5±0,4 ммоль/л); общего белка - 49,7±4,2/л. У крупного рогатого скота СОЭ повысилось в 3,9 раза (5,0±0,09 мм/ч); лейкоциты - в 3,9 раза (24,9±0,02×109/л); эритроциты понизились в 1,5 (2,9±0,61×1012/л); гемоглобин - в 1,6 (58,0±13,6 г/л). Отклонения от нормальных значений в уровне общего белка составили (54,7±4,2 г/л) (Р≤0,01), альбуминов (17,1±2,36%) (Р≤0,05); альфа глобулинов (24,11±0,05%). Соответствие гематологических показателей референтным значениям в опытной группе происходило: у лошадей на 12,3±2,3 сутки; собак - 11,7±3,1; крупного рогатого скота - на 19,1±3,3 сутки. Сроки полного заживления ран с прочной эпителизацией рубца составили: у кошек - 11,2±1,3 суток, у собак - 11,5±1,6 суток, у лошадей - 14,4±2,3, крупного рогатого скота - 20,1±4,2 (Р>0,01,0,001).The duration of treatment was 4-5 days. By 4-7 days in most animals, the surface of the wounds was completely cleared of necrotic tissue and was covered with normal bright pink dense granulations. In all animals, clinical and biochemical blood parameters were determined on days 1-5-7. Hematological indices at the initial examination indicated a hyper-inflammatory course of the inflammatory reaction. Leukocytosis, hemogolobinemia, and shifts in the electrolyte balance towards hyperkalemia and hypocalcemia were observed in all animal species. In horses, the ESR increased 2.3 times (115 ± 0.09 mm / h); leukocytes - 5.3 times (53.9 ± 0.02 × 10 9 / l); red blood cells decreased by 1.4 (4.3 ± 1.4 ± 10 12 / l); hemoglobin is 1.5 (98.0 ± 13.6 g / l); potassium increased by 1.4 (5.2 ± 2.3 mmol / l); calcium decreased by 1.4 (2.1 ± 0.4 mmol / l); total protein - 49.7 ± 4.2 / l. The dogs showed an increase in ESR - 4.1 times (25 ± 0.09 mm / h); leukocytes - 3.9 times (30.9 ± 0.02 × 10 9 / l); potassium - 1.4 (8.6 ± 1.56 mmol / l), while reducing red blood cells - 1.4 (4.3 ± 1.4 × 10 12 / l); hemoglobin - 1.5 (118.0 ± 13.6 g / l); calcium - 1.4 (1.5 ± 0.4 mmol / l); total protein - 49.7 ± 4.2 / l. In cattle, ESR increased 3.9 times (5.0 ± 0.09 mm / h); leukocytes - 3.9 times (24.9 ± 0.02 × 10 9 / l); red blood cells decreased by 1.5 (2.9 ± 0.61 × 10 12 / l); hemoglobin - 1.6 (58.0 ± 13.6 g / l). Deviations from normal values in the level of total protein were (54.7 ± 4.2 g / l) (P≤0.01), albumin (17.1 ± 2.36%) (P≤0.05); alpha globulin (24.11 ± 0.05%). The compliance of hematological parameters with reference values in the experimental group occurred: in horses for 12.3 ± 2.3 days; dogs - 11.7 ± 3.1; cattle - by 19.1 ± 3.3 days. The periods of complete wound healing with strong scar epithelization were: in cats - 11.2 ± 1.3 days, in dogs - 11.5 ± 1.6 days, in horses - 14.4 ± 2.3, cattle - 20 , 1 ± 4.2 (P> 0.01.0.001).
