Изобретение относится к медицине, а именно к сердечно - сосудистой хирургии, и может быть использовано для определения тандемного поражения внутренней сонной артерии у больных с хронической сегментарной окклюзией внутренней сонной артерии (ВСА).The invention relates to medicine, namely to cardiovascular surgery, and can be used to determine the tandem lesion of the internal carotid artery in patients with chronic segmental occlusion of the internal carotid artery (ICA).
При выполнении реконструктивных операций по поводу хронической атеросклеротической окклюзии ВСА может быть выявлен сегментарный характер окклюзии ВСА, т.е. окклюзирован только шейный сегмент ВСА, а просвет артерии перед входом в полость черепа и дистальнее свободен. При таком топическом поражении возможно восстановление магистрального кровотока по сонным артериям путем выполнения каротидной эндартерэктомии. Однако в послеоперационном периоде у данной категории пациентов имеется повышенный риск тромбоза и окклюзии ВСА из-за возможных редуцированных путей оттока (стеноз внутричерепной части ВСА). И перед хирургом, выполняющим операцию по восстановлению магистрального кровотока у больных с хронической сегментарной окклюзией ВСА путем выполнения каротидной эндартерэктомии, возникает во время операции вопрос о наличии у данного больного тандемного интракраниального поражения ВСА. При обнаружении значимого тандемного стеноза ВСА в интракраниальном сегменте необходимо выполнить стентирование этого сегмента для устранения второго блока и уменьшения риска тромбоза артерии в послеоперационном периоде.When performing reconstructive operations for chronic atherosclerotic occlusion of the ICA, the segmental nature of the ICA occlusion can be detected, i.e. occlusion of only the cervical segment of the ICA, and the lumen of the artery in front of the entrance to the cranial cavity and distal free. Недостатками способа являются: то, что на дооперационном периоде данная методика «работает» только при сохраненном кровотоке по ВСА, при окклюзии артерии - нет. Также это исследование невозможно выполнить во время операции, когда срочно требуется решить вопрос о наличии тандемного поражения и расширения объема операции. В большей части стационаров сосудистые операционные и данная диагностическая аппаратура (КТ, МРТ) находится в разных помещениях, и для проведения исследования необходима транспортировка пациента в другое помещение. При определении объемного кровотока по ВСА менее 150 мл/мин предполагалось, что у больного есть тандемное поражение внутричерепной порции ВСА. Уменьшение объемного кровотока по сонной артерии, после коррекции путей притока, связано с наличием препятствия в вышележащем сегменте (тандем-стеноз) ВСА. Этим пациентам выполняли рентгеноконтрастную ангиографию и стентирование тандем-стеноза за один наркоз. И наоборот, при объемном кровотоке по ВСА после эндартерэктомии более 150 мл/мин, предполагалось, что у больного нет тандемного стеноза. Данный способ диагностики тандемного поражения ВСА во время реконструктивной операции у больных с хронической сегментарной ее окклюзией применен в клинике факультетской хирургии Клиник СамГМУ у 12 пациентов. Пациенту выполнена эверсионная каротидная эндартерэктомия справа с восстановлением магистрального кровотока по ВСА. При интраоперационной флоуметрии после реконструкции объемный кровоток по ВСА оказался равен 210 мл/мин. Послеоперационный период протекал без особенностей. При УЗДГ БЦС и КТ-ангиографии после операции зона реконструкции без особенностей, данных за тандемное поражение интракраниального отдела ВСА нет.04/01/2013, the operation was performed. Пациенту выполнена эверсионная каротидная эндартерэктомия слева с восстановлением магистрального кровотока по ВСА. При интраоперационной флоуметрии после реконструкции объемный кровоток по ВСА оказался равен 90 мл/мин. При церебральной ангиографии выявлен 70% тандем-стеноз внутричерепной порции ВСА слева и стентирование этого сегмента артерии с полным восстановлением кровотока. Использование представленного способа определения тандемного поражения внутренней сонной артерии во время операции каротидной эндартерэктомии у больных с сегментарной окклюзией внутренней сонной артерии позволяет определить показания к выполнению церебральной ангиографии и выполнению стентирования тандемного поражения. Это позволяет уменьшить риск тромбоза сонной артерии после операции у данной категории пациентов.
