Предлагаемое изобретение относится к медицине, а именно к сосудистой хирургии, и может быть использовано для прогнозирования риска развития послеоперационных осложнений у больных с атеросклерозом сонных артерий.The present invention relates to medicine, namely to vascular surgery, and can be used to predict the risk of postoperative complications in patients with carotid atherosclerosis.
Одной из главных проблем мирового здравоохранения являются острые нарушения мозгового кровообращения. Ишемический инсульт широко распространен по всему миру, а его высокая заболеваемость и большой риск инвалидизации не позволяют переоценить его экономический и популяционный ущерб. Поэтому профилактика ишемического инсульта -одно из самых важных направлений современного здравоохранения. Эффективность проводимого лечения во много зависит от операционного риска, определяемого степенью сосудисто-мозговой недостаточности и сопутствующей патологией. Раннее выявление сопутствующих заболеваний позволяет улучшить течение послеоперационного периода и выбрать наиболее подходящий метод оперативного лечения.One of the main problems of world health is acute cerebrovascular accident. Ischemic stroke is widespread throughout the world, and its high incidence and high risk of disability do not allow to overestimate its economic and population damage. Therefore, the prevention of ischemic stroke is one of the most important areas of modern healthcare. The effectiveness of the treatment depends on the operational risk, which is determined by the degree of cerebrovascular insufficiency and associated pathology. Early detection of concomitant diseases can improve the course of the postoperative period and choose the most appropriate method of surgical treatment.
За аналог принят «Способ прогнозирования риска развития неблагоприятных сердечно-сосудистых событий в течение года после каротидной эндартерэктомии у пациентов с мультифокальным атеросклерозом». При данном способе пациентам, поступившим в стационар для проведения оперативного вмешательства, оценивают наличие дислипидемии, уровень интерлейкина-12, протяженность атеросклеротической бляшки, определяемую интраоперационно, и наличие кровоизлияний в атеросклеротической бляшке. За ближайший аналог принят способ определения риска смерти или инсульта в течении 30 дней после каротидной эндартерэктомии при бессимптомном поражении внутренней сонной артерии (СЕА-8). При данном способе у пациентов учитывают факторы риска, такие как: негроидная раса, инсульт/транзиторная ишемическая атака, стеноз контрлатеральной артерии 50% и выше, застойная сердечная недостаточность, ишемическая болезнь сердца, поражение клапанов сердца, тяжелая инвалидность. При этом каждому фактору присваивают балльное значение и по сумме баллов определяют риск наступления послеоперационных осложнений [Calvillo-King L. Для решения поставленных задач проведена апробация способа в ГБУЗ «Научно-исследовательском институте - Краевая клиническая больница №1 имени профессора С.В. Очаповского» министерства здравоохранения Краснодарского края, МБУЗ «Городская больница №1» г. Новороссийск, МБУЗ «Городская больница №4» г. Сочи, в клинических условиях было обследовано 1200 пациентов с различной степенью сосудисто-мозговой недостаточности.To solve the problems, the method was tested in GBUZ "Research Institute - Regional Clinical Hospital No. 1 named after Professor S.V. Всех пациентов подвергли оперативным вмешательствам на сонных артериях, после чего осуществляли контроль за состоянием пациентов и выявлением различных послеоперационных осложнений. При этом к послеоперационным осложнениям были отнесены: нагноение, несостоятельность швов, повреждение черепно-мозговых нервов, послеоперационная гематома, кровотечение, инсульт, транзиторная ишемическая атака, дистальная артериальная эмболизация, изменение когнитивных функций, острый инфаркт миокарда, острый коронарный синдром, пневмония, эпизоды стенокардии, смерть.All patients underwent surgical interventions on the carotid arteries, after which they monitored the condition of the patients and the identification of various postoperative complications. На амбулаторном этапе у всех пациентов выявили наличие факторов риска развития сердечно-сосудистых заболеваний, как: курение, наследственная предрасположенность, дислипидемия, ожирение, артериальная гипертензия.At the outpatient stage, all patients revealed the presence of risk factors for the development of cardiovascular diseases, such as smoking, hereditary predisposition, dyslipidemia, obesity, arterial hypertension.
Далее путем логистического регрессионного анализа, были выявлены независимые факторы, оказывающие наибольший вклад в послеоперационном периоде. В результате были отобраны показатели, достоверно влияющие на частоту развития осложнений в послеоперационном периоде, к ним отнесли: перенесенный инфаркт миокарда, инсульт, наличие стенокардии, наличие патологической извитости внутренней сонной артерии, хроническую обструктивную болезнь легких, кальциноз сонных артерий, сахарного диабета, ожирения 3 степени, перенесенную транзиторную ишемическую атаку, использование стента с открытой ячейкой (таблица 1).Further, through logistic regression analysis, independent factors were identified that have the greatest contribution in the postoperative period. Стенокардия ФК-1. Сахарный диабет 2 типа. Гипертоническая болезнь III степени, риск 4. По данным ТС БЦА (триплексного сканирования брахиоцефальных артерий) выявлен стеноз левой внутренней сонной артерии 85%. Пациенту выполнена проба с пережатием левой общей сонной артерии, к пережатию пациент клинически толерантен. Учитывая сопутствующую патологию, констатировано наличие двух факторов, предложенного способа, с суммой баллов 2, что говорит о низкой степени риска послеоперационных осложнений. Выполнена эверсионная каротидная эндартерэктомия из левой внутренней сонной артерии. В послеоперационном периоде у пациента достоверно отсутствовали послеоперационные осложнения.Example 1. Учитывая сопутствующую патологию, констатировано наличие двух факторов, предложенного способа, с общей суммой баллов 3, что говорит о средне низкой степени риска послеоперационных осложнений. Выполнена эверсионная каротидная эндартерэктомия из левой внутренней сонной артерии. В раннем послеоперационном периоде, у пациента зафиксировано нагноение послеоперационной раны.Example 2. Гипертоническая болезнь III степени, риск 4. По данным ТС БЦА (триплексного сканирования брахиоцефальных артерий) выявлен стеноз правой внутренней сонной артерии 75%. По результатам компьютерной томографии выявлен кальциноз общей и внутренней сонных артерий. Пациенту выполнена проба с пережатием правой общей сонной артерии, к пережатию пациент клинически толерантен. Учитывая сопутствующую патологию, констатировано наличие четырех факторов, предложенного способа, с общей суммой баллов 6, что говорит о средней степени риска послеоперационных осложнений. Выполнена эверсионная каротидная эндартерэктомия из правой внутренней сонной артерии. В послеоперационном периоде через 4 месяца после каротидной эндартерэктомии, у пациента зафиксирован инфаркт миокарда.Example 3. Occlusion of the posterior interventricular artery in the distal region. Stenosis of the right internal carotid artery 75%. CHFM IV. Ischemic heart disease. Angina pectoris FC I-II. PIM dated December 6, 2016. Grade III hypertension, risk 4. According to BCA TS (triplex scanning of the brachiocephalic arteries), stenosis of the right internal carotid artery 75% was revealed. According to the results of computed tomography, calcification of the common and internal carotid arteries was revealed. The patient underwent a test with compression of the right common carotid artery; the patient is clinically tolerant to compression. Given the concomitant pathology, the presence of four factors, the proposed method, with a total score of 6, which indicates an average risk of postoperative complications, was ascertained. An eversion carotid endarterectomy was performed from the right internal carotid artery. In the postoperative period 4 months after carotid endarterectomy, the patient has myocardial infarction.

Окклюзия передней межжелудочковой артерии в проксимальном отделе. Стеноз правой внутренней сонной артерии 85%. ХСМН IV. ИБС. Стенокардия ФК II. ПИМ от 17.09.2016 г., 22.09.2017 г. Гипертоническая болезнь III степени, риск 4. Сахарный диабет 2 типа. Ожирение 3 степени. По данным АГ БЦА (ангиографического исследования брахиоцефальных артерий) выявлен стеноз правой внутренней сонной артерии 85%. Учитывая сопутствующую патологию, констатировано наличие шести факторов, предложенного способа, с общей суммой баллов 8, что говорит о средне высокой степени риска послеоперационных осложнений. Пациенту выполнена балонная ангиопластика правой внутренней сонной артерии с имплантацией стента с открытой ячейкой. В раннем послеоперационном периоде, у пациента зафиксирована транзиторная ишемическая атака.Example 4. According to AG BCA (angiographic study of brachiocephalic arteries), 85% stenosis of the right internal carotid artery was revealed. Given the concomitant pathology, the presence of six factors, the proposed method, with a total score of 8, which indicates an average high degree of risk of postoperative complications. The patient underwent balloon angioplasty of the right internal carotid artery with implantation of a stent with an open cell. In the early postoperative period, the patient recorded a transient ischemic attack.

Стеноз левой общей сонной артерии 80%. Гемодинамически значимая извитость правой внутренней сонной артерии. ХСМН IV. ИБС. Стенокардия ФК II. ПИМ от 13.04.2017 г. Гипертоническая болезнь III степени, риск 4. ХОБЛ II стадии. Сахарный диабет 2 типа. Ожирение 3 степени. По данным АГ БЦА (ангиографического исследования брахиоцефальных артерий) выявлен стеноз левой внутренней сонной артерии 80%. Учитывая сопутствующую патологию, констатировано наличие семи факторов, предложенного способа, с общей суммой баллов 9, что говорит о высокой степени риска послеоперационных осложнений. Пациенту выполнена эверсионная каротидная эндартерэктомия из общей сонной артерии, резекция с редрессацией правой внутренней сонной артерии с реимплантацией в старое устье. В раннем послеоперационном периоде, у пациента развился острый коронарный синдром.Example 5. Ischemic heart disease. Angina pectoris FC II. PIM from 04/13/2017 Hypertensive disease of the III degree, risk 4. COPD II stage. Type 2 diabetes. Obesity 3 degrees. According to the data of AG BCA (angiographic study of brachiocephalic arteries), stenosis of the left internal carotid artery of 80% was revealed. Given the concomitant pathology, the presence of seven factors, the proposed method, with a total score of 9, which indicates a high degree of risk of postoperative complications. The patient underwent an eversion carotid endarterectomy from the common carotid artery, resection with a reduction of the right internal carotid artery with reimplantation into the old mouth. In the early postoperative period, the patient developed acute coronary syndrome.

Пациенту выполнена проба с пережатием левой общей сонной артерии, к пережатию пациент клинически толерантен. Учитывая сопутствующую патологию, констатировано наличие трех факторов, предложенного способа, с общей суммой баллов 5, что говорит о средней степени риска послеоперационных осложнений. Выполнена эверсионная каротидная эндартерэктомия из левой внутренней сонной артерии. В послеоперационном периоде через 2 месяца после каротидной эндартерэктомии, у пациента зафиксирована сегментарная нижнедолевая пневмония правого легкого.Example 6.
