Abstract
Description
Изобретение относится к медицине, к инфекционным болезням и может быть использовано для экстренной профилактики клещевого вирусного энцефалита (КВЭ).The invention relates to medicine, to infectious diseases and can be used for emergency prevention of tick-borne viral encephalitis (CVE).
The drug "Human immunoglobulin against tick-borne encephalitis" is the only drug recommended for emergency prevention of CVE by the current regulatory document SP 3.1.3.2352-08 "Prevention of tick-borne viral encephalitis." The drug is administered to persons not vaccinated against TBEV, who received an incomplete course of vaccination, who have defects in the vaccination course, and who have no documented evidence of preventive vaccinations. The introduction of human immunoglobulin (IG) against TBEV is recommended no later than 4 days after suctioning the tick. The drug is administered intramuscularly once at the rate of 0.1 ml per 1 kg of body weight. The protective effect is manifested after 24-48 hours and lasts about 4 weeks. Despite the fact that the protective ability of IG preparations can vary under the influence of factors such as the infecting dose of the virus, the immunobiological state of the macroorganism, the virus-specific activity of the drug and the timing of its administration, the data accumulated to date on the results of post-exposure use of this method of emergency prevention indicate high (up to 79% of cases) the degree of protection of people from CVE after tick suction.
A series of drugs (4-iodantipyrine, belonging to the group of non-steroidal anti-inflammatory drugs, pyrazole derivatives, Cucumarioside, which is the sum of triterpene glycosides, Ridostin and Cycloferon, which are interferon inducers), the effectiveness of which has not yet been convincingly demonstrated, has also been proposed for emergency prevention of CE.
27.03.2005).The closest analogue is the method for the prevention of CVE, which consists in introducing the drug "Realdiron" containing human recombinant interferon alpha-2b at a dose of 3 million ME into the site of a tick bite (patent RU No. 2261105, C1, A61K 38/19, A61P 37/4, published: 03/27/2005).
2b.The disadvantage of this analogue is the lack of virus-neutralizing properties of interferon alpha-2b.
The objective of the invention is to achieve a high concentration of virus-neutralizing antibodies directly in the tissues through the use of transdermal patch application at the site of tick suction for the introduction of the topical composition of immunoglobulin, a person against tick-borne encephalitis with tizol (IHT). In this case, the maximum concentration of antiviral antibodies is achieved at the site of the primary reproduction of the virus immediately after application of the drug, which helps to neutralize the pathogen and effectively prevent tick-borne encephalitis in the early stages of infection.
Effect: reducing the volume of the administered drug and reducing the load on the patient’s immune system, simplifying the administration procedure and eliminating the threat of additional infection of the patient’s body through non-invasive use of the drug as. independently and on an outpatient basis, several times lower cost of the preventive course.
The specified technical result is achieved by the fact that the claimed method for the prevention of tick-borne encephalitis by introducing an antiviral drug into the body, characterized in that after removing the tick at the place of suction, the transdermal patch is applied to the place of suction of the tick to introduce the drug composition of human immunoglobulin against tick-borne encephalitis with tizol: immunoglobulin with a titer of antibodies to TBEV 1: 320 1 ml, tizol up to 5.0 g on dry skin with an even thin layer at the rate of 0.2-0.5 g per 1 dm2 from 1 to 3 times a day ki.
An auxiliary substance in the composition of the complex preparation is tizol: an aquacomplex of titanium glycerosolvate, the general chemical formula TiO4 (С3Н7O2) 4 (С3Н8O3) 10 (Н2O) 40), which ensures the conductivity of active substances through the tissues of the human body and their transportation to the pathological focus. In addition, tizol has anti-inflammatory, decongestant, antiseptic effect, stimulates tissue regeneration, local blood flow. Tizol when mixed with the active substances of the drug forms stable gels with them, helps to increase their activity due to transcutaneous properties and provides general toxicological acceptability for the body of the active substances of the claimed preparation. Currently, transport systems for transdermal delivery of active substances included in the composition of these drugs are effectively used. One of the main technological trends in the development of transdermal transportation systems for active substances is the use of chemical compounds - enhancers.
The advantages of enhancers over other methods are that they are simply introduced into the composition of the product, while eliminating the need for additional production of carriers (liposomes, etc.), and additional equipment and tools are not required.
It is known that in the first 36 hours after sucking on a flare, the so-called delayed phase of blood sucking occurs when the amount of tick-infected virus saliva that penetrates the host dermis to a depth of not more than 1.0 mm does not exceed 1.0 μl. The cycle of reproduction of TBEV after penetration into sensitive vertebrate cells is 24-30 hours. Moreover, in the first few days, the highest concentration of the virus is at the site of suction of the tick. The virus from the entrance gate of the infection into the bloodstream of the host occurs 5-7 days after the suction of the infected tick. Thus, the most effective way to prevent the generalization of the infectious process of CVE is to neutralize the virus as early as possible at the site of entry of the infection.
The essence of the invention is as follows: after removing the tick, at the place of suction, the drug composition of a human immunoglobulin against tick-borne encephalitis with Tizol is applied (Immunoglobulin with a titer of antibodies to TBEV of 1: 320 - 1 ml., Tizol - up to 5.0 g) on dry skin with an even thin layer (at the rate of 0.2-0.5 g per 1 dm 2 ) from 1 to 3 times a day. In this case, the maximum concentration of antiviral antibodies is achieved at the site of the primary reproduction of the virus immediately after use of the drug, which helps to neutralize the pathogen and effectively prevent tick-borne encephalitis in the early stages of infection.
Materials of the interregional scientific-practical conference, Yekaterinburg, GOU VPO UGMA Roszdrav, 2010, 159 pp.).
The invention is illustrated by the following examples.
Example 1. Patient K., 46 years old, was referred for a tick bite, which he found in the left inguinal region 18 hours after returning from a summer cottage in the village of Raskuikha, Kamensk-Uralsky GO, located in an endemic territory. From tick-borne encephalitis has not previously been vaccinated. After removing the tick at the suction site, the drug composition of the human immunoglobulin against tick-borne encephalitis was applied with tizol: immunoglobulin with an antibody titer to TBEV of 1: 320 1 ml, tizol up to 5.0 g. In a suspension obtained from a removed tick by RT-PCR detected RNA of TBEV. With further observation during the month, clinical manifestations of the disease were not detected. The result of the study of paired blood serums taken on the day of treatment and after 3 weeks for the presence of IgM and IgG for TBEV by the ELISA method is negative.
Example 2. Patient R., 24 years old, came for a tick bite, which she found under the left mammary gland on the second day after visiting the forest garden zone "Gardeners of the Urals" in Yekaterinburg, located on an endemic territory. 6 years ago I received two vaccinations with a vaccine against CVE with an interval of 6 months and a year later revaccination. After removing the tick to the patient at the suction site, the drug composition of the human immunoglobulin against tick-borne encephalitis with tizol was applied: immunoglobulin with a titer of antibodies to TBEV of 1: 320 1 ml, tizol up to 5.0 g. Using RT-PCR in a suspension obtained from a removed tick , detected TBNA RNA. The result of the study of paired blood serums taken on the day of treatment and after 2 weeks for the presence of IgM to TBEV by ELISA is negative, the titer of IgG antibodies in both cases was 1: 200. With further observation during the month, clinical manifestations of the disease were not detected.
