Изобретение относится к медицине, а именно к нейрохирургии и может быть использовано для малоинвазивной имплантации электродов для стимуляции периферических нервов и нервных сплетений.The invention relates to medicine, namely to neurosurgery and can be used for minimally invasive implantation of electrodes to stimulate peripheral nerves and plexuses.
Нейропатия периферических нервов и сплетений, сопровождающаяся тяжелым нейрогенным болевым синдромом, является одной из актуальных медико-социальных проблем современной медицины, т.к. ведет к грубой социальной дезадаптации. Особенностью таких болей является их постоянный характер, высокая интенсивность, вегетативные проявления и сложность купирования при помощи консервативной терапии.Neuropathy of the peripheral nerves and plexuses, accompanied by severe neurogenic pain syndrome, is one of the urgent medical and social problems of modern medicine, because leads to gross social maladjustment. Оперативное вмешательство выполняется в условиях общей комбинированной внутривенной анестезии с протекцией дыхательных путей интубационной трубкой. Перед обработкой операционного поля в иннервированные нервом или сплетением мышечные группы устанавливают игольчатые электроды для проведения интраоперационного электрофизиологического мониторинга.Surgery is performed under conditions of general combined intravenous anesthesia with the protection of the respiratory tract with an endotracheal tube. Пациент П., 61 год. Находился на лечении с диагнозом: Невропатия икроножного нерва справа. Тяжелый медикаментозно-резистентный невропатический болевой синдром в пяточной области.Patient P., 61 years old. Под эндотрахеальным наркозом в положении пациента на спине через кожный разрез длиной 2 см за наружной лодыжкой слева (в проекции икроножного нерва, проксимальнее области боли) выделен и с помощью эндоскопической оптики прослежен проксимально на 15 см икроножный нерв (фиг. 1 Эндоскопическое выделение нерва, где: 1 - икроножный нерв; 2 - апоневроз; 3 - тубус эндоскопа). С целью определения оптимального положения электрода выполнена стимуляция нерва через эндоскопическую систему (фиг. 2. Электрофизиологический мониторинг, где: 1 - икроножный нерв; 4 - чувствительная ветвь икроножного нерва; 5 - электрод электрофизиологического мониторинга). Под визуальным контролем через порт системы на нерв имплантирован электрод (фиг. 3. Имплантация электрода, где: 1 - икроножный нерв; 6 - контакт электрода; 7 - электрод), который фиксирован эпиневральным швом и "якорем" (фиг. 4. Ранний послеоперационный период протекал без осложнений. Рана зажила первичным натяжением. Область парестезий при стимуляции полностью охватывает область боли с практически полным ее регрессом (улучшение более, чем на 90%).The early postoperative period was uneventful.
