Description
Предлагаемое изобретение относится к медицине, а именно к гинекологии, и может быть использовано для уменьшения интраоперационной кровопотери при хирургических вмешательствах по поводу миомы матки, в частности миомэктомии влагалищным доступом.The present invention relates to medicine, namely to gynecology, and can be used to reduce intraoperative blood loss during surgical interventions for uterine fibroids, in particular myomectomy by vaginal access.
Миома матки является наиболее распространенной доброкачественной опухолью женских половых органов. Согласно статистике миома матки диагностируется у 20-40% женщин репродуктивного возраста европеидной расы. В период перименопаузы число случаев миомы матки возрастает до 40-50%. Есть данные о том, что наиболее часто (до 70% в перименопаузе) миома матки встречается у женщин негроидной расы (Lopez FR, Ornat L, Ceausu I, Depypere H, Erel CT, Lambrinoudaki I, Schenck-Gustafsson K, Simoncini T, Tremollieres F, Rees M. С целью снижения кровопотери интраоперационно, за 3 месяца до операции назначают аГнРГ. Негативным момент использования агонистов являются побочные реакции, связанные с гипоэстрогенией: депрессии, лабильность, снижение либидо, приливы, деминерализация костей.In order to reduce blood loss intraoperatively, aGnRH is prescribed 3 months before the operation. Сущность изобретения заключается в том, что после формирования доступа обеспечивают временную окклюзию маточных сосудов путем наложения на крестцово-маточные связки и ребра матки слева и справа тонзиллярных диссекционных или длинных прямых гемостатических зажимов, на бранши которых предварительно надевают силиконовые трубки, по диаметру соответствующие толщине бранш зажимов.The essence of the invention lies in the fact that after access is formed, temporary occlusion of the uterine vessels is provided by superimposing on the sacro-uterine ligaments and uterine ribs left and right tonsillar dissection or long straight hemostatic clamps, on the branches of which they are previously put on silicone tubes, the diameter corresponding to the thickness of the jaws of the clamps .

Способ актуален особенно при наличии у больной множественной миомы матки с локализацией узлов по передней и задней стенкам матки, шеечная локализации узлов, миоме матки до 10 недель беременности, преимущественно у женщин с нереализованной репродуктивной функцией. Влагалищную миомэктомию проводят под спинномозговой/эпидуральной анестезией. Пациентку укладывают в литотомической позиции на операционном столе. Шейку матки фиксируют пулевыми щипцами и выполняют переднюю и заднюю кольпотомию. В брюшную полость вводят ретрактор, оценивают размеры матки и миоматозных узлов, их расположение. На крестцово маточные связки и ребро матки слева и справа накладывают тонзиллярные диссекционные длинные прямые зажимы или длинный прямой гемостатический зажим, на бранши которых надевают тонкие силиконовые трубки для избежания травмы сосудистой стенки во время сжатия. Разрезают серозную оболочку над выступающей частью узла, фиксируют его пулевыми щипцами, вылущивают. После энуклеации узла, матку выводят в рану и послойно ушивают с наложением мышечно-мышечных и серозно-мышечных швов. По аналогии удаляют все узлы. С ребер матки удаляют тонзиллярные диссекционные длинные прямые зажимы или длинный прямой гемостатический зажим, на бранши которых надеты тонкие силиконовые трубки. Выполняют перитонизацию, затем восстанавливают слизистую оболочку влагалища. Способ апробирован в течение 3-х лет на 18 женщинах. Время операции главным образом зависело от размеров, количества и расположения узлов. В среднем составило 50 мин. Объем кровопотери в среднем составил 100 мл. Переливания крови и ее компонентов не потребовалось ни одной пациентке. Послеоперационный период у всех пациенток протекал удовлетворительно: уже в первые сутки после хирургического вмешательства женщины вставали, передвигались. Среднее количество койко-дней, проведенных пациентками в стационаре, составило 4,8.The method was tested for 3 years on 18 women. Из анамнеза: беременностей - 0, в марте проведена гистероскопия, раздельное диагностическое выскабливание, по данным гистологического исследования - секреторный эндометрий. По данным УЗИ гениталий множественная миома матки: два интрамуральных узла по задней стенке матки 3 см и 5 см, один узел по передней стенке 8 см. В плановом порядке проведено оперативное лечение в объеме влагалищной миомэктомии через переднюю и заднюю кольпотомии с обратимой окклюзией маточных сосудов путем накладывания через проведенную переднюю и заднюю кольпотомии, длинных прямых зажимов, на бранши которых надеты тонкие силиконовые трубки. Время операции составило 50 минут. Общая кровопотеря 100 мл. Выписана на 5-е сутки в удовлетворительном состоянии.
