Abstract
Description
The invention relates to medicine, in particular to neurology and reflexology, and can be used to treat ischemic stroke in the acute period.
There is a method of treating ischemic stroke in the acute period (after stabilization of the general condition) by acupuncture points only Yang channels using a regulatory and mild stimulating method of acupuncture. The posterior median canal, channels of the 3 parts of the trunk and colon with paresis of the upper limbs, channels of the stomach, gall bladder with paresis of the lower extremities are used primarily on the affected side [1. Drobysheva N.A. A practical guide to reflexology. Reflexotherapy for various diseases. Prince 7. h. 1. - M., 1997. - 120 p. - S. 11] or bilaterally [2. Gaponyuk P.Ya. Guide to acupuncture therapy. - M .: "FIRN M", 1999. - 364 p. - S. 184-188]. The disadvantage of these methods is the lack of impact on the scalp zone, which is very effective in the treatment of central paresis and speech disorders.
A known method of treating cerebral stroke by exposure to acupuncture points II variant of the inhibitory method in the area of paretic muscles, in antagonistic muscle groups - tonic method. On healthy limbs, tonic or harmonizing techniques are used. The choice of points for the treatment of motor post-stroke disorders involves exposure to local points (on the affected extremities), symmetrical points (on healthy limbs), points of the cervical-collar zone (SCH) (in all cases) and lumbosacral zone (SCC) (with paresis lower extremities), as well as the scalp area on the side of the lesion and general strengthening points [3. Samosyuk I., Lysenyuk V. Acupuncture. M .: AST-Press book, 2004 .-- 528 p. - S. 492]. The disadvantage of this method is the difficulty of giving the patient the necessary position for simultaneous exposure to the SCH and SCC points in combination with local points of the limbs, due to the presence of motor disorders and low mobility of patients within the bed. In addition, the described method is proposed for use after 2-3 weeks from the onset of the disease, when spastic disorders in the affected extremities are already beginning to form (which explains the recommendations to use the II, and then the I variant of the inhibitory method). Treatment is best carried out as soon as possible (immediately after stabilization of the general condition of the patients), because it is the acute period of stroke that is most promising for the recovery of patients, since it is crucial for the formation of a residual neurological defect.
In recent years, combined methods of reflexology have been introduced into the practice of treating motor cerebral disorders. Closest to the claimed method, the technique of auriculocraniopuncture in combination with electrical stimulation of paretic limbs [3. Samosyuk I., Lysenyuk V. Acupuncture. M .: AST-Press book, 2004 .-- 528 p. - S. 493-494]. Acupuncture needles are introduced into the scalp motor zone towards each other. Further, on the side of paresis or paralysis, according to topographic schemes on the auricle, zones corresponding to paretic limbs, into which acupuncture needles are inserted subcutaneously through the entire "limb zone", are determined. After this, the needles are connected in pairs to the stimulator. The upper needle in the scalp connects on the auricle to the needle inserted into the “foot zone”, and the lower needle in the scalp connects to the needle in the “arm zone”. At the same time, plate electrodes are fixed on paretic limbs to stimulate the muscles of the antagonists of the lower leg, thigh, as well as the shoulder and forearm. The duration of the session is 20-30 minutes. The course of treatment is 20-30 procedures. Assign 2-3 courses. The disadvantages of this method are the technical complexity of its implementation, as well as restrictions on the use of electrical stimulation in patients suffering from arterial hypertension and cardiac arrhythmias.
A method is proposed for the treatment of cerebral ischemic stroke in the acute period, including the application of DENS to one of the universal zones of general action, the areas of the SCC or SCC, speech zones, the zone of the 2nd cervical vertebra, the correspondence zone along the suzok - depending on the lesion, stable method in the "Therapy" mode with a frequency of 77 Hz for 5 minutes each, points of the auricular correspondence system in the "Therapy" mode at a frequency of 77 Hz for 1-2 minutes each, 2-3 points per procedure, the effect on the scalp zones, depending on available focal neurological defect in the “Therapy” mode at a frequency of 20 and 77 Hz in a labile way for 7-10 minutes, choosing 2-3 treatment zones for one session, with simultaneous acupuncture of Jan points - channels of the affected limbs and symmetrical points with a harmonizing or soft stimulating method , for 10-15 days, 1 session per day [4. A method for the treatment of cerebral ischemic stroke in the acute period / E.E. Molchanova, Yu.S. Grishchenko, V.A. Ananenko. - RF patent No. 2531693 No. 2013148202/14; publ. 10/27/2014, Bull. No. 30. - 9 p.]. The disadvantage of this method is the need for expensive equipment for dynamic electro-neurostimulation and the presence of an operator for processing scalp zones and irritation of auricular points.
The technical problem solved by this invention is the creation of a method that allows the most effective early non-drug rehabilitation of patients in the acute period of ischemic stroke in order to maximize regression of neurological deficiency, reduce the recovery time of patients who have cerebral ischemic stroke and increase their motivation by intensifying the impact on cerebral hemodynamics and activation of the patient’s own compensatory mechanisms. The aim of the proposed method was the development and evaluation of the effectiveness of the differential application of the combined methods of corporal and microsystem (auricular and prolonged scalping therapy) acupuncture for restorative correction of the effects of ischemic stroke.
Yu Zhi Shun, Sai Xue Bo, Yu Hui Min Study of the way of acupuncture according to methods of scalp acupuncture in stroke treatment // Zhen Jiu. - 1993. - P. 36-37 Chinese.] в модификации Tang Qiang (КНР) [6. Tang Qiang Treatment and rehabilitation of strokes with acupuncture. - Province Heilongjiang publishing house of education, 2012. The method is as follows. In case of a stroke in the pool of the internal carotid artery, the scalp is exposed to the parietal and anteroposterior zones from 2 sides (5 needles per zone), in case of speech disorders, 3 needles are additionally indicated in the temporal zone on the left and one needle in the symmetrical zone of the rim (if the patient is dominated by the right hemisphere of the brain, then vice versa), with psycho-emotional and cognitive disorders, the frontal region is additionally irritated symmetrically from 2 sides; with a stroke in the vertebrobasilar basin - the lower occipital and cervical regions; corporal points: with paresis of the upper limb - GI (II) 11 qui-chi (alternate with GI (II) 10 show-san-li), TR (X) 5 wai-guan (alternate with MS (IX) nei-guan) , GI (II) 4 he-gu from 2 sides, GI (II) 15 jian-yu, IG (VI) 3 hou-si (alternate with IG (VI) 4 wan-gu), TR (X) 3 chjun-chu, PC 107 ba-se (on the paresis side); with paresis of the lower limb - E (III) tsu-san-li, F (XII) tai-chun, R (VIII) 3 tai-si, RP (IV) 6 san-yin-jiao (from 2 sides), E (III) 34 liang-qiu, RP (IV) 10 syue-hai, RP (IV) 9 yin-lin-quan, VB (XI) 34 yang-lin-quan, E (III) 41 jie-xi, PC 136 ba-feng (on the side of the paresis); with paresis of mimic muscles - E (III) 4 di-cyan, E (III) 6 jia-che, GI (II) 20 ying-xiang (on the side of paresis); auricular points - AR 55 Shen-men, AR 95 kidney, AR 100 heart, AP 29 occiput, AR 34 cerebral cortex, in the presence of vestibulo-atactic syndrome - additional AR 25 brain stem, 2-3 points per procedure, treatment begins on 2-3rd day of stay of patients in the hospital, the time of exposure to corporal and auricular points - 20-30 minutes by the harmonizing method, the duration of exposure to the scalp 4-6 hours with periodic irritation of the zones by rotating the needles for 1-2 minutes each hour. During the procedure of prolonged craniopuncture (after extraction of the corporal and auricular needles), patients are recommended to engage in physical therapy, speech therapy (in the presence of aphatic disorders), and move as much as possible.
1).To assess the effectiveness of the proposed methodology, its influence on the rate of regression of neurological deficit, the degree of adaptation to everyday life and the level of motivation in patients in the acute period of ischemic stroke, 60 patients were randomly selected, who were divided into 2 groups (30 people each), identical by sex, age, severity of stroke (table. 1).
Для оценки тяжести неврологического дефицита применялась шкала NIHSS, активности повседневной жизни больного после инсульта - Barthel index, а уровня мотивации пациентов - Тест-опросник «Мотивация успеха и мотивация боязни неудачи» (МУН)).In the 1st group of patients (main), simultaneously with classical and auricular acupuncture, prolonged scalping was performed (according to the method described above). Patients of the 2nd group (control) underwent acupuncture in combination with scalping according to the classical method. To assess the severity of neurological deficit, the NIHSS scale was used, the daily life activity of the patient after a stroke was Barthel index, and the patient motivation level was the Test Questionnaire “Motivation for Success and Motivation for Fear of Failure” (EOR)).
В обеих группах больных уровень мотивации, оцененный по опроснику МУН, повысился, и пациенты были ориентированы уже на успех, однако в основной группе эти изменения оказались более значимыми (р<0,05).Monitoring was carried out on the 1st and 15th day from the start of treatment. In the main group of patients, on the 15th day from the start of treatment, the decrease in the severity of neurological deficit according to the NIHSS scale was 5.0 points (p <0.001), the Bartel index increased by 31.0 points (p <0.001) (Table 2). In the control group, similar indicators amounted to 3.5 points (on the NIHSS scale) (p <0.05) and 23.5 points (Barthel index) (p <0.05). In both groups of patients, the level of motivation, as assessed by the EOR questionnaire, increased, and the patients were already oriented toward success, however, in the main group, these changes were more significant (p <0.05).
The results obtained indicate that a more intense effect on the scalp area and, accordingly, on cerebral hemodynamics, with prolonged scalping, can significantly accelerate the recovery of motor functions by restoring active movements in the limbs. It should be noted that under the influence of prolonged craniopuncture, speech was restored faster, the possibilities of contact with the patient expanded due to improved attention and mood, the appearance of a mood to achieve a positive treatment result. Almost all patients tolerated the procedures well. No side effects were noted.
