Известен способ лечения привычного вывиха плеча по А.В. Краснову [2]. Суть операции состоит в следующем. Передним доступом обнажают область межбугорковой борозды плечевой кости. Ложе сухожилия длинной головки двуглавой мышцы плеча вскрывают вверх на 1 см проксимальнее вершины большого бугорка и вниз до перехода сухожилия в мышечное брюшко. Сухожильное прикрепление большой грудной мышцы частично надсекают сверху вниз на протяжении 1,5-2 см. Освобожденное сухожилие длинной головки бицепса на влажной марлевой «держалке» временно оттягивают кнутри. Тонким плоским долотом большой бугорок отщепляют кнаружи и в виде створки отворачивают на 80-90 градусов в латеральную сторону, с которой она сохраняет питающую надкостнично-мышечную ножку. В обнаженной кости выдалбливают канавку, идущую сверху вниз параллельно межбугорковому желобу. Стенки канавки выравнивают и моделируют по размеру сухожилия бицепса, краям ее придают плавные изгибы кнутри в верхней части по направлению к эпифизарному хрящу и в нижней - к выходу на область хирургической шейки. Сухожилие бицепса освобождают от оболочек, перемещают во вновь созданный костный канал и прикрывают костной створкой, которая фиксируется на прежнее место тремя чрескостными кетгутовыми швами. Прототипом изобретения является способ хирургического лечения привычного вывиха плеча [6], включающий передний доступ к плечевому суставу с отсечением и низведением верхушки клювовидного отростка с прикрепленными к нему мышцами и вертикальным рассечением мышечно-сухожильного комплекса подлопаточной мышцы, наложение и фиксирование металлического имплантата для усиления связи сочленяющихся костей плечевого сустава, сшивание мышечно-сухожильного комплекса подлопаточной мышцы, репозицию и остеосинтез верхушки клювовидного отростка, в котором вертикальное рассечение мышечно-сухожильного комплекса осуществляют на его сухожильном участке вблизи малого бугорка плечевой кости. В качестве фиксирующего элемента используют плетеную сетку из никелид-титановой проволоки, которую располагают над фиброзной капсулой и фиксируют никелид-титановой нитью трансгленоидным швом к шейке лопатки и чрескостным швом к плечевой кости. Then, a polypropylene mesh implant (9) is modeled using scissors (the size of the implant is determined intraoperatively according to the individual anatomical features of the structure of the shoulder joint) and the anterior section of the joint capsule is strengthened (4) by fixing it with suture material: proximal to the front-lower edge of the coraco-clavicular ligament (10); laterally - to the case of the tendon of the long head of the biceps (11); medially - to the joint capsule along the edge of the articular cavity of the scapula (12); distally - to the axillary inversion (13), in the center - to the joint capsule (14). A layer-by-layer suturing of the postoperative wound is performed and the limb is immobilized in the physiological position.

Больной Н., 24 года, госпитализирован в травматологическое отделение с диагнозом: Хроническая передняя нестабильность правого плечевого сустава (привычный вывих правого плеча). Из анамнеза: травма спортивная, 2 года назад во время борьбы почувствовал резкую боль и деформацию правого плечевого сустава. За медицинской помощью обратился в травматологический пункт, где вывих был вправлен и наложена гипсовая повязка. После снятия повязки через 4 недели приступил к разработке плечевого сустава. При увеличении объема движений вновь возник вывих плеча, который устранил самостоятельно. После этого вывихи правого плеча повторялись до 3-4 раз в месяц, вправление осуществлял самостоятельно. Выполнена операция по предлагаемому способу: «транспозиция сухожилия длинной головки двуглавой мышцы правого плеча, укрепление переднего отдела капсулы сустава полипропиленовым сетчатым имплантом». В асептических условиях, под эндотрахеальным наркозом, произведен разрез кожных покровов по передней поверхности плечевого сустава до 8,0 см. Осуществлен передний доступ к капсуле сустава (4). Первым этапом выделена межбугорковая борозда (5) плечевой кости (1) и освобождено сухожилие длинной головки двуглавой мышцы плеча на протяжении (6). В большом бугорке (3) плечевой кости (1) сформирован полуовальный паз выпуклой частью наружу (7). У основания паза (7), с помощью желобоватого долота, создан канал (8), идущий параллельно межбугорковой борозде (5). Сухожилие длинной головки двуглавой мышцы (6) перемещено в сформированный канал (8). Затем с помощью ножниц смоделирован полипропиленовый сетчатый имплант (согласно индивидуальным особенностям строения плечевого сустава данного пациента) (9) и осуществлена его фиксация к переднму отделу капсулы рассасывающимся шовным материалом: проксимально - к передненижнему краю клювовидно-ключичной связки (10); латерально - к футляру сухожилия длинной головки двуглавой мышцы (11); медиально - к капсуле сустава по краю суставной впадины лопатки (12); дистально - к подмышечному завороту (13), в центре - к капсуле сустава (14). После чего выполнено послойное ушивание послеоперационной раны. Иммобилизация верхней конечности осуществлена ортезом жесткой фиксации в физиологическом положении. Послеоперационный период протекал без осложнений. Швы сняты через 8 суток. Сроки иммобилизации составили 21 день. К полноценным тренировкам приступил через 3 месяца. При контрольных осмотрах пациента через 6, 12, 24 и 48 месяцев рецидива вывиха не зарегистрировано, болевого синдрома нет, движения в правом плечевом суставе в полном объеме.Patient N., 24 years old, was hospitalized in the trauma unit with a diagnosis of Chronic anterior instability of the right shoulder joint (habitual dislocation of the right shoulder). From the anamnesis: sports injury, 2 years ago during the struggle I felt sharp pain and deformation of the right shoulder joint. He turned to a trauma center for medical help, where the dislocation was set and a plaster cast was applied. After removing the bandage after 4 weeks, he began to develop the shoulder joint. With an increase in range of motion, a dislocation of the shoulder arose again, which eliminated independently. After this, dislocations of the right shoulder were repeated up to 3-4 times a month, reduction was carried out independently. The operation was performed according to the proposed method: "transposition of the tendon of the long head of the biceps of the right shoulder, strengthening the front of the joint capsule with a polypropylene mesh implant." Under aseptic conditions, under endotracheal anesthesia, an incision was made on the skin along the front surface of the shoulder joint up to 8.0 cm. Front access to the joint capsule was made (4). At the first stage, the inter-tubercle groove (5) of the humerus (1) was isolated and the tendon of the long head of the biceps of the shoulder muscle was released (6). In the large tubercle (3) of the humerus (1), a semi-oval groove is formed with the convex part outward (7). At the base of the groove (7), with the help of a grooved chisel, a channel (8) was created, running parallel to the inter-tubercle groove (5). The tendon of the long head of the biceps muscle (6) is moved into the formed channel (8). Then, with the help of scissors, a polypropylene mesh implant was modeled (according to the individual structural features of the shoulder joint of this patient) (9) and it was fixed to the anterior part of the capsule with absorbable suture material: proximal to the anterior lower edge of the clavicular-clavicular ligament (10); laterally - to the case of the tendon of the long head of the biceps (11); medially - to the joint capsule along the edge of the articular cavity of the scapula (12); distally - to the axillary inversion (13), in the center - to the joint capsule (14). Then performed layer-by-layer suturing of the postoperative wound. The upper limb was immobilized by a rigid fixation in the physiological position. The postoperative period was uneventful. Sutures were removed after 8 days. The terms of immobilization amounted to 21 days. I started full training after 3 months. During the follow-up examinations of the patient after 6, 12, 24 and 48 months, no relapse of the dislocation was recorded, there was no pain, there was full movement in the right shoulder joint.
Предлагаемым способом пролечено 6 пациентов с хронической передней нестабильностью плечевого сустава. Срок наблюдения составил от 2 до 5 лет. Во всех случаях отмечено отсутствие болевого синдрома, полное восстановление функции плечевого сустава. Каких-либо осложнений не зарегистрировано.The proposed method treated 6 patients with chronic anterior instability of the shoulder joint. The observation period ranged from 2 to 5 years. In all cases, the absence of pain, a complete restoration of the function of the shoulder joint. No complications have been reported.
Проведенный нами анализ эффективности различных современных методик оперативного лечения хронической нестабильности плечевого сустава (по данным литературы) показал, что рецидивы вывихов составляют от 2-6%, кроме того, отмечаются такие осложнения, как жировая дистрофия подлопаточной мышцы (до 41%), несращение аутотрансплантата (до 26%), разрушение и резорбция трансплантанта (до 8%), стойкая утрата ротационных движений, быстрое развитие остеоартроза плечевого сустава и др. [11, 12].Our analysis of the effectiveness of various modern methods of surgical treatment of chronic instability of the shoulder joint (according to the literature) showed that relapses of dislocations range from 2-6%, in addition, complications such as subscapular muscle fatty degeneration (up to 41%), autograft nonunion (up to 26%), transplant destruction and resorption (up to 8%), persistent loss of rotational movements, rapid development of shoulder joint osteoarthritis, etc. [11, 12].
