На протяжении десятилетий был накоплен значительный опыт лечения больных с привычными вывихами плеча. На смену сухожильно-мышечной пластике пришли реконструктивные костно-пластические операции и артроскопические технологии. Наметилась тенденция к общему улучшению результатов лечения. Однако, дискутабельными остались вопросы обоснованности выбора методики с точки зрения надежности, анатомичности, травматичности и других факторов. Благодаря современным методам лучевой диагностики появились новые сведения о структурных изменениях в плечевом суставе и патобиомеханике нестабильности. Это заставило подойти к вопросу планирования оперативного вмешательства с точки зрения патогенетической обоснованности.Over the decades, considerable experience has been gained in treating patients with habitual shoulder dislocations. Replaced bone-plastic surgery and arthroscopic technology replaced the tendon-muscle plastic surgery. There has been a trend towards an overall improvement in treatment outcomes. However, the questions of the validity of the choice of methodology from the point of view of reliability, anatomy, traumatism and other factors remained debatable. Thanks to modern methods of radiation diagnostics, new information has appeared about structural changes in the shoulder joint and the pathobiomechanics of instability. This forced us to approach the issue of planning surgical intervention from the point of view of pathogenetic validity.
A.S. Bankart в 1923 г. подробно изучил и описал ранее выдвинутые положения о повреждениях суставной губы и капсулы плечевого сустава, возникающих при травматических вывихах плеча. Он создал концепцию, согласно которой наиболее часто встречающимся и основным повреждением, возникающим при травматической дислокации плеча, является отрыв суставной губы с капсульно-лабральным комплексом от переднего края суставного отростка лопатки. Р.Е. Greis с соавторами в 2002 г. установили влияние повреждения суставной губы и костного дефекта суставного отростка лопатки (СОЛ) на конгруэнтность суставных поверхностей. Е. В основу изобретения положено создание компьютерно-томографического способа оценки взаимодействия дефектов суставных поверхностей плечевого сустава при планирования хирургического лечения передней нестабильности, позволяющей выявить истинные причины патологии, тем самым снизив количество послеоперационных рецидивов, болевого синдрома и контрактур.The basis of the invention is the creation of a computed tomographic method for assessing the interaction of defects in the articular surfaces of the shoulder joint in the planning of surgical treatment of anterior instability, which allows to identify the true causes of pathology, thereby reducing the number of postoperative relapses, pain and contractures.

В зависимости от полученных результатов пациентам планировали различный объем оперативного вмешательства. В случае коэффициента K≤1 больным дополняли оперативное вмешательство (операцию Bankart, операцию Bristow-Latarjet) процедурой «reimplissage» (капсулодез и тенодез подостной мышцы в зону повреждения Hill-Sachs). Интраоперационно всем больным выполняли динамическую диагностику на предмет «зацепления» суставных поверхностей.Depending on the results obtained, patients were planned a different volume of surgical intervention. Был получен один ложноотрицательный результат: у одного пациента с K>1 была запланирована операция Bankart без проведения процедуры «reimplissage», однако, при динамической артроскопии было обнаружено наличие «зацепления» и выполнено подшивание сухожилия подостной мышцы в область дефекта. Ложноположительных результатов не получено. Причиной ложноотрицательного результата считаем погрешность в измерении величины повреждения Hill-Sachs за счет линейных артефактов и ухудшения визуализации сустава у пациентов с массивным поясом верхних конечностей.One false negative result was obtained: in one patient with K> 1, Bankart surgery was planned without the reimplissage procedure, however, with dynamic arthroscopy, the presence of “engagement” was detected and the tendon was sutured to the defect area. Пациент М., 25 лет. Пять лет назад попал в ДТП, находясь на заднем сидении легкового автомобиля. При выполнении рентгенографии левого плечевого сустава костно-патологических изменений выявлено не было. На соревнованиях по перетягиванию каната почувствовал острую боль в левом плечевом суставе. Был госпитализирован в клинику по поводу передне-нижнего вывиха левого плеча (фиг. 6).Patient M., 25 years old. Five years ago, he got into an accident while in the back seat of a car. When performing radiography of the left shoulder joint, bone-pathological changes were not detected. At the tug-of-war competitions I felt a sharp pain in my left shoulder joint. He was hospitalized in the clinic for antero-inferior dislocation of the left shoulder (Fig. 6).
Больному выполнена операция Bristow-Latarjet без процедуры «reimplissage». При динамической артроскопии также выявлено отсутствие «зацепления».The patient underwent surgery Bristow-Latarjet without the procedure "reimplissage". Dynamic arthroscopy also revealed the absence of “engagement”.
Разработанный способ КТ-исследования позволяет неинвазивно выявить «зацепление» костных структур при биполярных повреждениях головки плечевой кости и суставной поверхности лопатки и более точно планировать хирургическое лечение. Стоит отметить, что при получении значения критерия К≤1 возможно планирование процедуры «reimplissage», при К>1 - необходимость в ее проведении отсутствует.The developed CT scan method allows non-invasive detection of “engagement” of bone structures with bipolar injuries of the humeral head and articular surface of the scapula and more accurately plan surgical treatment.
