Данный способ лечения медиального вывиха коленной чашки у декоративных пород собак осуществляется путем использования методики периартикулярной чрескостной бедроберцовой контрпронационной стабилизации с применением аллотрансплантатов с одновременным выполнением миопластики коленной ветви двуглавого мускула бедра, блоковидной остеохондропластики межмыщелковой ямки дистального эпифиза бедренной кости и дубликатурой капсулы коленного сустава.This method of treating medial dislocation of the kneecap in decorative breeds of dogs is carried out using the technique of periarticular transosseous femoral torsion counterprotection stabilization using allografts with simultaneous myoplasty of the knee branch of the biceps femoris muscle, block osteochondroplasty of the intercondylar fossa of the distal and epiphyseal tubercle.
Из уровня техники не выявлено единого подхода к тактике лечения данной патологии, не разработан единый унифицированный подход к планированию оперативного лечения медиального вывиха коленной чашки 1-3 степени, по классификации Putnam's (1968 г), у декоративных пород собак. При хирургическом лечении вывихов 1-2 стадии есть данные, что не всегда требуется артротомия коленного сустава и может быть достаточным ограничение пронационной способности голени; при вывихах 2-4 стадии возможно выполнение латеральной транспозиции шероховатости большеберцовой кости; вывихи 3 и 4 стадии требуют увеличения углубления межмыщелковой ямки дистального эпифиза бедренной кости и корригирующей остеотомии бедренной и/или большеберцовой костей. Однако эффективность вышеуказанных методов, по данным разных авторов, составляет в среднем 65%, что диктует необходимость в разработке новых, более современных методов хирургического лечения вывиха коленной чашки у собак [1, 2, 4, 5, 7].From the prior art, a unified approach to the treatment tactics of this pathology has not been identified, a unified unified approach to the planning of surgical treatment of medial dislocation of the kneecap 1-3 degree, according to Putnam's classification (1968 g), in decorative dog breeds has not been developed. Способ хирургического лечения привычного медиального вывиха коленной чашечки у декоративных пород собак (массой тела до 7 кг), не сопровождающегося осевой деформацией костного остова тазовой конечности, основан на последовательном выполнении комплекса хирургических приемов, направленных на коррекцию биомеханики коленного сустава при вывихе коленной чашки, и осуществляется: артротомией коленного сустава; блоковидной остеохондропластикой межмыщелковой ямки дистального эпифиза бедренной кости; эндопротезированием латеральной коллатеральной бедроберцовой связки; миопластикой коленной ветви двуглавого мускула бедра и дубликатурой капсулы коленного сустава.The method of surgical treatment of the usual medial dislocation of the patella in decorative breeds of dogs (body weight up to 7 kg), not accompanied by axial deformation of the bony skeleton of the pelvic limb, is based on the sequential execution of a set of surgical techniques aimed at correcting the biomechanics of the knee joint during dislocation of the kneecap, and is carried out : arthrotomy of the knee; block osteochondroplasty of the intercondylar fossa of the distal epiphysis of the femur; arthroplasty of the lateral collateral femoral ligament; myoplasty of the knee branch of the biceps femoris muscle and duplicate capsule of the knee joint.
Хирургическую операцию по эндопротезированию аллотрансплантата латеральной коллатеральной связки коленного сустава с одновременным выполнением миопластики коленной ветви двуглавого мускула бедра, блоковидной остеохондропластикой межмыщелковой ямки дистального эпифиза бедренной кости и дубликатурой капсулы коленного сустава проводят в несколько этапов: Выполнение артротомии - кожу разрезают вдоль ближайшей к коленному суставу трети бедра и голени. Разрез проводят рядом с латеральным гребнем блока сустава и шероховатостью большеберцовой кости. Так же рассекают поверхностную фасцию, затем ее мобилизируют и широко разводят. Поверхностный листок глубокой фасции бедра рассекают около коленной чашки, затем проводят ревизию краев раны разорванной в результате вывиха фасции, бедрочашечной связки и капсулы сустава. Глубокий листок глубокой фасции рассекают медиально, рядом с коленной чашкой. Разрез продолжают дистально до шероховатости большеберцовой кости и проксимально вдоль латеральной головки четырехглавого мускула бедра. Результаты оперативного лечения вывиха коленной чашки данным методом доказывают, что для лечения собак декоративных пород с привычным медиальным вывихом коленной чашки 1-3 степени, не сопровождающимся варусной или вальгусной деформацией костей, целесообразно применять разработанную методику, состоящую из уменьшения пронации коленного сустава методом периартикулярной чрескостной бедроберцовой контрпронационной стабилизации с применением аллотрансплантатов с одновременным выполнением миопластики коленной ветви двуглавого мускула бедра, блоковидной остеохондропластикой межмыщелковой ямки дистального эпифиза бедренной кости и дубликатурой капсулы коленного сустава.
