Abstract
Description
The invention relates to medicine, in particular dermatovenerology and pathological anatomy, and is a method for diagnosing a lichenoid skin reaction.
Breathnach. Lichen planus and Lichenoid Disorders, editors. Rook's textbook of dermatology 8th ed. Singapore: Wiley-Blackwell; 2010, p.41).A known method for the diagnosis of lichenoid reaction by studying clinical, medical history and follow-up data (S.M. Breathnach. Lichen planus and Lichenoid Disorders, editors. Rook's textbook of dermatology 8th ed. Singapore: Wiley-Blackwell; 2010, p.41).
The disadvantage of this method is that in the clinical diagnosis of lichenoid reaction, instrumental methods are not used. In this case, the doctor relies only on non-specific clinical signs, such as psoriasiform papules, papule-eczematous rashes, accompanied by various degrees of desquamation. The listed clinical symptoms are characteristic of many chronic dermatoses (lichen planus, psoriasis, toxidermy), which leads to an increased likelihood of making an erroneous diagnosis at the initial examination, a skin biopsy (an invasive diagnostic method) and an extended diagnostic process.
A, Mays SR, Prieto VG, Curry JL.. Lichenoid Dermatologic Toxicity From Immune Checkpoint Blockade Therapy: A Detailed Examination of the Clinicopathologic Features. Am J Dermatopathol 2017; 39: 121-129). Closest to the claimed is a method for diagnosing a lichenoid reaction using a skin biopsy and subsequent histological examination. For this method, a dermatologist should be able to perform a skin biopsy, and a pathologist should have an idea of the features of the course of the lichenoid reaction and narrowly engage in skin pathomorphology (Tetzlaff MT, Nagarajan P, Chon S, Huen A, Diab A, Omar P, Aung PP, Torres-Cabala CA, Mays SR, Prieto VG, Curry JL .. Lichenoid Dermatologic Toxicity From Immune Checkpoint Blockade Therapy: A Detailed Examination of the Clinicopathologic Features. Am J Dermatopathol 2017; 39: 121-129). Meanwhile, in the territory of the Russian Federation there are a handful of specialists focused on the study of skin pathology. The disadvantage of the method chosen as a prototype is the almost complete absence of specialists (dermatologists) performing a biopsy of the skin, invasiveness of the research method with violation of the integrity of the skin, late diagnosis due to histological examination and the absence of specific morphological signs, which reduces the accuracy of the diagnosis.
Another disadvantage of this method is the difficulty of applying a histological study in clinical practice due to the lack of competent specialists in this field who have a sufficient understanding of both clinical manifestations and histopathological changes characteristic of this skin pathology.
The objective of the present invention is the early clinical diagnosis of lichenoid reaction for primary care physicians while increasing its accuracy.
The technical result of the task is achieved by the fact that in the method for diagnosing a lichenoid reaction of the skin, which consists in visual inspection, liquid is injected into the lesion and, when the epidermis is separated from the dermis, a lichenoid reaction is diagnosed.
The pathogenetic basis of the diagnostic effect of fluid injection is that it exerts pressure in the papillary layer of the dermis, where loose connective tissue and a large amount of inflammatory infiltrate, consisting of lymphocytes and histiocytes, are located. An important mechanism for separating the epidermis from the dermis when using fluid is that when the lichenoid reaction in the epidermis is observed hydropic (vacuole) dystrophy of the cells of the basal layer. Due to the expansion of the papillary layer by liquid, the epidermis (basement membrane, basal layer) is instantly separated from the dermis.
The application of this method allows you to accurately diagnose lichenoid skin reactions, conduct differential diagnosis with similar dermatoses, quickly determine the appropriate treatment tactics, and therefore improve the immediate and long-term results of treatment.
The introduction of fluid into the lesion, which is performed at the time the patient goes to the doctor, is important, since most patients come with a violation of the quality of life, excruciating itching and concomitant diseases. In clinical practice, timely accurate diagnosis and withdrawal of the drug that caused the lichenoid skin reaction is of great importance for the appointment of adequate therapy and prevention. In this case, a skin biopsy and violation of the integrity of the skin and other complications can be avoided.
The method is as follows. If localized or generalized psoriasiform papules, papulo-eczematous rashes similar to red flat lichen, with a history of taking medication in a home medicine kit, are identified in the patients who received, the fluid is injected into the lesion. The skin is treated with alcohol. A thin intradermal needle is injected into the skin at a shallow depth, 0.5 ml of liquid is slowly poured under the epidermis. A positive result is the separation of the epidermis from the dermis. To perform the procedure, it is recommended to use insulin syringes with a thin needle - 27 or 30 G. To make the injection less painful, compress the skin with fingers before folding it, and before making a puncture, warn the patient about this. The needle is injected intradermally (not subcutaneously) into the base of the papule with a slice down. Injections are carried out slowly, often pulling the piston toward themselves to check if the needle has entered the vessel.
Example 1. Patient G. 46 l was admitted to the Lenoblcenter of specialized types of medical care on March 13, 2017. with complaints of skin rashes of the trunk and limbs. Sick for 4 years, when for the first time appeared nodules with slight peeling on the surface in the back of the hands. When contacting a dermatologist, a diagnosis of lichen planus was established, and external hormone therapy and the use of emollients were prescribed. When re-examined due to the lack of effect from therapy, the drug plaquenil with a temporary positive clinical effect is recommended. In the next few years, the disease progressed: as before, papules began to appear on the trunk, upper and lower extremities, the number of elements increased significantly. At the time of the examination, a large number of flat papules with slight peeling on the surface, red in color, resembling the clinical manifestations of lichen planus, not merging with each other, were noted in the area of the back surface of the hands, trunk, and hips.
With the introduction of fluid into the lesions in different parts of the body, a separation of the epidermis from the dermis was observed. A detailed history of the patient revealed periodic intake of Citramon tablets for headache.
Based on the proposed diagnostic method, the diagnosis was made: lichenoid skin reaction. It is recommended that you first stop taking the tablets of your first-aid kit and consult a physician in order to replace these drugs with other groups.
Example 2. Patient B. 48 l. turned to the Lenobltsentr specialized types of medical care 12.12.2016, with a clinical picture of lichen planus and psoriasis. On examination, numerous papules were found at different stages of development, in places merging with each other with slight peeling on the surface.
In accordance with the inventive method, 0.5 ml of liquid was introduced per lesion. At the time of the procedure, the epidermis was clearly separated from the dermis, which led to the diagnosis of "lichenoid reaction." The cancellation of all drugs is recommended, and external hormonal therapy with emollients is prescribed. When re-examined after a month, the skin was free of rashes.
The inventive method is a simple, accurate, fast and budgetary method for early diagnosis of lichenoid reaction. The implementation of the proposed method can be carried out at the time of examination of the patient and is convenient, especially for primary care physicians. Given the absence of specific clinical manifestations, the difficulties of differential diagnosis with similar diseases, such as psoriasis and lichen planus, a small number of medical institutions where a skin biopsy is performed routinely, a small number of dermatopathologists focused on studying skin pathology, the time factor and the accuracy of the diagnosis are key in deciding on the appointment of specific therapy.
The inventive method is available to doctors of any specialty, does not require expensive equipment and a large number of laboratory and instrumental studies. The method allows to diagnose a lichenoid reaction in the early stages, to prevent a delay in the appointment of treatment to patients in need of therapy, and to choose the patient management tactics, as well as to prevent the disease.
Along with this, the method allows to reduce the time of diagnosis and reduce the costs of studying both exogenous and endogenous antigens using patch tests.
