Abstract
Description
The invention relates to medicine, namely to ophthalmology, and can be used to prevent recurrence of pterygium after surgical treatment: excision of pterygium with conjunctival autoplasty.
A known method for the prevention of recurrence of pterygium after surgical treatment (http://www.vseozrenii.ru/glaznye-bolezni/pterigium/), which consists in the instillation of drugs from the first day of treatment, including antibacterial and anti-inflammatory drops.
However, the use of these drops alone is not enough, since in the early postoperative period against the background of postoperative inflammation, the processes of reparative regeneration and fibrotization of the tissues of the postoperative zone are active. Moreover, against the background of local hypoxia in the area of the postoperative corneal wound, vascularization of the limbic zone is possible with further progressive growth of the vasculature in the cornea outside the limb. After removal of the pterygium, the superficial conjunctival epithelium is damaged, and the mechanism of enhanced collagen production is triggered. In the presence of postoperative scars in the area of excision of the pterygium and while maintaining the instability of the tear film, the pathogenetic chain does not break and the process recurs, despite the barrier function of the conjunctival autolap. In addition, the result of surgical intervention is traumatic damage to the tissues of the eye, which means, as in any other organ, it develops the processes of reparative regeneration aimed at restoring lost structures.
The present invention solves the problem of creating a new method for the prevention of relapse of pterygium after surgical treatment. The resulting technical result consists in increasing the effectiveness of preventive measures to reduce the likelihood of recurrence of pterygium after surgical treatment. Given the fact that there is a clear relationship between the development of pterygium and dysfunction of the tear film, surgical intervention results in damage to the surface conjunctival epithelium, a violation of the lacrimal film, which triggers the mechanism of enhanced collagen production. The presence of postoperative scars in the area of excision of the pterygium, while maintaining the instability of the tear film, exacerbates the situation, the pathogenetic chain does not break, and the process recurs, despite the barrier function of the conjunctival autologous flap. To stabilize the tear film from the first day after the operation, additional instillations of tear substitutes are carried out, which has a positive effect on reducing the frequency of relapse of pterygium after surgical treatment.
Against the background of the use of tear replacement drugs after complete regeneration of the cornea and removal of sutures in the postoperative period, they additionally instill one of the steroidal anti-inflammatory drugs for at least 10 days, and one of the antioxidants, for example, emoxipin, for at least one month. Dexamethasone is used as a steroidal anti-inflammatory drug, and emoxipin as an antioxidant.
Comprehensive treatment of diseases associated with the violation of trophic processes in tissues is impossible without the use of angioprotectors and antioxidants. The drug emoxipin has an antioxidant effect on free radical complexes through a neutralizing effect, improves and increases the penetration of oxygen through membrane structures, maintains the integrity and stability of the most important structures: enzymes, DNA, proteins, cell membranes and fights tissue hypoxia. Thanks to these eye drops, the chain of oxidative processes at the cellular and tissue level is suspended. Dexamethasone inhibits the proliferation of fibroblasts and their synthesis of collagen, inhibiting the growth of connective tissue, which prevents the formation of coarse scars, reduces the penetration of the vascular walls and stops the process of recurrence of pterygium after surgical treatment. The complex used in the postoperative period of drugs aimed at correcting inflammatory phenomena and the implementation of actions leading to a decrease in scarring after surgical treatment, ensures the achievement of the claimed positive result when using the proposed method.
The specified technical result is achieved by the fact that in the method of preventing relapse of pterygium after surgical treatment, which consists in the instillation of drugs from the first day of treatment, including antibacterial and anti-inflammatory drops, from the first day after the operation, additional instillations of lacrimal preparations are carried out, on the background of which after full regeneration cornea and suture removal in the postoperative period additionally carry out instillation with one of the steroid -inflammatory drugs for at least 10 days and one of the antioxidants for at least one month. The specified technical result is also achieved by the fact that dexamethasone is used as a steroidal anti-inflammatory drug, and emoxipin is used as an antioxidant.
A method for the prevention of recurrence of pterygium after surgical treatment is as follows.
When removing pterygium from the first day of treatment, instillations of drugs are performed, including antibacterial and anti-inflammatory drops. After removal of the pterygium, consisting in the excision of the pterygium with conjunctival autoplasty, from the first day after the operation, additional instillations of lacrimal preparations are carried out.
