Прямоходный патрубок трехходового крана заглушают запорным блокирующим устройством ИН-стоппер (B/Braun), через резиновую мембрану которого проводят в просвет эпидуральной иглы катетер, и выдвигают его за срез иглы в сторону головы на расстояние 4-5 см внутри эпидурального пространства. К эпидуральному катетеру присоединяют шприц с физиологическим раствором, а к боковому патрубку трехходового крана - шприц с раствором местного анестетика. Способ применяют следующим образом.The method is used as follows.
Для проведения эпидуральной анестезии в асептических условиях и под местной анестезией (лидокаин 0.5% - 5.0) выполняют катетеризацию эпидурального пространства на уровне L3-L4. Выполняют пункцию эпидурального пространства иглой Тюохи. Вводят 4-5 мл 2% лидокаина в качестве так называемой, тест-дозы с целью идентификации случайного и непреднамеренного прокола твердой мозговой оболочки. В случае отрицательного теста подсоединяют к павильону иглы трехходовый кран «Дискофикс» 360° (B/Braun), прямоходный патрубок трехходового крана заглушают запорным блокирующим устройством ИН-стоппер (B/Braun), через резиновую мембрану которого проводят в просвет эпидуральной иглы катетер и выдвигают за срез иглы на расстояние 4-5 см внутри эпидурального пространства в сторону головы. К эпидуральному катетеру присоединяют шприц с 15 мл физиологического (индифферентного) раствора. К боковому патрубку трехходового крана присоединяется шприц с местно-анестезирующим (рабочим) препаратом (15 мл 0.75% ропивакаин, или 0.5% бупивакаин, или 2% лидокаин). Регулирующий кран трехходового крана «Дискофикс» устанавливают в положение, позволяющее вводить два раствора одновременно. Растворы вводят медленно, в течение 3 минут. Через 15-20 минут поясничные и крестцовые спинальные нервы, оказавшись в зоне блока, анестезируются, а зона эпидурального блока по высоте ограничивается уровнем L1, что соответствует расположению конца катетера внутри эпидурального пространства. 1. Пациент Ш-н В.К., 63 г., поступил с переломом правой бедренной кости 10.12.2015. Сопутствующие заболевания: хроническая ишемическая болезнь сердца. Операция остеосинтеза перелома кости под эпидуральной анестезией по заявленному способу. Выполнена катетеризация эпидурального пространства на уровне L3-L4 и одноуровневая эпидуральная пункция. Эпидуральный катетер провели через просвет эпидуральной иглы и выдвинули за ее срез на расстояние 4-5 см внутри эпидурального пространства в сторону головы. Синхронно и соосно, медленно, в течение 3 минут, ввели физиологический раствора через катетер, а местно - анестезирующий раствор через просвет эпидуральной иглы. Проведена операция. Гемодинамика стабильная, кровопотеря около 200 мл, пациент переведен после операции в профильное отделение в удовлетворительном состоянии.1. 2. Пациент К-в Р.И, 50 лет, поступил с переломом левой большеберцовой кости 11.12.2015. Сопутствующие заболевания: гипертоническая болезнь 2 ст. Проведена операция остеосинтеза перелома левой большеберцовой кости. Под эпидуральной анестезией по заявленному способу выполнена катетеризация эпидурального пространства на уровне L3-L4 и одноуровневая эпидуральная пункция. Эпидуральный катетер провели через просвет эпидуральной иглы и выдвинули за ее срез на расстояние 4-5 см внутри эпидурального пространства в сторону головы. Синхронно и соосно, медленно, в течение 3 минут, ввели физиологический раствор через катетер, а местно - анестезирующий раствор через просвет эпидуральной иглы. В период операции и по ее завершении гемодинамика стабильная, кровопотеря около 100 мл, пациент переведен в профильное отделение в удовлетворительном состоянии.2.
