Вместе с тем, такие операции нередко проводятся в условиях высокого анестезиологического риска, связанного с наличием у пациента тяжелой сопутствующей патологии. В этих условиях большое значение для успешной операции имеет правильно подобранный вариант анестезиологического пособия.Abdominal operations are among the most common in surgery. Одним из перспективных направлений в анестезии является использование ксенона. Так, известны способы анестезии с использованием ксенона (RU 2102068, 1998; RU 2195327, 2002), включающие в себя премедикацию, интубацию трахеи с последующей денитрогенизацией, однако проведение наркоза ксеноном в режиме моноанестезии не отвечает требованиям к анестезиологическому пособию в хирургии пациентов высокого риска, так как подобные операции отличаются высокой травматичностью и обширностью и анестезия при подобных вмешательствах должна быть упреждающей и многоуровневой, с акцентом на превентивную блокаду патологической афферентной импульсации из операционной раны. В этой связи более надежным является использование ксенона в рамках сочетанной анестезии.One of the promising areas in anesthesia is the use of xenon. Наиболее близким по технической сущности к заявляемому способу является способ сочетанной анестезии при высокотравматичных операциях у онкологических больных, включающий общую анестезию ксеноном и эпидуральную анальгезию путем постоянной инфузии 0,2%-ного раствора ропивакаина в сочетании с фентанилом и адреналином (RU 2444380, 2011). Способ осуществляется следующим образом. После поступления больного в операционную осуществляют пункцию эпидурального пространства на уровне Т4-T11 с последующим введением 40 мг лидокаина. Далее осуществляют инфузию анальгетической смеси, состоящей из 47 мл 0,2% ропивакаина, 2 мл 0,005% фентанила и 0,1 мл 0,1% адреналина со скоростью 15 мл⋅час-1 в течение 20 минут, после чего скорость введения снижают до 7-10 мл⋅час-1. Через 20-25 мин после начала эпидурального введения смеси внутривенно вводят 2 мл 0,005% фентанила и 0,5-0,6 мл 0,1% атропина. Затем осуществляют индукцию пропофолом в дозе 2-2,5 мг⋅кг-1 с последующей интубацией больного. Денитрогенизацию проводят в течение 8-10 мин 100% кислородом по полуоткрытому контуру. В период денитрогенизации проводят внутривенную анестезию пропофолом в дозе 4 мг⋅кг-1⋅ч-1.
