Abstract
Description
The invention relates to x-ray surgical methods for the treatment of coronary heart disease.
(QCA, qualitive coronary angiography) U Sigwart and others, 'Intravascular Stents to Prevent Occlusion and Restenosis after Transluminal Angioplasty.', The New England Journal of Medicine, 316 (1987), 701-6 <http://dx.doi.org/10.1056/NEJM198703193161201>. Currently, there is a method for the treatment of significant stenosis of the coronary arteries by implantation of self-absorbable vascular scaffolds. According to the traditional implantation technique, at the initial stage of the operation, the reference diameter of the vessel is estimated by quantitative coronary angiography (QCA, qualification coronary angiography) U Sigwart and others, 'Intravascular Stents to Prevent Occlusion and Restenosis after Transluminal Angioplasty.', The New England Journal of Medicine, 316 (1987), 701-6 <http://dx.doi.org/10.1056/NEJM198703193161201>. After the preparation of the target section of the vessel by predilation, the self-absorbable skeleton is implanted by inflating the balloon.
В мировой практике отмечены случаи подострого тромбоза в скафолде, вызванные эксцентричным раскрытием страт саморассасывающегося сосудистого каркаса [, Sofia Pereira and Daniela de Araujo, 'Bioresorbable Vascular Scaffold Collapse Causes Subacute Thrombosis.', The Journal of Invasive Cardiology, 26 (2014), E98-99 <http://www.ncbi.nlm.nih.gov/pubmed/24993999>; and others, 'Subacute Drug-Eluting Stent Thrombosis Caused by Stent Underexpansion: Evaluation by Optical Coherence Tomography', Case Reports in Medicine, 2011 (2011), 1-3 <http://dx.doi.org/10.1155/2011/129341>]. In world practice, there have been cases of subacute thrombosis in a scaffold, caused by the eccentric opening of the strata of a self-absorbable vascular skeleton [ , Sofia Pereira and Daniela de Araujo, 'Bioresorbable Vascular Scaffold Collapse Causes Subacute Thrombosis.', The Journal of Invasive Cardiology, 26 (2014), E98-99 <http://www.ncbi.nlm.nih.gov/pubmed/ 24993999>; and others, 'Subacute Drug-Eluting Stent Thrombosis Caused by Stent Underexpansion: Evaluation by Optical Coherence Tomography', Case Reports in Medicine, 2011 (2011), 1-3 <http://dx.doi.org/10.1155/2011/ 129341>]. For the development of such a formidable complication, there are a number of prerequisites laid down in the design features of the scaffolds and the properties of its material:
А. [John A. Ormiston and Patrick W S Serruys, 'Bioabsorbable Coronary Stents', Circulation: Cardiovascular Interventions, 2009, 255-60 <http://dx.doi.org/10.1161/CIRCINTERVENTIONS.109.859173>].A. Self-absorbable vascular scaffold consists of a mixture of L- and D-polymers of lactic acid, which is an X-ray negative substance. Consequently, self-absorbable vascular scaffolds are invisible during x-ray [John A. Ormiston and Patrick WS Serruys, 'Bioabsorbable Coronary Stents', Circulation: Cardiovascular Interventions, 2009, 255-60 <http://dx.doi.org/10.1161/CIRCINTERVENTIONS.109.859173 >].
Б. [Patrick W. Serruys and others, 'Evaluation of the Second Generation of a Bioresorbable Everolimus-Eluting Vascular Scaffold for the Treatment of de Novo Coronary Artery Stenosis: 12-Month Clinical and Imaging Outcomes', Journal of the American College of Cardiology, 58 (2011), 1578-88 <http://dx.doi.org/10.1016/j.jacc.2011.05.050>]. B. Insufficient radial stiffness of the self-absorbable skeleton leading to an eccentric, slit-like opening [Patrick W. Due to the invisibility of the stat, the eccentricity of the scaffold remains invisible on radiography.
В свете этих предположений, казалось бы логично избегать недораскрытия путем рутинной постдилатации всех скафолдов некомплаинсными баллонами [Barbanti М. and others, 'Impact of Balloon Post-Dilation on Clinical Outcomes after Transcatheter Aortic Valve Implantation with the Self-Expanding Prosthesis', EuroIntervention, 2014 <http://www.pcronline.com/eurointervention/AbstractsEuroPCR2014/288\n http://ovidsp.ovid.com/ovidweb.cgi?T=JS&PAGE=reference&D=emed11&NEWS=N&AN=71538627>]. In light of these assumptions, it would seem logical to avoid under-disclosure by routine post-dilatation of all scaffolds with non-compliance balloons [Barbanti M. and others, 'Impact of Balloon Post-Dilation on Clinical Outcomes after Transcatheter Aortic Valve Implantation with the Self-Expanding Prosthesis', EuroIntervention, 2014 <http://www.pcronline.com/eurointervention/AbstractsEuroPCR2014/288\n http://ovidsp.ovid.com/ovidweb.cgi?T=JS&PAGE=reference&D=emed11&NEWS=N&AN=71538627>]. However, current studies have not been able to prove the effectiveness of this method. In addition, mandatory post-dilatation leads to an increase in the time of the operation, the dose of radiation and contrast medium, and the cost of the procedure. And additional temporary occlusion of the vessel with a balloon can increase the number of side effects.
The eccentricity of the opening can be detected using additional methods of intravascular imaging, such as optical coherence tomography (OCT) and intravascular ultrasound (IVUS). However, not all clinics have in their arsenal equipment for these studies. In addition, additional studies also lead to an increase in the time of the procedure, the dose of radiation and contrast medium, and its cost. And the need for a single administration of a large dose of contrast in OCT alone can cause adverse side effects.
[Sigwart and others, 'Intravascular Stents to Prevent Occlusion and Restenosis after Transluminal Angioplasty.', The New England Journal of Medicine, 316 (1987), 701-6].The prototype of the invention is a method for assessing the adequacy of the disclosure of implantable coronary intravascular scaffolds by angiography [Sigwart and others, 'Intravascular Stents to Prevent Occlusion and Restenosis after Transluminal Angioplasty.', The New England Journal of Medicine, 316 (1987), 701-6].
The eccentric opening of self-absorbable vascular scaffolds can cause thrombosis, with the development of acute coronary syndrome [ , Pereira and de Araujo]. Due to the invisibility of the scaffold material in x-ray radiation, there is a problem of timely detection of this complication. After installing the scaffold and removing the balloon, the eccentric opening may remain invisible due to the flow of contrast along the periphery of the striations and the creation of a falsely successful picture.
The objective of the invention is to develop a method for evaluating the eccentricity of the disclosure of an implantable absorbable framework.
The technical result when using the invention is to increase the accuracy of the assessment.
The invention is illustrated in the drawing, which shows the X-ray diffraction in several projections to identify the eccentricity of the disclosure of the scaffold.
The proposed method for evaluating the eccentricity of the opening of the implantable absorbable skeleton is as follows. At the time of installation of the scaffold, X-rays are performed in 4 classical projections: the right caudal and cranial, the left caudal and cranial to visualize the self-absorbable vascular skeleton (drawing). The eccentricity index (iE) is calculated by the formula:
где Dmax - максимальный диаметр артерии, определяемый в момент раздутия баллона.where Dmax is the maximum diameter of the artery, determined at the time of ballooning.
Dmin - the minimum diameter of the artery, determined at the time of ballooning.
If the value of iE is more than 0.2, the eccentricity of the implantable absorbable scaffold is evaluated as significant and post-dilatation is performed with a non-compliant balloon.
An eccentricity index of more than 0.2 is significant due to the fact that with a given degree of eccentricity, a turbulent flow may appear, predisposing to thrombosis in the skeleton. With an eccentricity index value of less than 0.2, post-dilatation is not required.
The invention is illustrated by the following examples.

A 67-year-old patient was admitted to the department of X-ray surgery with a diagnosis of Coronary heart disease. At the time of balloon inflating, coronarography was performed in 4 orthogonal projections: right caudal and cranial, left caudal and cranial. The eccentric opening of the stent was revealed (drawing), Dmax = 3.5 mm and Dmin = 2.9 mm, the eccentricity index was 0.17. In this case, the eccentricity is recognized as insignificant, the patient was discharged on the 4th day after the operation. On the control coronarography, 1 year after the operation, there were no signs of restenosis, thrombosis.
