Известен способ оценки гипермобильности суставов с помощью тестов шкалы Бейтона с балльным счетом возможности обследуемого выполнить 5 движений, 4 парных для конечностей и одного непарного для туловища: 1. пассивно привести I палец кисти к предплечью; 2. пассивно отогнуть назад V палец в пястно-фаланговом суставе более, чем на 90°; 3. пассивно разогнуть локтевой сустав ≥10°; 4. пассивно разогнуть коленный сустав ≥10°; 5. интенсивно прижать ладони к полу, не сгибая коленей. Суть модификации заключается в дополнении тестов Бейтона «синдромом Паганини» на определение объема движений в суставах кистей для рук (9 тестов): 1. активная ульнарная девиация II-V пальцев кисти за счет отклонения в пястно-фаланговых и межфаланговых суставах; 2. активное переразгибание II-V пальцев кистей рук ≥90°; 3. переразгибание запястно-пястного сустава большого пальца кисти; 4. выступание ногтевой фаланги за ульнарный край ладони при фиксации большого пальца поперек ладони; 5. пассивное приведение большого пальца к тыльной стороне кисти руки; 6. активное сгибание в дистальных межфаланговых суставах II-V пальцев кистей рук, которое может напоминать «шею лебедя»; 7. поперечное растяжение пальцев в противоположном направлении в пястно-фаланговых суставах в виде шпагата, этот тест проверяется попарно в II-III, III-IV пальцах кисти; 8. пассивное поочередное укладывание III, IV, V пальцев кистей друг на друга, «палец-улитка»; 9. поворот руки на 360° в плечевом и локтевом суставе [Schilling F. Способ включает определение объема движений в суставах кистей (при проведении 9 тестов): 1 - активной ульнарной девиации II-V пальцев кисти (за счет отклонения в пястно-фаланговых и межфаланговых суставах), 2 - активного переразгибания II-V пальцев кистей рук ≥90°; 3 - выступания ногтевой фаланги за ульнарный край ладони при фиксации большого пальца поперек ладони; 4 - пассивного приведения большого пальца к тыльной стороне кисти руки; 5 - переразгибания запястно-пястного сустава большого пальца кисти; 6 - активного сгибания в дистальных межфаланговых суставах II-V пальцев кистей рук; 7 - поперечного растяжения пальцев в противоположном направлении в пястно-фаланговых суставах в II-III, III-IV пальцах кисти; 8 - пассивного поочередного укладывания III, IV, V пальцев кистей друг на друга; 9 - поворота руки на 360° в плечевом и локтевом суставе. Дополнительно выявляют из анамнеза жизни возраст дебюта суставных болей и определяют наличие симптома «щелкающего» бедра (чувство щелчка в тазобедренном суставе, систематически возникающее при вставании, ходьбе или вращательных движениях ногой) [Wahl CJ et al: Internal coxa saltans (snapping hip) as a result of overtraining: a report of 3 cases in professional athletes with a review of causes and the role of ultrasound in early diagnosis and management. При наличии у лиц с гипермобильностью суставов дебюта суставных болей в возрасте до 38 лет и четырех и более выполненных тестов на объем движений в суставах кистей риск развития остеоартроза составляет 100%. У лиц с гипермобильностью суставов, дебютом суставных болей в возрасте 38 лет и более, наличием двух и более выполненных тестов на объем движений в суставах кистей риск развития остеоартроза составляет 69,6%. У лиц с гипермобильностью суставов, дебютом суставных болей в возрасте 38 лет и более, выполнением менее двух тестов на объем движений в суставах кистей и наличием симптома «щелкающего» бедра риск развития остеоартроза составляет 25%. У лиц с гипермобильностью суставов, дебютом суставных болей в возрасте 38 лет и более, выполнением менее двух тестов на объем движений в суставах кистей при отсутствии симптома «щелкающего» бедра риск развития остеоартроза составляет 0%. A method for predicting the risk of osteoarthritis in people with joint hypermobility was developed using the classification tree method based on a combination of tests performed for the amount of movement in the joints of the hand, the age of the onset of joint pain, and the symptom of a “clicking” hip (table 1).
Технический результат: предлагаемый способ позволяет повысить точность прогноза риска возникновения остеоартроза у лиц с гипермобильностью суставов. Клинические наблюдения свидетельствуют о том, что использование предлагаемого технического решения позволяет осуществить прогноз риска возникновения остеоартроза у лиц с гипермобильностью суставов в процентном отношении. При проведении ROC-анализа оценки качества заявляемой модели прогноза риска возникновения остеоартроза у пациентов с гипермобильностью суставов диагностическая чувствительность составила 82,0%), специфичность - 88,3%, эффективность - 85,2% (рисунок 1).Effect: the proposed method improves the accuracy of predicting the risk of osteoarthritis in individuals with hypermobility of the joints. I, ФН I на фоне гипермобильности суставов (7 баллов по шкале Бейтон) Из амнеза жизни определено, что боли в коленных суставах беспокоят с 20 лет. При объективном обследовании симптом щелкающего бедра отрицательный. При определении дополнительного объема движения в кистях из девяти тестов выполнено четыре: активное переразгибание II-V пальцев кистей рук ≥90°; пассивное приведение большого пальца к тыльной стороне кисти руки; активное сгибание в дистальных межфаланговых суставах II-V пальцев кистей рук; пассивное поочередное укладывание III, IV, V пальцев кистей друг на друга. На основании клинического обследования и анамнеза жизни выявлена следующая комбинация признаков: возраст дебюта боли в суставах <38 лет, выполнение ≥4 тестов на объем движений в суставах кистей. Клинические исследования заявляемого способа проводились с 2011 по 2014 год на базе поликлиники бюджетного учреждения здравоохранения Омской области (БУЗОО) «Городская клиническая больница №1 им. Кабанова А.Н.». Всего был обследован 61 пациент с гипермобильностью суставов в возрасте от 21 до 61 года. Для прогнозирования риска возникновения остеоартроза у пациентов с гипермобильностью суставов применялся метод дерева классификаций. Дерево классификации позволяет построить группы пациентов с разными уровнями рисков развития остеоартроза (высоко-, средне-, низкорисковые классы) на основе комбинации значимых показателей. Дерево классификации представляет собой простую интуитивно понятную структуру в виде дендрограммы, на которой в каждом узле указаны объем выделенной группы, риск развития остеоартроза (рисунок 2).Clinical studies of the proposed method were carried out from 2011 to 2014 on the basis of the clinic of a budgetary health institution in the Omsk Region (BUZOO) "City Clinical Hospital No. 1 named after Kabanova A.N.
