Description
6.The invention relates to the field of pharmaceuticals and medicine and relates to a simple method for producing a storage stable and effective in its specific action drug composition for photodynamic therapy (PDT) based on chlorin E6 and phospholipid nanoparticles with a high percentage of chlorin E6.
One of the modern approaches in the development of new-generation drugs is the construction of original forms based on substances of a known spectrum of activity using modern technologies that make it possible to obtain drugs with high bioavailability, therapeutic efficacy and reduced side effects.
Nanotechnology has great potential. Their use in medicine has now become possible due to the unique properties of new nanomaterials and technologies that have emerged, which make it possible to construct new biologically active nanocompositions.
New technologies that can "extend the life" of known drugs include the development of drug transport systems in the body.
The most significant successes were achieved in the development of transport systems based on phospholipid nanoparticles, which have several advantages over others. They are non-toxic, biodegradable, do not cause allergic reactions, have a high affinity for cell membranes. One of the earliest systems of transport of biologically active substances in the body was liposomes.
Chlorin E6 is the most effective photosensitizer successfully used in medical practice for the treatment of cancer and other neoplasms by the method of photodynamic therapy (FTD). Chlorin E6 has a number of spectral, physico-chemical and energy characteristics that distinguish it from other photosensitizers. For example, (1) the presence of an absorption band in the long wavelength region of the visible spectrum (with a maximum of 662 nm in biological media), which corresponds to the region of greatest optical transparency for biological tissues; (2) high contrast index; (3) a high level of accumulation in target tissues compared to intact tissues; (4) high photodynamic activity when using small doses; (5) low dark and light toxicity; (6) fast, within 24-36 hours, elimination from the body, etc.
For the first time, water-soluble chlorophyll derivatives were used by E. Snyder (USA) in 1942, and were used for the prevention and treatment of cardiovascular diseases, atherosclerosis, and rheumatoid arthritis.
In the scientific literature on the use of derivatives of the chlorin series for photodynamic therapy was announced in 1986. In 1994-2001. in Russia A.V. Reshetnikov [1] developed a technology for extracting a complex of biologically active chlorins from plant materials, which contain E6 chlorin as the main component, the photocytotoxic effect of which on the tumor is enhanced, and overall pharmacological parameters are improved due to two other natural chlorins contained in the extract.
Over the past 30 years, intensive searches have been conducted for the most effective photosensitizers. Currently, around 20 different compounds for PDT have been developed and are undergoing clinical trials in the world.
Given that FTD has been included in the treatment standards since 2010, chlorin E6 and its derivatives are in demand on the domestic pharmaceutical market and are increasingly used, for example, for the treatment of cancer of the cervix, bladder, and various mucous membranes. However, despite the demand, chlorine E6 as a substance has a number of disadvantages that limit its widespread consumption, namely: (1) a short shelf life and loss of optical properties; (2) as a rule, the liquid form of various finished preparations; (3) elimination from the body for 48 hours or more, etc.
Therefore, the search for new and the development of effective forms of chlorin E6 as a drug for PDT and methods for their preparation is an urgent and timely task.
One of the most convenient, suitable for PDT dosage forms of chlorin E6, possessing a number of the above advantages over others, is a composition based on chlorin E6 included in phospholipid nanoparticles.
At the same time, since chlorin E6 is a hydrophobic compound, the preparation of phospholipid nanoparticles with chlorine E6 included in them is a rather complicated problem.
Known pharmaceutical composition for PDT based on phospholipid nanoparticles containing up to 90% E6 chlorin (Strekalova OS “Phospholipid nanoparticles: preparation, characterization, use for transporting drugs in the body”, Abstract of dissertation for the degree of candidate of biological sciences, Moscow, 2010). However, in this document, a method for producing phospholipid nanoparticles with drugs included in them, in particular chlorin E6, and the preparation of the corresponding compositions based on them are not described.
There is also known a method for producing phospholipid compositions of drugs in the form of phospholipid nanoparticles. Moreover, in the case of hydrophobic drugs, the drug and phosphatidicholine are dissolved in ethanol, ethanol is distilled off, water is added, suspended, maltose is added and the resulting suspension is homogenized (RF patent 2391966, 06/20/2010). This requires the use of a solvent such as ethanol, and its subsequent distillation, which, on the one hand, complicates the process, and on the other hand, the use of any additional and, at the same time, sufficiently toxic agents is undesirable in the manufacture of pharmaceutical compositions.
6.The objective of the present invention is to develop a simple method for producing a medicine based on chlorin E6 included in phospholipid nanoparticles for photodynamic therapy that does not require the use of sufficiently toxic solvents and provides phospholipid nanoparticles with a high, up to 93%, content of chorin E6.
In accordance with the invention, a method is described for preparing a medicament for photodynamic therapy based on chlorin E6 and phospholipid nanoparticles, characterized in that the aqueous solution of maltose obtained by heating is mixed with chlorine E6, followed by mixing with an aqueous emulsion of phosphatidylcholine and the resulting emulsion is subjected to several homogenization cycles under high pressure of 500-1000 atm at a temperature of 40-50 ° C, followed by neutralization with an aqueous solution of alkali and lyophilization.
93%.The implementation of the above method provides a means for PDT based on chlorin E6 included in phospholipid nanoparticles up to 30 nm in size and having a high percentage of inclusion of chlorin E6 in nanoparticles up to 93%.

6.In in vivo experiments on mice inoculated with tumor LLC, the comparative efficacy of the specific action of chlorin E6 embedded in phospholipid nanoparticles (Nanochlorin) was studied. As a comparison drug used the commercial drug Radachlorin, in which the active substance is the glucurin salt of chlorin E6.
Для Нанохлорина торможение роста опухоли (ТРО) составляет 96,1-100%, увеличение продолжительности жизни (УПЖ) - 57,0%, критерий излеченности (КИ) - 50,0%; для препарата "Радахлорин", соответственно ТРО составляет 93,1-100%, УПЖ - 63,3%, КИ - 60,0%.A comparison of the photoactivity of Nanochlorin and Radachlorin showed that both drugs have high antitumor activity under the same conditions for photodynamic therapy (PDT). For Nanochlorin, the inhibition of tumor growth (SRW) is 96.1-100%, the increase in life expectancy (ALC) is 57.0%, the cure rate (CI) is 50.0%; for the drug "Radachlorin", respectively, SRW is 93.1-100%, VLF - 63.3%, CI - 60.0%.
Preliminary studies of the toxicity of the drug "Nanochlorin", intended for the photodynamic treatment of malignant neoplasms, were also carried out on two types of laboratory animals - mice and rats.
