Abstract

SUBSTANCE: invention refers to medical equipment and may be used in surgical interventions from a laparotomy approach. An apparatus consists of two elongated branches whereon holes are checkered, and provided with buttons with two holes in the middle. Each branch has pivoting devices. One of the branches is provided with a ratchet screw assembly.

EFFECT: possibility to create a programmed laparostoma.

Description

The invention relates to medical equipment and can be used in surgical interventions with laparotomic access.

There are various devices for creating a laparostomy that are used during periodic examinations, sanitation of internal organs, branches of various configurations (Semenov G.M. Modern Surgical Instruments. -St. Petersburg, 2006, pp. 264-265, Fig. 75.).

But the disadvantages of these devices are:

- the inability to use for repeated continuous rehabilitation;

- the possibility of the formation of purulent streaks between the device and the edge of the wound, which can lead to complications, in particular to phlegmon.

The closest analogue prototype is a retractor used during abdominal operations (Semenov G.M. Modern Surgical Instruments. St. Petersburg, 2006, pp. 264-265, Fig. 75, d). It is a retractor, consisting of a jaw and a screw device with a handle. On the distal parts of the branch there are edges curved inward. With these edges, the branches cling to the edges of the laparotomic access for its expansion during surgery.

The disadvantage of the analogue prototype is:

- the impossibility of creating a programmable laparostomy;

- the ability to use only during surgery.

The objective of the invention is to propose a device that can be used to create a programmable laparostomy, that is, repeated expansion of the wound.

Technically, the result is a retractor that can be used to create a programmable laparostomy.

In Fig.1 shows the proposed retractor, front view, in Fig.2 - buttons.

The technical result is achieved in that the retractor according to the invention is a removable structure consisting of 2 parts 1 and 2 (Fig. 1), each of which is equipped with branches 3, 4 (Fig. 1). On branches 3 and 4 (Fig. 1), holes 5 are staggered in two rows. Each jaw 3 and 4 (FIG. 1) is also provided with hinge devices 6 (FIG. 1), so that the jaws 3 and 4 can rotate up and down. On the jaw 4 (Fig. 1) a hinge device 7 (Fig. 1) is made, which allows the jaw 4 to be diverted from the jaw 3 at an angle. The jaw 3 is equipped with a hinge device 8 for advancing along its toothed screw device 9. All hinge devices are equipped with handles 10 (Fig. 1).

Figure 2 presents the buttons 11 with a diameter of 1.5 cm with two holes 12 (figure 2) with a diameter of up to 2 mm in the center.

The retractor may be disposable.

The design is used as follows. The retractor is fixed for the time of abdominal sanitation to the edges of the laparotomy wound over the skin with non-absorbable atraumatic suture material. In this case, the needle is passed through the buttons 11 (Figure 2) to prevent injury by the threads of the peritoneum, and all layers of the laparotomy wound. The buttons 11 are located on the back surface of the peritoneum. The edges of the wound do not suture, but bring them together by rotating the branches of the retractor. If necessary, in the rehabilitation of the abdominal cavity, the branches of the retractor are bred in any planes: sagittal, frontal and horizontal.

The advantages of the proposed device.

- the ability to use the device for any need for rehabilitation of the abdominal cavity.

- removal of the retractor is carried out by cutting threads anywhere.

- the possibility of repeated expansion of the wound during rehabilitation of the abdominal cavity.

A retractor containing two jaws with a gear device for moving one jaw along the gear device and with hinged devices for breeding the jaw in the sagittal, frontal and horizontal planes, characterized in that it further comprises buttons to prevent peritoneal injury by threads that are made with two central holes for holding the needle, while the jaws are made with holes for attachment to the anterior abdominal wall located in a checkerboard pattern and have the ability to rotate niya to reduce the edges of the wound, and one of the branches is equipped with a screw device for moving along the other branches.
