Abstract
Description
Изобретение относится к области медицины, а именно, предназначено для оценки индивидуального ответа мотонейронов пораженной и здоровой стороны головного мозга пациентов, перенесших острую церебральную недостаточность (инсульт, травму).The invention relates to the field of medicine, namely, it is intended to assess the individual response of motor neurons of the affected and healthy side of the brain of patients after acute cerebral insufficiency (stroke, trauma).
MMSE (5).Earlier, when conducting studies to evaluate the effectiveness of drugs in rehabilitation after a stroke, a rather large number of different scales were used, mainly scales for scoring. Assessment on scales was carried out directly by neurologists / neuropsychologists during examination of the patient. The most commonly used scales are the NIH-NINDS scale (1), the Barthel index (2), the Rankin scale (3), the Ashfort scale (4), the MMSE scale (5).
(6, 7, 8, 9).It is also known to use for assessing the functional state of the brain and its response to various effects of electroencephalography and functional magnetic resonance imaging (6, 7, 8, 9).
(10).There is also a method for evaluating the cardioprotective effect of pharmacological agents according to the results of measuring left ventricular pressure during resistance loading in rats with modeling of hypertension, in which, as a criterion for evaluating the protective effect of drugs, the results of the percentage ratio between the left ventricular pressure on the 25th and 5th seconds of resistance load (10).
This method is simple and effective for assessing the cardioprotective effect of drugs, however, it is applicable only when conducting experimental pharmacological studies in the field of cardiology.
The objective of the invention is to provide an effective method for assessing the individual sensitivity of the patient's brain to the effects of chemical (pharmacological), physical and psychological factors.
The technical result that will be achieved from the use of this invention is to increase the effectiveness of assessing individual sensitivity to the effects of chemical, physical and psychological factors on the patient's brain.
The technical result is achieved by assessing individual sensitivity to the effects of chemical (pharmacological), physical and psychological factors on the patient’s brain by determining the functional state of neurons in the motor zones using transcranial magnetic brain stimulation (NBS): determining the parameters of the motor response (latency and amplitude), a comparison of these parameters with the original after a single treatment exposure and, the full course of treatment. With a reduction in latency time of 0.4 ms or more, and / or an increase in amplitude of 0.5 mV or more, the factor is regarded as capable of affecting the functional state of motor neurons, and the therapeutic effect is effective for the rehabilitation of this particular patient.
The essence of the invention lies in the fact that to evaluate individual sensitivity to the effects on the brain of various factors, the method of transcranial magnetic stimulation of the brain is first used.
The determination of such evaluated parameters as the latency and amplitude of the evoked motor response, and their comparison with the initial one after a single treatment exposure and after a full course of treatment, is safe for the patient and allows highly accurate assessment of the effects of various physical (magnetic stimulation, rehabilitation measures, etc. .), pharmacological (drugs of neuroprotective, muscle relaxant, and other actions) and psychological (verbal stimulation, speech / commands, neuropsychological understanding) factors on the human brain.
The indicated changes in latency (a decrease of 0.4 ms or more) and amplitudes (an increase of 0.5 mV or more) are based on data obtained experimentally in a series of studies. Statistical analysis of the data obtained by us when examining more than 40 patients according to our method, allowed us to conclude that these parameters are sensitive and allow even slight changes in the functional state of motor neurons to be recorded when at least one of them is changed.
From the analysis of scientific, technical and patent literature of methods for assessing individual sensitivity to the effects of chemical (pharmacological), physical and psychological factors on the patient’s brain by assessing the functional state of motor zone neurons using navigation transcranial magnetic brain stimulation (NBS), during which determination is made parameters of the motor response: latency and amplitude, comparison of these parameters after a single treatment exposure and the full course of treatment, p Allowing with high reliability and safety for the patient to evaluate the effectiveness of the therapeutic effect for the rehabilitation of this particular patient, we have not identified, which allows us to conclude that the proposed solutions meet the criteria of "novelty" and "inventive step". The invention is as follows:
Stage 1 - Before the treatment, a complete mapping of the patient’s cerebral cortex is performed using the NBS Nexstim navigation transcranial brain stimulation system, the point of the best motor response is found (a zone of no more than 2 mm 2 in which the evoked motor response (WMO) of the highest intensity was obtained At this point, the control WMO test parameters (latency and amplitude) are determined by deriving the average of these values obtained by producing 10 stimuli with an interval of 5-10 s. values obtained are the amplitude and latency. taken as baseline.
Stage 2 - A single treatment session is carried out directly in the clinic. The interval between exposure and the implementation of stimulation varies; it depends on the pharmacokinetics of each specific drug (increase in plasma concentration) or the timing when the maximum effect of non-drug exposure should be expected (it can be from several minutes to several hours). Make a conclusion about the presence or absence of the effect of a single dose of the drug or one session of non-drug treatment. At this stage, any change in the studied parameters in a positive direction is regarded as the presence of an effect.
Stage 3 - A full course of treatment is carried out (within 3-5 days), the control parameters of WMO are again evaluated. If they change in the positive direction by values equal to or greater than: a decrease in latency of 0.4 ms or more or an increase in amplitude of 0.5 mV or more, a conclusion is drawn about the positive effect exerted by this course of therapeutic effect, and this therapeutic effect is recommended for further use in a complex of rehabilitation measures for this patient.
Estimated parameters of navigational brain stimulation (NBS) (indicators of evoked motor response (WMO) at the point of assessment) and their changes, regarded as a positive effect, are given in the table.
1.Example 1
Patient G, who suffered an ischemic stroke and has motor impairments, underwent a study of the individual sensitivity of motor neurons to the effects of the widely used neurometabolic drug Cp. Initial WMO parameters: amplitude 0.18 mV, latency 24.1 ms. After a single dose of a standard dose of drug C, the dropwise amplitude was 0.27 mV, and latency 23.6 ms. Given the positive dynamics of the parameters, a preliminary conclusion was made about the presence of the effect, the patient underwent a full course of the specified drug in the dose prescribed by the manufacturer of this drug. After completing the course, the amplitude was 0.33 mV, latency -23.0 ms. Given a stable decrease in latency time by an amount greater than 0.4 ms (in this example, latency decreased by 1.1 ms), it was concluded that this drug has a positive effect on the functional state of motor neurons in this patient and can be recommended for use in complex rehabilitation measures.
2.Example 2
Patient G, who suffered an ischemic stroke and has motor impairments, underwent a study of the individual sensitivity of motor neurons to the effects of the widely used neurometabolic drug C 2 . Initial WMO parameters: amplitude 0.14 mV, latency 37.3 ms. After a single dose of a standard dose of C 2, an intravenous droplet amplitude of 0.13 mV, latency of 37.0 ms. Given the dynamics of the parameters, a preliminary conclusion was made about the presumed presence of the effect, the patient underwent a full course of the indicated drug in the dose prescribed by the manufacturer. After completion of the course, the amplitude is 0.13 mV, the latency is 37.7 ms. Considering the insignificant dynamics of the amplitude and the negative dynamics of latency, it was concluded that the patient is individually insensitive to this drug, the drug was not recommended for use during rehabilitation measures of this patient.
