Ускорению данного процесса способствует ряд факторов, главным из которых является проведение эпидуральной электростимуляции спинного мозга на уровне повреждения. При осуществлении последней происходит активация не только вновь образующихся мотонейронов, но и сохранившихся после травмы. Причем последние продолжают выделять CNTF, который стимулирует процессы роста и созревания вновь образующихся, а также трофику поврежденных мотонейронов. Аналогично на 7-е сутки после начала проведения эпидуралыюй электростимуляции спинного мозга определяют количественное знамени уровня CNTF - Ртек. и сравнивают его с исходным значением. При Рисх.<Ртек. судят о повышении активации функционального состояния спинальных мотонейронов, что свидетельствует об эффективности проводимой электростимуляции. В зависимости от этого принимают решение о необходимости продолжения электростимуляции спинного мозга. Пациент П., 45 лет, поступил в нейрохирургическое отделение одного из лечебно-профилактических учреждений на 2-м часу с момента получения травмы с диагнозом: «Закрытая осложненная травма верхнешейного отдела позвоночника. Нестабильный перелом зуба С2 позвонка, II тип по Anderson D'Alonso. Ушиб спинного мозга средней степени тяжести с частичным нарушение проводимости и формированием центромедуллярного синдрома». В неврологическом статусе были выявлены: парез обеих кистей до 3-х баллов, нижний вялый парапарез до 4-х баллов, нарушение чувствительности по сегментарно-диссоциированному типу с обеих сторон, нарушение тазовых органов по центральному типу. Осуществили забор 5 мл крови из периферической вены, получили сыворотку крови путем центрифугирования и осуществили иммуноферментный анализ полученной сыворотки крови. При этом определили количественное значение уровня содержания CNTF в сыворотке крови, которое составило 8,75 пг/мл. Для дальнейшего лечения после стабилизации витальных функций пациент был переведен в нейрохирургическое отделение для осуществления ранних реабилитационно-восстановительных мероприятий. Эпидуральную электростимуляцию спинного мозга осуществляли аппаратом «Нейрон-02» со следующими параметрами: частота - 100 Гц, длительность - 0,2 мс, амплитуда - 4-6 В в зависимости от ощущений больного, длительностью 40 мин ежедневно. На 7-е сутки с момента начала эпидуральной электростимуляции спинного мозга пациенту повторно осуществили забор крови из периферической вены для выполнения иммуноферментиого анализа. Получили сыворотку крови и определили количественное значение уровня содержания CNTF, которое составило Ртек.=16,47 пг/мл. Сравнили полученный результат с исходным значением: Рисх.<Ртек.. С учетом полученных данных судили о повышении активации функционального состояния спинальных мотонейронов, что свидетельствовало об эффективности проводимой электростимуляции.
