Известен способ пункционного лечения непаразитарной кисты печени с аспирацией ее содержимого и проведением склерозирующей терапии (М.Н. Новикова Чрескожные прицельные диагностические и лечебные пункции очаговых образований печени под контролем эхографии: Автореф. дисс. канд. мед. наук - Киев, 1986, 21 с.), который заключается в выполнении пункции полости кисты под контролем УЗИ, удалением ее содержимого, введении в остаточную полость склерозирующего раствора - 96% этилового спирта, в объеме до 1/3 эвакуированного объема. Время воздействия склерозанта составляет 5-10 мин, затем склерозирующий раствор аспирируют. Известен способ лапароскопического лечения непаразитарных кист печени (пат. RU 2402982 10.11.2010). Способ заключается в пункции кисты, аспирации ее содержимого, деэпителизации стенок кисты, иссечении выступающих стенок. Деэпителизацию стенок кисты проводят плазменным потоком при силе тока 15-30 А, расходе рабочего газа аргона от 0,8 до 1,5 л/мин, с расстояния 5-10 мм от стенки кисты в течение 1-3 минут, до появления белесоватого цвета внутренней стенки кисты. Иссечение выступающей стенки кисты проводят плазменным скальпелем с силой тока 20-40 А, при расходе рабочего газа аргона 1-2,5 л/мин, с расстояния 2-3 мм. Способ осуществляется следующим образом. В условиях операционной после обработки операционного поля выполняют местную инфильтрационную анестезию раствором новокаина 0,5% - 40 мл; разрез кожи выполняют в месте наиболее оптимального акустического доступа: эпигастрий - при локализации кисты во 2-3 сегментах печени, правое подреберье - при локализации кисты в 4-5-частично в 6 сегментах печени, или 7-8-9 межреберья - при локализации кисты в 6-7 сегментах печени. Под контролем УЗИ вводится игольчатый электрод для проведения радиочастотной деструкции с максимальной рабочей поверхностью до 4 см. Электрод позиционируют в полости кисты. Затем производят пункцию кисты пункционной иглой с внутренним диаметром 18G. По игле полностью аспирируют содержимое кисты, отправляют на экспресс-цитологическое и биохимическое исследование. Пункционную иглу извлекают. Выполняют сеанс радиочастотной деструкции полости кисты - включают генератор токов высокой частоты в минимальном режиме - происходит прогревание эпителиальной выстилки полости кисты до 45°-55°С. В дальнейшем электрод пошагово с длиной шага до 1,5 см извлекают, по мере извлечения происходит коагуляция пункционного канала, что позволяет предотвратить внутрибрюшное кровотечение и кровотечения в полость кисты. Клинический пример. Больной Л., 57 лет, история болезни N 9859, поступил 19.08.2009 г. в плановом порядке с диагнозом киста печени. По данным УЗИ от 20.08.2009 г. в задне-диафрагмальных отделах правой доли печени лоцируется анэхогенное образование - непаразитарная киста размерами 88×70 мм. Больной подготовлен к малоинвазивному лечению непаразитарной кисты печени. 20.08.2009 г. под местной анестезией, под контролем УЗИ в полость кисты установлен игольчатый электрод с рабочей поверхностью 3 см. Из разреза 5 мм иглой 18 G выполнена пункция полости кисты, по игле удалено до 60 мл прозрачной жидкости. Жидкость направлена на цитологическое исследование. Игла извлечена. Под нейролептаналгезией произведен сеанс радиочастотной обработки полости кисты. После вмешательства состояние больного стабильное, болей нет. При контрольном УЗИ от 25.08.2009 г.: в заднее-диафрагмальных отделах правой доли печени на месте кисты зона пониженной эхогенности 54×36 мм. В анализах крови патологии нет. Выписан в удовлетворительном состоянии 26.08.2009 г. При контрольном УЗИ через 12 месяцев после пункции в 7-8 сегментах правой доли печени, в проекции кисты лоцируется гиперэхогенное образование веретенообразной формы 38×14 мм. During the control ultrasound scan dated August 25, 2009: in the posterior-diaphragmatic parts of the right lobe of the liver at the site of the cyst, a zone of reduced echogenicity is 54 × 36 mm. In blood tests, there is no pathology. Discharged in satisfactory condition on August 26, 2009. With a control ultrasound 12 months after a puncture in 7-8 segments of the right lobe of the liver, a hyperechoic formation of a spindle-shaped 38 × 14 mm is located in the projection of the cyst. По описанной методике пролечено 27 больных с непаразитарными кистами с хорошими непосредственным и отдаленными результатами. Способ легко выполним, малотравматичен.
