В качестве дренажей используют катетеры для катетеризации центральных вен, которые вводят в брюшную полость по методике Сельдингера. Использование данной методики дренирования под контролем (УЗИ) позволяет безопасно выполнять вмешательство, в том числе при небольшом количестве жидкости по правому боковому каналу (0,5-1 см при УЗИ). Полученный темный геморрагический экссудат, подтверждающий диагноз, отправляют на бактериологическое и биохимическое исследования. После эвакуации выпота из брюшной полости через поставленный катетер начинают струйное введение в свободную брюшную полость теплого физиологического раствора в объеме 2000-2500 мл, что является безопасной и контролируемой манипуляцией.Under local anesthesia, thin-lumen drainage of the abdominal cavity is performed under the supervision of an ultrasound scan. Пример. Больной Г., 45 лет поступил в клинику общей хирургии в экстренном порядке в тяжелом состоянии. На основании клинических, инструментальных, лабораторных данных был поставлен диагноз острого панкреатита тяжелого течения. Пациент был госпитализирован в отделение реанимации и интенсивной терапии, где начал получать интенсивное комплексное лечение в полном объеме, в том числе, блокаду секреторной функции поджелудочной железы, дезинтоксикационную терапию, коррекцию гиповолемии, нарушений микроциркуляции, кислотно-основного состояния, профилактику гнойных осложнений, нутритивно-метаболическую поддержку. Несмотря на проводимое лечение на фоне эндогенной интоксикации отмечалось прогрессирование полиорганной недостаточности, при УЗИ брюшной полости отмечалось нарастание свободной жидкости (до 1 см по боковым каналам), в связи с чем были поставлены показания к санации и дренированию брюшной полости. Учитывая тяжесть состояния больного, были выполнены под контролем УЗИ чрескожно санация и дренирование по предлагаемому способу. В правом подреберье по I.axillaries anterior чрескожно под контролем УЗИ был поставлен тонкопросветный катетер в брюшную полость - выделилась темно-коричневого цвета жидкость в объеме около 150,0 мл. Через поставленный дренаж в брюшную полость введено 2000,0 мл теплого физиологического раствора. После данной инфузии при УЗИ в брюшной полости визуализировались полосы жидкости по боковым каналам толщиной до 3-х см. Увеличение количества свободной жидкости в брюшной полости позволило выполнить дополнительное дренирование: в левой подвздошной области через 5 мм троакар под контролем УЗИ установлен широкопросветный дренаж в малый таз. Через установленный дренаж произведена полная эвакуация введенного ранее раствора с примесью панкреатического экссудата (что подтверждено при УЗИ). В последующем был налажен фракционный перитонеальный лаваж физиологическим раствором в объеме 1500,0 мл, 5 раз день.Example. Given the severity of the patient’s condition, transdermally sanitation and drainage were performed under the supervision of an ultrasound in accordance with the proposed method. In the right hypochondrium according to I.axillaries anterior, a thin lumen catheter was placed percutaneously under ultrasound control in the abdominal cavity - a dark brown liquid was released in a volume of about 150.0 ml. Through the delivered drainage, 2000.0 ml of warm saline was introduced into the abdominal cavity. After this infusion, ultrasound in the abdominal cavity visualized fluid bands along the lateral canals up to 3 cm thick. An increase in the amount of free fluid in the abdominal cavity allowed for additional drainage: in the left iliac region, a 5-mm wide pelvic drainage was installed under the ultrasound control in the left iliac region . Through the installed drainage, the previously injected solution with an admixture of pancreatic exudate was completely evacuated (which was confirmed by ultrasound). Subsequently, fractional peritoneal lavage was adjusted with physiological saline in a volume of 1500.0 ml, 5 times a day.
На фоне проводимого интенсивного лечения, перитонеального лаважа в течение 4 суток отмечалась положительная динамика в виде купирования симптомов интоксикации, явлений почечной, энтеральной недостаточности, относительной нормализации лабораторных показателей. На пятые сутки с момента дренирования брюшной полости дренажи были удалены, при УЗИ в динамике свободная жидкость не определялась.
