Известен единственный способ оценки состояния реставраций зубов на основании клинических критериев, разработанных G.Ryge (1980), включающих определение цветовых характеристик пломб, их краевой адаптации, анатомической формы, наличия вторичного кариеса и тестовую систему их оценки, выраженную в буквенном эквиваленте: A (Alfa), В (Bravo), С (Charlie), D (Delta), H (Hotel), О (Oscar) по каждому из критериев. Способ осуществляют следующим образом. Клиническую объективную оценку степени износа проводят в разные сроки динамического наблюдения после терапевтического лечения с использованием стоматологического зеркала и инструментально с помощью стоматологического зонда путем скольжения острия инструмента перпендикулярно поверхности пломбы. Обращают внимание на цвет, форму реставрированных участков и клинических коронок зубов, состояние пломб и твердых тканей зуба, наличие в области границы «полость-пломба» ретенционных пунктов или щелей, проникновения и задержки в них зонда, с последующей необходимостью приложения усилий для его извлечения. Отмечают наличие меловидной окраски (очагов деминерализации) краев полости, размягченного дентина, сколов части пломб и их подвижности, отломов стенок зубов. В соответствии с таблицей 1 фиксируют параметры состояния зубной пломбы: соответствие цвета тканям зуба, поверхность реставрации, ее конфигурация, краевое прилегание, изменение цвета между пломбой и стенкой зуба, наличие вторичного кариеса, а также субъективное мнение пациента об исследуемой пломбе. Каждый последующий параметр состояния реставрации и критерий его оценки вторичен от предыдущего по клинической значимости и расположен в возрастающем порядке в зависимости от повышения уровня дефектности зубной пломбы, за исключением первых критериев оценки каждого из параметров, которым присвоен 1 балл, соответствующий идеальному состоянию пломбы для данного параметра.The method is as follows. In accordance with table 1, the parameters of the dental fillings state are recorded: the color of the tooth tissues, the restoration surface, its configuration, marginal fit, the color change between the fillings and the tooth wall, the presence of secondary caries, as well as the patient's subjective opinion about the test fillings. Each subsequent parameter of the state of restoration and the criterion for evaluating it is secondary from the previous one in clinical importance and is arranged in increasing order depending on the increase in the level of defectiveness of the dental filling, with the exception of the first criteria for evaluating each of the parameters that are assigned 1 point corresponding to the ideal state of the filling for this parameter .

Пломбировочный материал вносили в полость порциями толщиной не более 2 мм и полимеризовали галогеновой лампой в течение 40 сек. Окончательным этапом пломбирования являлось шлифование и полирование реставраций. При использовании в качестве пломбировочного материала композитов светового отверждения состояние реставраций в течение 12 месяцев динамического наблюдения во всех клинических случаях оставалось удовлетворительным. В ходе дальнейшего обследования удельный вес пломб, не требующих каких-либо терапевтических вмешательств, постепенно уменьшался и достигал к 60 месяцам наблюдений 53%. Замена части пломбы с целью устранения незначительных дефектов в первой исследуемой группе к 24 месяцам была показана в 9% случаев. В ходе динамического наблюдения количество данных реставраций увеличивалось и к 60 месяцам достигало 34,4%. В полной замене с ревизией кариозной полости нуждалось 6,2% реставраций, выполненных фотокомпозитами, к 48 месяцам клинических наблюдений, а к 60 месяцам количество пломб, степень износа которых соответствовала данному диапазону, увеличилось до 8,4%. Ортопедическое лечение было рекомендовано пациентам в первой исследуемой группе в 6,2% клинических случаев.When light curing composites were used as filling material, the state of restorations during 12 months of dynamic observation in all clinical cases remained satisfactory. Orthopedic treatment was recommended to patients in the first study group in 6.2% of clinical cases.
При применении компомеров с целью лечения неосложненных и осложненных форм кариеса процентное количество пломб, не требующих каких-либо терапевтических вмешательств, в среднем было выше, чем в первой группе и через 60 месяцев динамических наблюдений достигало 58,1%. В течение всего периода обследований количество реставраций, нуждающихся в частичной замене, составляло от 9,7 до 29%. Замена всей пломбы с ревизией полости была показана во второй исследуемой группе в 129% клинических случаев, что превышает аналогичный показатель в первой группе.
