Термин «патологическая извитость» является обобщающим названием патологического изгиба сосуда. Известные в литературе методы реконструкций деформации сонных артерий, в основном, имеют место при наличии изолированной ПИВСА. Однако нередко имеется сочетание атеросклеротического процесса каротидной зоны и ПИВСА. Предлагаемый способ осуществляется следующим образом. Surg. 1970; 120: 275] производят пластику каротидного бассейна: проксимальный 4 и дистальный 5 отрезки ОСА сегментарно иссекают (резецируют) со стороны ВСА до восстановления ее прямолинейного хода (фиг.2). Контрольную группу составили 20 пациентов, которым выполнялись изолированные эверсионные КЭАЭ путем отсечения устья ВСА от ОСА. Проводилась эверсия атеросклеротической бляшки из ВСА и, в дальнейшем, эндартерэктомия из ПСА и ОСА. Устранение патологической извитости ВСА достигалось путем резекции избытка и подтягивания ВСА к ОСА. Устранение избытка (патологической извитости) ВСА достигалось путем подтягивания дистальной и проксимальной частей ОСА со стороны полуокружности ВСА и сегментарной (краевой) их резекции с дальнейшим сопоставлением и наложением анастомоза на эти отрезки по типу «конец в конец».The main group consisted of 17 patients who underwent eversion CEAE by crossing the OSA below the bifurcation (according to De Bakey). Результаты: Для оценки состояния каротидных сосудов проведены поэтапные исследования в различные сроки (от 2 дней до 12 месяцев) после оперативного лечения в основной и контрольной группах с помощью ультразвукового дуплексного сканирования (изучались форма оперированной артерии, состояние анастомоза, наличие градиентов, турбуленции кровотока), транскраниальной допплерографии (изучался регионарный мозговой кровоток), а также КТ-ангиографии (определялось восстановление прямолинейного хода ВСА). Сосудисто-мозговых осложнений не отмечено.
