В качестве прототипа выбран способ, заключающийся в имплантации экспандера, наполнении его с растяжением мягких тканей, последующей хирургической тренировке дистрагированного лоскута этапным рассечением тканей подлиннику будущего лоскута, выкраивании и перемещении растянутого лоскута на дефект после заживления раны (Ваганова Н.А., Сарыгин П.В. Поставленная задача решается за счет того, что в способе, включающем имплантацию экспандера, проведении этапной дерматензии, на этапе растяжения тканей одновременно с увеличением объема экспандера проводят дискретное, пролонгированное и возрастающее во времени пережатие основания купола дистрагируемых тканей.The problem is solved due to the fact that in a method involving implantation of an expander, conducting stage dermatensia, at the stage of tissue stretching, simultaneously with an increase in the volume of the expander, a discrete, prolonged and time-increasing compression of the base of the dome of the tissue to be distributed is performed.
Способ осуществляют следующим образом: производят разрез кожи 5-10 мм по линии оси имплантируемого экспандера, отступя на 3-4 см от предполагаемой зоны основания экспандера. Производят отслойку мягких тканей на глубину, соответствующую зоне имплантации с образованием ложа для экспандера. Производят имплантацию экспандера, вводят начальный объем физиологического раствора с целью гемостаза полости ложа и расправления эндоэкспандера и ушивают рану вокруг порта. Через трое-четверо суток начинают плановую дерматензию в режиме постоянного введения раствора. С учетом наличия различных типоразмеров эндоэкспандеров скорость введения жидкости подбирают в зависимости от размера основания и заданного объема экспандера. В большинстве случаев растяжение неповрежденных кожных покровов производят в непосредственной близости от дефекта, при этом венозный отток в сторону последнего ограничен наличием рубцовых тканей. Затем в течение двух недель производят пережатие основания купола дистрагируемых тканей для формирования направленного венозного оттока. На последнем этапе лечения производят удаление экспандера с формированием экстензионного лоскута и перемещение последнего в область рубцового дефекта.The method is as follows: make a skin incision of 5-10 mm along the axis of the implantable expander, departing 3-4 cm from the proposed zone of the base of the expander. At the last stage of treatment, the expander is removed with the formation of an extensional flap and the latter is moved to the area of the scar defect.
Клинический пример: Б-й С., 24 года (и.б. №271143), поступил в стационар с последствием отморожения правой кисти. При поступлении имеются культи II-V пальцев на уровне пястно-фаланговых суставов. Торцевая поверхность культей представлена пересаженным ранее свободным кожным трансплантатом и склонна к изъязвлению. В проекции тыла кисти произведена гидропрепаровка подкожно-жирового слоя раствором новокаина 0,5% - 20 мл. По локтевому краю кисти произведен разрез кожи 1,5 см. Края раны взяты на держалки. Тупым и острым путем произведена отсепаровка кожно-жирового лоскута с созданием ложа для экспандера размерами 90×40 мм. В полость введен экспандер с площадью основания 80×30 мм. Рана вокруг порта имплантированного экспандера ушита тремя узловыми швами. В экспандер введено 20 мл физиологического раствора. Через 3 суток после имплантации начата дерматензия в постоянном режиме нагнетания жидкости с использованием портативного инфузомата. Через 7 суток плановой дерматензии параллельно оси конечности начато пережатие основания купола дистрагируемых тканей. Пережатие производилось в течение двух недель три раза в сутки с увеличением во времени от 45 минут до 3 часов.Clinical example: B. S., 24 years old (ib.
