Известен способ хирургического доступа к внутримозговой опухоли, расположенной в функционально-значимой зоне головного мозга, при котором осуществляют транскортикальный подход путем рассечения коры непосредственно над опухолью с последующей ее резекцией (Yasargil М.С. Microneurosurgery, Vol IV В. Способ осуществляется следующим образом. После краниотомии над областью мозга, пораженной бластоматозным процессом, рассекают твердую мозговую оболочку и по совокупным данным КТ, МРТ, ЭЭГ, интраоперационной УЗИ и морфометрии определяют расположение опухоли, локализацию и границы функционально-значимых зон. Выделяют борозду в проекции опухоли на границе функционально-значимой зоны длиной не менее 2 см и глубиной не менее 1,5 см. Мягкую мозговую оболочку рассекают и отодвигают стенку борозды до обнажения ее дна. Затем коагулируют и рассекают мягкую мозговую оболочку на дне борозды разрезом длиной 2-3 см и обнажают опухоль. Новообразование поэтапно фрагментированием удаляют. Послойно ушивают оболочки мозга, фиксируют костный лоскут и мягкие ткани.The method is as follows. Заявляемый способ разработан и прошел клинические испытания в РНХИ им. проф. А.Л.Поленова при лечении 5 больных с внутримозговыми опухолями больших полушарий мозга, расположенными в функционально-значимых зонах (2 больных с опухолями левой лобной доли, 2 пациента с новообразованиями левой теменной и лобной долей, 1 больной с опухолью правой лобной и теменной долей). В результате лечения сократилось число послеоперационных осложнений, реже отмечался неврологический дефицит в послеоперационном периоде, уменьшилась продолжительность восстановительного периода пациентов.The inventive method is developed and passed clinical trials at the Russian National Scientific Research Institute named after prof. Больной Я., 5 лет, ИБ №2258, находился на лечении с диагнозом: Астроцитома правой теменной и лобной долей. Проведена операция: костно-пластическая трепанация в правой лобно-теменной области, удаление опухоли теменной и лобной долей. После краниотомии и рассечения твердой мозговой оболочки выявлено, что поверхность передних отделов деформированной моторной коры на участке размерами 2,5×3,5 см имеет серовато-синюшный цвет, гладкую поверхность. При помощи ультразвуковой навигации определена опухоль размерами 3,5×4,0 см, расположенная под моторной корой, не доходя до средней линии на 1 см. Параллельно переднему краю моторной коры и 1,0 см кпереди, по краю патологически измененной ткани мягкая мозговая оболочка на протяжении 2,5 см рассечена. Выделена борозда длиной 23 мм, глубиной 16 мм. Патологическая ткань отодвинута дорсально. Бластоматозная ткань расположена субкортикально (1,5 мм). Опухоль удалена внепроекционным доступом ультразвуковым дезинтегратором по границе с малоизмененными тканями. Поверхность мозга приобрела более розовый цвет, венозное полнокровие регрессировало. Твердая мозговая оболочка ушита наглухо. Послойные швы на рану.Patient I., 5 years old, IB No. 2258, was treated with a diagnosis of Astrocytoma of the right parietal and frontal lobe. Parallel to the front edge of the motor cortex and 1.0 cm anteriorly, along the edge of the pathologically altered tissue, the pia mater over 2.5 cm dissected. A furrow 23 mm long and 16 mm deep was highlighted. The pathological tissue is moved dorsally. Blastomatous tissue is located subcortical (1.5 mm). The tumor was removed by out-of-projection access with an ultrasonic disintegrator along the border with little-changed tissues. The surface of the brain became more pink, venous plethora regressed. The dura mater is sutured tightly. Layered seams on the wound.
Течение ближайшего послеоперационного периода гладкое. Рана зажила первичным натяжением, швы сняты на 10 сутки. В течение первых 3 дней после операции отмечался парез левой кисти, который постепенно регрессировал. Больной выписан для прохождения лучевой терапии.The course of the immediate postoperative period is smooth. The wound healed by first intention, the sutures were removed on the 10th day.
