Abstract
Description
Изобретение относится к области медицины, конкретно к хирургическому лечению глубоких дефектов роговицы, явившихся следствием асептического кератита при лагофтальме. Суть метода заключается в применении лечебной послойной кератопластики аллотрансплантатом в сочетании с тектоническим покрытием роговицы и временной блефароррафией. Приводятся результаты лечения 7 пациентов, прооперированных по описанной методике.The invention relates to medicine, specifically to the surgical treatment of deep corneal defects resulting from aseptic keratitis in lagophthalmos. Основными причинами развития асептических (стерильных) язв роговицы являются: синдром «сухого глаза», нейротрофическая кератопатия, кератопатия из-за неполного смыкания век (лагофтальма), поражение роговицы при аутоиммунных заболеваниях (ревматоидном артрите и др.) [1]. Кератит вследствие несмыкания глазной щели (лагофтальма различной этиологии) является одним из клинических проявлений особо тяжелого роговично-конъюнктивального ксероза [2]. Однако при использовании данного способа наносится дополнительная травма конъюнктивы глаза, возможно скопление патогенной микрофлоры под конъюнктивальным лоскутом, отсутствуют стимулы к регенерации роговицы. Наиболее близким к предлагаемому решению является способ замещения послойных дефектов роговицы с использованием аллопланта для послойной типичной кератопластики (Вестник ОГУ, Специальный выпуск «Новые технологии микрохирургии глаза». - 2004. - С.88-90), заключающийся в следующем. В зоне поражения надсекаются трепаном и отсепаровываются поверхностные измененные слои роговицы. После этого выполняется круговой туннель в слоях роговицы глубиной 1,5-2 мм, в который заправляется шпателем аллоплант, превосходящий по диаметру трепан на 3 мм. Завершающим этапом является фиксация трансплантата П-образными перекидными швами [6]. Однако при использовании данного метода возможны технические сложности при диффузном истончении роговицы. Недостаточно надежен такой способ фиксации трансплантата в случаях состоявшейся перфорации роговицы и при сохраняющемся лагофтальме. After that, a circular tunnel is performed in the layers of the cornea with a depth of 1.5-2 mm, into which the alloplant is tucked, which is 3 mm larger in diameter than the trepan. The final stage is the fixation of the transplant with U-shaped flip seams [6]. However, when using this method, technical difficulties are possible with diffuse thinning of the cornea. This method of transplant fixation is not sufficiently reliable in cases of perforated cornea and with persistent lagophthalmos. Задачей изобретения является разработка клинически эффективного способа хирургического лечения глубоких дефектов роговицы, явившихся следствием асептического кератита при лагофтальме. Технический результат заявляемого изобретения заключается в том, что в случаях угрожающей или состоявшейся перфорации роговицы и при сохраняющемся лагофтальме надежность фиксации трансплантанта повышается за счет дополнительного тектонического покрытия роговицы и временной блефароррафии, к тому же расширяется спектр показаний к использованию метода в случаях диффузного истончения роговицы, перфорации роговицы, сохраняющемся лагофтальме.The objective of the invention is to develop a clinically effective method for the surgical treatment of deep corneal defects resulting from aseptic keratitis with lagophthalmos. Шаг 1 - Подготовка ложа для послойной кератопластики. Выполняется традиционно с помощью трепана и роговичных расслаивателей. Трансплантат фиксировался к ложу непрерывным либо отдельными узловыми швами 10:0 (Фиг.3.). Данный материал выполняет функцию субстрата для заполнения роговичного дефекта, однако известны и его недостатки (невозможность достижения конгруэнтного прилежания к поверхности роговицы обычными непрерывным или узловыми швами, частое прорезывание трансплантата швами). Для более прочной и надежной фиксации трансплантата мы использовали следующий шаг.Step 2 - Layer keratoplasty. Шаг 4 - Временная блефароррафия. Накладывалось 2-3 узловых шва 5:0, проведенных через хрящи век. При этом мы считаем наиболее целесообразным наложение швов на наружные 1/2-3/5 длины глазной щели, что позволяет осуществлять визуальный контроль за передним отрезком глаза (состояние трансплантатов, глубина передней камеры и т.д.) и беспрепятственно инстиллировать препараты в конъюнктивальную полость (Фиг.5). По описанной методике нами прооперированы 7 пациентов (5 женщин, 2 мужчины; возраст от 18 до 73 лет) с трофическим кератитом на фоне лагофтальма, обусловленного параличом лицевого нерва. Следует отметить, что все больные впервые осматривались нами с далеко зашедшими изменениями роговицы: десцеметоцеле - в 5 случаях; перфорация роговицы диаметром около 1 мм, прикрытая радужкой - в 1; обширная перфорация (расплавление) роговицы диаметром 7×3 мм - у 1 пациентки. Во всех случаях операции выполнялись в неотложном порядке. У 6 пациентов применялась местная анестезия с внутривенным потенцированием, в одном случае из-за тяжелого общего состояния пациентки потребовалось общее обезболивание. Послеоперационный период во всех случаях протекал гладко. Все пациенты получали антибиотики и кортикостероиды в виде парабульбарных инъекций; антибиотики, нестероидные противовоспалительные препараты и стимуляторы репарации роговицы эпибульбарно.According to the described technique, we operated on 7 patients (5 women, 2 men; age from 18 to 73 years) with trophic keratitis on the background of lagophthalmus due to facial paralysis. All patients received antibiotics and corticosteroids as parabulbar injections; antibiotics, non-steroidal anti-inflammatory drugs and corneal repair stimulants are epibulbar.

Во всех случаях удалось добиться формирования эпителизированного умеренно васкуляризированного помутнения роговицы (в одном случае - сращенного с радужкой). Толщина роговицы в зоне помутнения оценивалась с помощью пахиметрии у 3 пациентов и составила в среднем 386,3±17,3 мкм. Острота зрения составила: от 0,01 до 0,05 - у 3 человек, 0,06-0,1 - у 3, 0,2 - у одного пациента.In all cases, it was possible to achieve the formation of an epithelized moderately vascularized clouding of the cornea (in one case, fused with the iris). Заключение. Таким образом, предлагаемый способ лечебной послойной кератопластики в сочетании с тектоническим покрытием роговицы и временной блефароррафией позволяет сохранить глазное яблоко с остаточным или низким предметным зрением. К достоинствам метода, по нашему мнению, относятся техническая простота и возможность выполнения по неотложным показаниям, в том числе при отсутствии глазного банка тканей.Conclusion Thus, the proposed method of therapeutic layer-by-layer keratoplasty in combination with a tectonic coating of the cornea and temporary blepharorography allows you to save the eyeball with residual or low objective vision.
