Прототипом изобретения является способ стадирования рака in vitro по критерию N путем тщательного изучения удаленного макропрепарата, представляющего из себя клетчатку с лимфатическими узлами, путем их выделения скальпелем и ножницами [Летягин В.П. Сигнальный («сторожевой») лимфатический узел рака молочной железы // Вестник Московского онкологического общества, 2000-2003, с.16-17]. Выделить все лимфатические узлы, как правило, с помощью режущих инструментов не представляется возможным. Этому препятствует наличие жировых отложений. По критерию N стадия рака определяется как размерами лимфатических узлов, так и количеством. Поэтому количественная характеристика является очень важной для окончательного стадирования рака по критерию pN [TNM Атлас: Иллюстрированное руководство по TNM классификации злокачественных опухолей/ К. Указанный технический результат достигается тем, что в способе, включающем выделение лимфоузлов и лимфатических сосудов с последующим их морфологическим исследованием на наличие опухолевых клеток, в удаленный макропрепарат с жировой клетчаткой вводят физиологический раствор в количестве 10 мл на 20-30 мг жира, затем производят ультразвуковую кавитацию тканей, выделяют лимфоузлы и сосуды единым блоком.The specified technical result is achieved by the fact that in the method, including the allocation of lymph nodes and lymph vessels with their subsequent morphological examination for the presence of tumor cells, physiological saline is injected into a distant macro-preparation with fatty tissue in the amount of 10 ml per 20-30 mg of fat, then ultrasonic cavitation is performed tissues, lymph nodes and vessels are isolated in a single block.
Предлагаемый способ осуществляется следующим образом. После операции in vitro производят обработку удаленного макропрепарата с жировой клетчаткой. В клетчатку с помощью шприца вводят физиологический раствор из расчета 10 мл на 20-30 мг жира. Затем с помощью аппарата Soft Lipomodel, сертифицированного в EEC №0068/ETI-ОМ/057-99 для липосакции, производят ультразвуковую кавитацию жира, превращение его в эмульсию и удаление с помощью салфеток. Полная обработка препарата предусматривает полное удаление жира. Как правило, на эту процедуру затрачивается 30-40 мин. Принцип работы аппарата позволяет, не повреждая трубчатые структуры, лимфатические узлы, удалять жир, сохраняя всю архитектонику лимфатической системы, верифицируя 100% лимфатических узлов, даже размерами 5-7 мм. В дальнейшем все лимфатические узлы могут быть исследованы на предмет метастатического поражения, а капилляры - на предмет опухолевых эмболов, что повышает точность стадирования рака по критерию pN.The proposed method is as follows. Subsequently, all lymph nodes can be examined for metastatic lesions, and capillaries for tumor emboli, which increases the accuracy of staging of cancer according to the pN criterion.

Пример. Больной 3., 58 лет, оперирован в Башкирском клиническом онкологическом диспансере в феврале 2007 года по поводу метастазов меланомы в левую подмышечную область (первичная операция была год назад). Под общим обезболивание выполнена операция лимфаденэктомия, которая прошла без особенностей. При макроскопическом изучении удаленного препарата обнаружено два крупных лимфатических узла размерами до 3 см. Другие лимфатические узлы окутаны жиром и поэтому не пальпируются. В жировую клетчатку равномерно введено до 100 мл физиологического раствора. Включен аппарат, рабочей частью аппарата начата кавитация и удаление жировой ткани, которая переходила в эмульсию. Манипуляции проходили 25-30 мин. Был удален весь жир. Это позволило полностью выделить все лимфатические узлы, сосуды. Также появилась возможность верифицировать все узлы на предмет их метастатического поражения.Example.
